INTRODUCTION: Sleepiness is one of the cardinal complaints of patients in tertiary referral centres for sleep disorders. It is commonly associated with sleep disorders, but the additional influence of obesity remains unclear. We hypothesised that obesity may be an independent predictor of sleepiness in this population.
PATIENTS AND METHODS: We reviewed 335 patients undergoing overnight polysomnography over a 3 month period (May-July 2011), recording selected sleep parameters in addition to subjective sleepiness as measured by the Epworth Sleepiness Scale (ESS, 0-24 points).

Excessive daytime sleepiness is a common presentation to physicians both in general practice and hospital settings. In this review, we provide an update on the latest theories on the pathogenesis of the condition, and discuss the approach to investigation of the sleepy patient, with particular reference to narcolepsy. Recommended therapy is reviewed for both narcolepsy and cataplexy, to provide physicians with an important reference on the investigation and management of these troubling conditions.

Pupil diameter was measured in light and dark conditions every half hour for 6.5-10 h in 3 normal controls and 3 narcoleptics. Mean pupillary diameter was significantly smaller in the narcoleptic group than in the normal group. Pupil activity was correlated with pupil diameter only in the dark condition in narcoleptics.
