[Closure of soft tissue defects in bone infections using free and pedicled muscle flaps].

Over the period of three years 33 free latissimus dorsi-flaps and 23 pedicled myoplasties of various types for the coverage of large tissue and bone defects are performed. The operative technique of the free muscle transfer with microvascular anastomosis are described in detail. The postoperative management is outlined. The results of the free and pedicled muscle flaps are reported. In free transplantations the success-rate was 88% and in pedicled flaps 65% There is good evidence that bone consolidation and healing of the chronic bone infection can be improved by healthy vital soft tissue cove

An Unusual Suspect in a Case of Fungal Necrotizing Soft Tissue Infection.

Necrotizing soft tissue infection (NSTI) is a rapidly progressive and potentially fatal infection that is most commonly bacterial in origin but can also be fungal. We present the case of a 79-year-old woman who developed NSTI after gastrostomy tube (G-tube) manipulation. She presented to the emergency department (ED) with abdominal pain and inflammation, and imaging revealed the G-tube to be positioned within the soft tissues. The patient then underwent emergent surgical debridement while decompensating from septic shock. Tissue cultures identified an unusual combination of Pichia and Lactobac

Skin and soft tissue infection by Mycobacterium intracellulare in an immunocompetent patient.

Nontuberculous mycobacteria (NTM) infections still represent a large group of insidious diseases hard to deal with. Traditionally, immunocompromised patients suffer from NTM infections, especially with respiratory involvement or disseminated diseases due to MAC (Mycobacterium avium complex). Here we report a rare case of Mycobacterium intracellulare infection involving skin and soft tissue, manifested as a chronic cutaneous ulcer in an immunocompetent patient with several comorbidities, including seizures. Accurate diagnosis of species was obtained with in vitro culture and RT-PCR (Real Time-P

Coccidioides immitis soft tissue infection mimicking pseudofolliculitis barbae.

Endemic fungal infections can present atypically and should be considered in the differential diagnosis of any soft tissue infection not responding appropriately to antibiotic therapy. Diagnosis can be confirmed with a biopsy. Most fungal soft tissue infections require extended duration of treatment.

Aggressiveness - The key to a Successful Outcome in Necrotizing Soft Tissue Infection.

16 patients with necrotizing soft tissue infections were managed during last three years in various service hospitals. The experience indicates that there is considerable overlap in clinical findings and bacteriology. The infections seem to be variations of the same disease process, a spreading necrotizing infection. The number of these patients suggests that there is an increasing incidence of this entity. Staphylococcus and coliforms were the commonest organisms cultured in most of these patients. Because of the high mortality rate upto 50% as reported, we advocate aggressive and early treat

A Rare Case of Soft-Tissue Infection Caused by Raoultella planticola.

Raoultella species are Gram-negative, non-motile bacilli primarily considered to be environmental bacteria. Raoultella planticola is a rare cause of human infections. We report a case of serious soft-tissue infection in a young male tiler who presented with cellulitis of his left thumb. He had sustained a crush injury to his left thumb 10 days earlier in a soiled environment. He noted a minor break in the skin and he washed the wound out with running water. One week later, he experienced pain, erythema, and swelling of his thumb and attended his general practitioner who prescribed oral fluclox

Audit of diabetic soft tissue infection and foot disease in Accra.

BACKGROUND: Soft tissue infection and foot disease are well known complications among diabetes mellitus patients. With an increasing prevalence of diabetes mellitus in Africa, management of these complications is expected to become a major problem. OBJECTIVE: To audit the surgical management of diabetic soft tissue infection and foot disease over a two-year period in Accra, Ghana. METHODS: A prospective study of all patients admitted to the General Surgical Unit of the 37 Military Hospital between May 2005 and April 2007 was conducted. Diabetic patients with soft tissue infections and foot d

Effect of bedside ultrasound on management of pediatric soft-tissue infection.

BACKGROUND: Superficial soft-tissue infections (SSTI) are frequently managed in the emergency department (ED). Soft-tissue bedside ultrasound (BUS) for SSTI has not been specifically studied in the pediatric ED setting. OBJECTIVE: To evaluate the effect of a soft-tissue BUS evaluation on the clinical diagnosis and management of pediatric superficial soft-tissue infection. METHODS: We conducted a prospective observational study in two urban academic pediatric EDs. Eligible patients were aged < 18 years presenting with suspected SSTI. Before BUS, treating physicians were asked to assess the li

A Case of Skin and Soft Tissue Infection Caused by Mycobacterium abscessus.

Mycobacterium abscessus (M. abscessus) is an acid-fast bacillus that's classified as a pathogenic "rapid growing" nontuberculous mycobacteria. It is an uncommon cause of human disease, but it can cause skin and soft tissue infection after skin injury following inoculation, minor trauma and surgery. The single most important factor for determining the course and prognosis of a M. abscessus infection is the underlying immune status of the host. We report here on a 71-year-old female who presented with multiple painful erythematous cutaneous nodules on her left forearm. She had diabetes mellitus

Can paronychia cause a remote necrotizing soft tissue infection?

Remote necrotizing soft tissue infection (NSTI) resulting from paronychia is very unusual but potentially lethal. We report a case of a 39-year-old woman affected by this unusual infection. The paronychia completely resolved in less than 2 weeks, however, a NSTI involved the right chest and flank and the left thigh. The patient required intensive care and multiple surgical debridements. This recent experience and literature data suggest that paronychia can cause a remote NSTI that can rapidly spread and become life-threatening. Broad spectrum antibiotics and aggressive surgical debridement are

Alternaria alternata soft tissue infection in a forearm transplant recipient.

BACKGROUND: Composite tissue allograft recipients require intensive immunosuppression and are therefore at risk of infections. Filamentous fungal infections are among the complications most difficult to manage in organ transplant recipients. METHODS: Case report and literature review. CASE REPORT: Two years after bilateral forearm transplantation, a 35-year-old patient presented with a tumor-like lesion on his thigh after a penetrating injury caused by a sliver acquired during hiking. The lesion was excised completely, and microscopic examination revealed a filamentous fungal infection. Alte

[A report of two cases of cervical necrotizing soft-tissue infection].

Two recent cases of cervical necrotizing soft-tissue infection are herein presented. Case 1. A 52-year-old man with uncontrolled diabetes was hospitalized because of an erythematous swelling of the left side of his neck and high grade fever. Fetid yellowish pus exuded from the left parotid area. The swelling extended from the left temporal area to the left supraclavicular fossa, with necrosis of the parotid gland, sternocleidomastoid, masseter and a portion of the strap muscles. Wound cultures revealed Staphylococcus aureus and alpha-hemolytic streptococcus. No anaerobic bacteria were detected

Piperacillin/tazobactam in the treatment of serious acute soft tissue infection.

Twenty-five consecutive patients admitted to hospital with severe soft tissue infection were entered into an open trial designed to evaluate safety and efficacy of the drug combination tazobactam/piperacillin. The dosage regimen was 4 g piperacillin/500 mg tazobactam Q8H. There were twenty cases of uncomplicated cellulitis and five cases with associated abscess formation. These five cases required adjunctive surgical drainage. Mean duration of therapy was 7.6 days. No other antibiotics were administered unless treatment with the study drug failed. There were eight treatment failures, six relat

Skin and Soft Tissue Infections Clinical Pathway Concordance in Pediatric Emergency and Urgent Care Sites.

INTRODUCTION: Skin and soft tissue infections (SSTIs) are major drivers of antibiotic prescribing in pediatric emergency departments (EDs) and urgent care clinics (UCCs). Although guidelines recommend limited diagnostics and antibiotics, practice variation persists, and the influence of sociodemographic factors remains underexplored. We evaluated concordance with a pediatric SSTI clinical pathway (CP) and examined variation across sociodemographic factors. METHODS: This retrospective cohort study included children aged 6 months to 18 years evaluated for SSTIs at 2 EDs and 3 UCCs in a Midweste
