Understanding, Recognizing, and Managing Functional Speech Disorders: Current Thinking Illustrated With a Case Series.

PURPOSE: Functional speech disorders (FSDs), a subtype of functional neurological disorders, are distinguishable from neurogenic motor speech disorders based on their clinical features, clinical course, and response to treatment. However, their differential diagnosis and management can be challenging. FSDs are not well understood, but growing evidence suggests a biopsychosocial basis distinct from structural lesions that cause neurogenic motor speech disorders. METHOD AND RESULTS: Following an overview of FSDs, four patients are described to illustrate the range of clinical manifestations, bi…

Differentiation of Motor Speech Disorders through the Seven Deviance Scores from MonPaGe-2.0.s.

For the clinical assessment of motor speech disorders (MSDs) in French, the MonPaGe-2.0.s protocol has been shown to be sensitive enough to diagnose mild MSD based on a combination of acoustic and perceptive scores. Here, we go a step further by investigating whether these scores-which capture deviance on intelligibility, articulation, voice, speech rate, maximum phonation time, prosody, diadochokinetic rate-contribute to the differential diagnosis of MSDs. To this aim, we trained decision trees for two-class automatic classification of different pairs of MSD subtypes based on seven deviance s…

Expansion Points Intervention in Young Children with Speech Sound Disorders: A Multiple Baseline Design.

The purpose of this investigation was to evaluate the efficacy of expansion points (EXP) intervention with a modified criterion for preschool children with speech sound disorders (SSD). Three preschool-aged children were enrolled in a single-subject multiple baseline intervention study. Intervention took place over 16 sessions. Pre- and post-intervention data are provided. Three outcome measures (generalization to probe words and gains in percent consonants correct, PCC, in words and in conversation) were evaluated to measure the effectiveness of the EXP intervention. All three of the particip

Motor speech disorders in the nonfluent, semantic and logopenic variants of primary progressive aphasia.

OBJECTIVE: Motor speech disorders (MSDs) are characteristic for nonfluent primary progressive aphasia (nfvPPA). In primary progressive aphasia (PPA) of the semantic (svPPA) and of the logopenic type (lvPPA), speech motor function is considered typically intact. However, knowledge on the prevalence of MSDs in svPPA and lvPPA is mainly based on studies with a priori knowledge of PPA syndrome diagnosis. This fully blinded retrospective study aims to provide data on the prevalence of all types of MSDs in a large sample of German-speaking patients with different subtypes of PPA. METHOD: Two raters…

Changes of some functional speech disorders after surgical correction of skeletal anterior open bite.

Skeletal anterior open bite (AOB) or apertognathism is characterized by the absence of contact of the anterior teeth and affects articulation parameters, chewing, biting and voice quality. The treatment of AOB consists of orthognatic surgical procedures. The aim of this study was to evaluate the effects of treatment on voice quality, articulation and nasality in speech with respect to skeletal changes. The study was prospective; 15 patients with AOB were evaluated before and after surgery. Lateral cephalometric x-ray parameters (facial angle, interincisal distance, Wits appraisal) were measure

Functional MRI evidence for fine motor praxis dysfunction in children with persistent speech disorders.

UNLABELLED: Children with persistent speech disorders (PSD) often present with overt or subtle motor deficits; the possibility that speech disorders and motor deficits could arise from a shared neurological base is currently unknown. Functional MRI (fMRI) was used to examine the brain networks supporting fine motor praxis in children with PSD and without clinically identified fine motor deficits. METHODS: This case-control study included 12 children with PSD (mean age 7.42 years, four female) and 12 controls (mean age 7.44 years, four female). Children completed behavioral evaluations using s…

Reading skills of students with speech sound disorders at three stages of literacy development.

PURPOSE: The relationship between phonological awareness, overall language, vocabulary, and nonlinguistic cognitive skills to decoding and reading comprehension was examined for students at 3 stages of literacy development (i.e., early elementary school, middle school, and high school). Students with histories of speech sound disorders (SSD) with and without language impairment (LI) were compared to students without histories of SSD or LI (typical language; TL). METHOD: In a cross-sectional design, students ages 7;0 (years;months) to 17;9 completed tests that measured reading, language, and n

Sensory integration dysfunction affects efficacy of speech therapy on children with functional articulation disorders.

BACKGROUND: Articulation disorders in young children are due to defects occurring at a certain stage in sensory and motor development. Some children with functional articulation disorders may also have sensory integration dysfunction (SID). We hypothesized that speech therapy would be less efficacious in children with SID than in those without SID. Hence, the purpose of this study was to compare the efficacy of speech therapy in two groups of children with functional articulation disorders: those without and those with SID. METHOD: A total of 30 young children with functional articulation dis

Role of developmental stage of occlusion for articulatory disorders in speech among first-graders.

The purpose of this study was to examine the role of the eruption stage and that of occlusal anomalies among etiological factors for articulatory speech disorders in a series of first-graders. The results suggest that even though speech and sound production have been reported to be mature by the age of five, some spontaneous correction of speech sound articulation occurs with maturing of the articulators with age and with eruption of some permanent teeth during the first phase of the mixed dentition.

Acoustic recognition of voice disorders: a comparative study of running speech versus sustained vowels.

The signals of running speech and sustained vowels of normals and subjects suffering from dysphonia were analyzed statistically with respect to the signal-to-noise ratio (SNR). The distribution of the SNR measured in multiple overlapping frames in the speech signal was described by a linear combination of the distribution frequencies for SNR = 0 dB, 0 dB less than SNR less than 15 dB, and SNR greater than or equal to 15 dB. The values of the linear combination, the SNR of the vowels, and clinical assignment of the voices to normal and pathologic populations based on laryngoscopic and strobosco

Central auditory processing and interhemispheric integration: the relationship between speech sound coding disorder and articulation disorder.

This study aims to demonstrate that speech sound disorders associated with central auditory processing disorder stem from a processing disorder in the central auditory pathways during listening, rather than from an articulation-based disorder. Included 25 children with articulation problems (WAP; 7 females, 18 males) and 25 typically developing children without articulation problems (WoAP; 10 females, 15 males), aged 5-8 years. Assessments comprised the Hacettepe Articulation Test, Scan-C subtests (Filtered Words, Auditory Figure-Ground, Competing Words, Competing Sentences), and the Denver II

Differential Diagnosis of Speech Sound Disorder (Phonological Disorder): Audiological Assessment beyond the Pure-tone Audiogram.

BACKGROUND: According to the Diagnostic and Statistical Manual of Mental Disorders, Fifth Edition, diagnosis of speech sound disorder (SSD) requires a determination that it is not the result of other congenital or acquired conditions, including hearing loss or neurological conditions that may present with similar symptomatology. PURPOSE: To examine peripheral and central auditory function for the purpose of determining whether a peripheral or central auditory disorder was an underlying factor or contributed to the child's SSD. RESEARCH DESIGN: Central auditory processing disorder clinic pedi

The voice of emotional memory: content-filtered speech in panic disorder, social phobia, and major depressive disorder.

We asked patients with either panic disorder, social phobia, or major depressive disorder and healthy control participants to describe their most frightening experience and to describe an emotionally neutral experience. Both fear and neutral autobiographical memories were audiotaped and processed through a low-pass filter that eliminated frequencies above 400 Hz, thereby abolishing semantic content but leaving paralinguistic aspects like rate, pitch, and loudness intact, and these convey emotional cues. Raters blind to content and diagnosis rated the content-filtered speech clips on emotional

Children's Communication Rights in the Holistic Assessment of Speech Sound Disorders: Do Speech-Language Pathologists Use the International Classification of Functioning, Disability and Health Framework?

PURPOSE: During the assessment process, speech-language pathologists (SLPs) have the opportunity to account for the involvement, thoughts, and views of children with speech sound disorders (SSDs). This is in alignment with the Convention on the Rights of the Child (CRC; UNICEF, n.d.; United Nations, 1989) including CRC Article 12 (Respect for children's views), Article 13 (Sharing thoughts freely), and Article 23 (Children with disabilities), which affirms SLPs' role to facilitate participation and consider perspectives. Construction of an automatic score for the evaluation of speech disorders among patients treated for a cancer of the oral cavity or the oropharynx: The Carcinologic Speech Severity Index.

BACKGROUND: Speech disorders impact quality of life for patients treated with oral cavity and oropharynx cancers. However, there is a lack of uniform and applicable methods for measuring the impact on speech production after treatment in this tumor location. OBJECTIVE: The objective of this work is to (1) model an automatic severity index of speech applicable in clinical practice, that is equivalent or superior to a severity score obtained by human listeners, via several acoustics parameters extracted (a) directly from speech signal and (b) resulting from speech processing and (2) derive an a…

Diagnostic accuracy of parent reports for speech-language disorders in young children: a meta-analysis.

BACKGROUND: Speech and language disorders are among the most common developmental concerns in early childhood, yet there is limited consensus in pediatrics regarding how accurately parent-report instruments identify affected children. Prior reviews have combined heterogeneous screening formats, making it difficult to evaluate the performance of parent-completed measures widely used in developmental surveillance and primary care. METHODS: We conducted a systematic review and meta-analysis of studies evaluating the diagnostic accuracy of parent-report instruments for identifying speech and/or l
