Sphenoid Sinus Involvement in Chronic Rhinosinusitis Without Polyps.

BACKGROUND: Chronic rhinosinusitis (CRS) is a common chronic disease. It has 2 main clinical subtypes: CRS with nasal polyposis (CRSwNP) and without nasal polyps (CRSsNP). The sphenoid sinus appears to be less frequently involved in CRSsNP cases. Thus, we aimed to compare the incidence of sphenoid sinus involvement between CRSsNP and CRSwNP cases. METHODS: A retrospective chart review of CRS cases was performed. The clinical and imaging findings, including age, sex, adenoid, and inferior turbinate hypertrophy (ITH), deviation of the nasal septum (DNS), presence of polyps, Lund-McKay scores, a

Giant ethmoid mucocele presenting as a nasal region mass with intracranial extension, a case report.

Background: Mucocele is expansile slowly growing benign cystic lesion that occurs as a result of occlusion of the ostia of the paranasal sinuses. It mostly involves the frontal sinus followed by ethmoid and rarely sphenoid and maxillary sinuses. Case presentation: Authors present a rare case of giant ethmoid mucocele in 34-year-old adult male presenting as nasal region mass with intracranial extension. Conclusions: Authors highlight that giant ethmoid mucoceles are rare and may have intracranial or extracranial extension resulting in craniofacial disfigurement which should be considered in pat

The relationship of the natural sphenoid ostium to the skull base.

OBJECTIVES/HYPOTHESIS: Various intranasal landmarks have been described to aid in the localization of the natural sphenoid sinus ostium. The objectives of this study are to identify the ostium location relative to the skull base and assess the relationships between the location of the ostium and sphenoid disease or pneumatization pattern. DESIGN: Descriptive study. METHODS: Consecutive Xoran Mini-CAT (Ann Arbor, MI) scans of patients with no history of sinus surgery (n = 202) were evaluated. The natural sphenoid ostium was identified in axial, coronal, and sagittal planes. Distances from the

Branch retinal artery occlusions after balloon sinuplasty due to formation of a traumatic pseudoaneurysm in the internal carotid artery and embolization treatment.

PURPOSE: To describe a case of multifocal branch retinal artery occlusions due to a traumatic internal carotid artery pseudoaneurysm and endovascular treatment after a balloon sphenoid sinuplasty procedure. OBSERVATIONS: A 62-year-old female with no significant past medical history underwent an elective outpatient balloon sinuplasty of the right frontal sinus and bilateral sphenoid sinuses. Shortly after the balloon sinuplasty device was inflated in the left sphenoid sinus, acute, severe, pulsatile bleeding was encountered from the posterior wall of the sinus. The patient underwent emergent c

Extrasinonasal infiltrative process associated with a sinonasal fungus ball: does it mean invasive fungal sinusitis?

PURPOSE: Invasive fungal sinusitis (IFS) has rarely been reported to develop from non-IFS. The purpose of this study was to disclose the nature of the extrasinonasal infiltrative process in the presence of a sinonasal fungus ball (FB). METHODS: We retrospectively reviewed the medical records, computed tomography, magnetic resonance images of 13 patients with sinonasal FB and the extrasinonasal infiltrative process. Based on histology and clinical course, we divided the extrasinonasal infiltrative process into IFS and the nonfungal inflammatory/infectious process (NFIP). The images were analyz

The delta F508 mutation in cystic fibrosis and impact on sinus development.

BACKGROUND: Cystic fibrosis (CF) patients often have widespread inflammatory paranasal sinus disease with an increased incidence of frontal, maxillary, and sphenoid hypoplasia. The most common genetic defect in CF is the delta F508 mutation. The effect of specific CF genotypes on phenotypic sinus development and infections is not well understood. The purpose of this study was to determine whether the homozygous delta F508 mutation is associated with an increased incidence of sinus hypoplasia when compared with other mutations. METHODS: This study is a retrospective review of all adult patient

Site of origin of nasal polyps. Transcranially removed naso-ethmoidal blocks as a screening method for nasal polyps in autopsy material.

In autopsy material naso-ethmoidal blocks were removed in 19 patients. None of the patients had a preceding rhinoscopy. Before removal, the maxillary and sphenoidal sinuses were opened and screened for polyps. After removal the total naso-ethmoidal complex was carefully examined for nasal polyps. The site of origin of the individual polyp was registered and documented photographically. Nine polyps were found in five patients, of which four had no history of asthma, allergy or sinusitis. Two patients had unilateral solitary polyps, three had polyps bilaterally. All polyps were found in the meat

Orbital tuberculosis.

A 37-year-old man had epistaxis, headache, intermittent swelling of the periorbital region, proptosis, epiphora, and chronic sinusitis. X-ray films showed marked erosion of the midline facial bones, total opacification of both maxillary sinuses, and clouding of the sphenoid and left ethmoid sinuses. He also had a cutaneous-maxillary sinus fistula and a purulent discharge. A histopathologic examination disclosed areas of acute and chronic inflammation with occasional noncaseating granulomas. Biopsy specimens and postoperative sputum cultures grew Mycobacterium tuberculosis, although the patient

Morphometric examination of the paranasal sinuses and mastoid air cells using computed tomography.

BACKGROUND: The paranasal sinuses and mastoid air cells vary considerably in size and shape from person to person. The main structures are pneumatic. In this study, we investigated the relationship between right and left sides and evaluated the volume changes according to age and sex. METHODS: Of all patients attending the radiology department, 91 cases without paranasal sinuses and mastoid air cells pathology (i.e., inflammation, operation or trauma) were selected for evaluation. Axial computed tomography (CT) scans were obtained for both paranasal sinuses and temporal bones. In all scans, t

Endoscopic endonasal optic nerve decompression in children younger than 2 years old with congenital optic canal stenosis: illustrative cases.

BACKGROUND: Congenital optic canal stenosis causing compressive optic neuropathy is a rare disorder that presents unique diagnostic and treatment challenges. Endoscopic endonasal optic nerve decompression (EOND) has been described for optic nerve compression in adults and adolescents but has never been reported for young children without pneumatized sphenoid sinuses. The authors describe preoperative and intraoperative considerations for three patients younger than 2 years of age with congenital optic canal stenosis due to genetically confirmed osteopetrosis or chondrodysplasia. OBSERVATIONS:

Isolated oncocytic sinonasal papilloma of the sphenoidal sinus.

The ectodermally derived mucosa of the nose and paranasal sinuses may give rise to three morphologically different papillomas referred to individually as exophytic papilloma, inverted papilloma (IP) and oncocytic sinonasal papilloma (OSP). OSP is the rarest of the three different types, frequently affects the maxillary sinus, ethmoid sinus and nasal cavity while sphenoidal sinus involvement is exceedingly rare. The tumour has no gender predilection and has a wide age range with the majority above 50 years of age. Clinically, its behaviour parallels IP due to local recurrence and tendency to ma

Complicated allergic fungal rhinosinusitis: diagnostic complexities and management strategies.

A man in his 60s presented with diplopia, headache and facial numbness. Examination demonstrated a unilateral right abducens nerve palsy which rapidly progressed to a bilateral abducens palsy, partial right third nerve palsy and bilateral facial numbness. Nasendoscopy and imaging findings suggested an erosive fungal lesion centred on the sphenoid sinus. He underwent aggressive clearance with functional endoscopic sinus surgery, which revealed pan-sinus allergic fungal disease with marked bony erosion of the sphenoid, exposing the dura and optic chiasm. Corticosteroids and systemic antifungals

The Ethmomaxillary Sinus-A False Duplicate Maxillary Sinus.

Functional endoscopic sinus surgery consistently benefits from good anatomical knowledge and modern imaging techniques. Ethmoid air cells migrate in specific topographical sites near the ethmoid. Posterior ethmoid air cells that descend into the maxillary sinus (MS) are ethmomaxillary sinuses (EMSs) that into the superior nasal meatus. Few previous studies found EMSs in 0.68% to 16.48% of cases. An EMS differs from a Haller's infraorbital cell nearing the ethmoidal infundibulum. A posterior ethmoid air cell intercalated between the ethmoid, MS and sphenoidal sinus is a Sieur's cell, but it cou
