Economic considerations of the treatment of ankylosing spondylitis.

Ankylosing spondylitis (AS) is associated with both significant direct and indirect costs, which vary by country, and have generally increased dramatically since the introduction of anti-tumor necrosis factor therapy. The cost-effectiveness of biologic agents is controversial, although cost-effectiveness studies need to consider the potential impact of anti-tumor necrosis factor treatments on work ability. Alternatives to reduce costs associated with biologics have been examined, including on-demand dosing and lower dose alternatives. Other treatment measures, such as total hip arthroplasty an…

Periodontal disease in patients with ankylosing spondylitis.

OBJECTIVE: Ankylosing spondylitis (AS) and periodontal disease (PD) are characterised by dysregulation of the host inflammatory response, resulting in soft and hard connective tissue destruction. AS has been related to other inflammatory diseases, however, there is a paucity of data on whether AS is associated with inflammatory PD. METHODS: The association between AS and PD was examined in 48 patients with AS and 48 healthy controls, matched for age and gender. AS was diagnosed according to the modified New York criteria. Periodontal examination included probing pocket depth (PPD), clinical a…

Treatment of ankylosing spondylitis and other spondyloarthritides.

PURPOSE OF REVIEW: The management of ankylosing spondylitis and other spondyloarthritides (SpAs) such as psoriatic arthritis (PsA) is becoming more and more complex due to major advances, especially in the pharmacological therapy of these diseases. Biologics such as the anti-tumour necrosis factor agents have the potential to improve almost all outcome parameters usually assessed. The purpose of this review was to collect all available information related to the treatment of ankylosing spondylitis published in the year 2008. This was achieved by a systematic literature search in PubMed. RECEN…

Management of cervical deformity in ankylosing spondylitis.

Ankylosing spondylitis can lead to severe cervical kyphosis, causing problems with forward vision, swallowing, hygiene, patient functionality, and social outlook. Evaluation of patients with cervical flexion deformity includes assessment of global sagittal balance and chin-brow angle. The primary treatment in extreme disabling cases is surgical correction involving a posterior cervical extension osteotomy, which is a technically demanding procedure with considerable risk of neurological injury. To address the potential complications with extension osteotomy, the authors of several reports have…

[Quality of life of patients with ankylosing spondylitis].

Ankylosing spondylitis is ranked second among "great rheumatic diseases" which lead to considerable disability. The course of ankylosing spondylitis is chronic with variations in intensity of symptoms and is either continuously progressive or alternates with exacerbations and remissions. Difficulties in everyday life are due to limited motoric activity which makes full self-functioning rather impossible. The specificity of ankylosing spondylitis negatively affects the emotional state of patients. Patients are depressed, apathetic, discouraged to undertake treatment and rehabilitation. The aim …

Determinants of hyperkyphosis in patients with ankylosing spondylitis.

OBJECTIVE: To determine clinical and radiographic determinants of hyperkyphosis in patients with ankylosing spondylitis. METHODS: Spinal hyperkyphosis was assessed by occiput to wall distance (OWD) in 135 patients participating in the OASIS cohort and defined as OWD >0. Disease activity was assessed by the Bath ankylosing spondylitis disease activity index (BASDAI). Wedging of the vertebrae was calculated as the Ha/Hp ratio. Structural damage of the spine was assessed by the modified Stoke ankylosing spondylitis spine score (mSASSS). Hip involvement was assessed by the Bath ankylosing spondyl…

Audiovestibular disorders in patients with ankylosing spondylitis.

OBJECTIVE: Audiovestibular disorders have been described in several autoimmune diseases but have not been studied in patients with ankylosing spondylitis (AS). The aim of this study was to evaluate the audiovestibular function in patients with AS. METHODS: We prospectively evaluated 22 consecutive patients with AS. Clinical, radiologic, and immunogenetic features were analyzed. All patients underwent a complete ear, nose, and throat physical examination and audiologic evaluation that included pure-tone audiometry thresholds at octave frequencies of 250 to 8000 Hz, impedance audiometry (tympan…

[Diagnosis and early diagnosis of ankylosing spondylitis].

Ankylosing spondylitis (AS) is often diagnosed not before 5-10 years after onset of clinical symptoms. The main reason for this delay is the late appearance of unequivocal radiographic sacroiliitis which is a prerequisite for making the diagnosis. It is important to realize that both the early stage without radiographic sacroiliitis and the late stage with radiographic sacroiliitis are part of the same disease and, therefore, should be considered as a disease continuum. The term axial spondyloarthritis (SpA) appears appropriate to describe this disease continuum. Diagnosing axial SpA without r…

[Treatment of spinal fracture in ankylosing spondylitis].

Clinical guidelines for the treatment of vertebral fractures associated with ankylosing spondylitis are derived from case reports and a review of literature. The coincidence of paravertebral calcifications and fracture formations leads to problems in the establishment of a proper initial diagnosis. Therefore computed tomography and magnetic resonance imaging have to be employed to define the extent of fracture and the presence of spinal lesions. As a rule vertebral fractures based upon spondylitic alterations are extremely unstable and tend to secondary dislocation with a high risk of spinal c…

A case of osteomalacia mimicking ankylosing spondylitis.

A 39-year-old woman presented with symptoms of pain in the lumbar region and lower extremities. Physical findings included restricted movement of the lumbar spine, sacroiliac joint tenderness, positive Schober's test (10-12.5 cm), and bilaterally positive Mennel and Fabere tests. Although these symptoms and findings were suggestive of ankylosing spondylitis, osteomalacia was diagnosed with the appearance of multiple pseudofractures in her pelvic X-ray and laboratory abnormalities. All her symptoms and signs resolved in 6 months with vitamin D and calcium treatment.

Osteotomy of the cervical spine in ankylosing spondylitis.

Fifteen patients with ankylosing spondylitis who had developed a severe flexion deformity of the cervical spine which restricted their field of vision to their feet, were treated by an extension osteotomy at the C7/T1 level. The operation was performed under general anaesthesia with the patient in the prone position and wearing a halo-jacket. Three had internal fixation using a Luque rectangle and wiring. Their mean age was 48 years. Before operation the mean cervical kyphosis was 23 degrees; this was corrected to a mean of 31 degrees of lordosis, a mean correction of 54 degrees. All the patie…

Spinal fractures in patients with ankylosing spondylitis.

Thirty-one consecutive patients with ankylosing spondylitis and spinal fractures were reviewed. There were 6 women and 25 men with a mean age of 60 +/- 11 years; 19 had cervical and 12 had thoracolumbar injuries. Of the patients with cervical fracture, two had an additional cervical fracture and one had an additional thoracic fracture. Three trauma mechanisms were identified: high-energy trauma in 13 patients, low-energy trauma in 13 and insufficiency fracture in 5. One-third of the patients suffered immediate neurological impairment, a further one-third developed neurological impairment befor…

Clinical course and prognosis of Brucella spondylitis.

A multicentre prospective study of 593 patients with brucellosis, of whom 58 (9.7%) had spondylitis, was performed in order to evaluate the possible clinical, radiological and evolutionary differences in the different segments of the spinal column. Five of the patients with cervical spondylitis (71%) had compression of the medulla or roots, versus just two (11%) in the dorsal group and nine (21%) in the lumbar group (p less than 0.05). There were no other clinical, haematological or biochemical differences between the three spinal segments, and both the serological response and the percentage …

New light on uveitis in ankylosing spondylitis.

Inflammatory eye disease is well recognized in ankylosing spondylitis (AS) but the relationship between the uveitis and the spondyloarthropathy is poorly defined. The following conclusions may be drawn from a study of 1331 consecutive patients with AS: the prevalence of uveitis was 40% (535 subjects), almost half of whom had greater than 5 attacks. Family studies of sib pairs, concordant for AS, showed that uveitis occurred randomly with a concordance for uveitis/no uveitis of only 43%. A comparison of patients with (n = 535) and without (n = 796) uveitis showed no important differences. Analy…
