A case of gastric volvulus with atypical clinical features is reported. The predisposing aetiological factors and the mechanism of genesis of this very rare condition are discussed. The diagnosis was established by barium meal examination after an unsuccessful gastroscopy. The patient was successfully treated by emergency surgery.

A 51-year-old man with extramedullary plasmacytoma of the stomach is presented. He complained of epigastralgia. Barium meal revealed 2 submucosal tumors in the gastric body. Laboratory investigations showed neither hypergammaglobulinemia nor proteinuria. Bone X-ray examinations were not remarkable. Teleangiectatic polyps (TP) of the stomach arise from granulation OFFsue mostly in elderly people in the area of gastrojejunostomy decades after surgery. During endoscopy usually polyps of a size up to 2 cm can be found with a diffuse erosion of the surface and bleeding immediately upon being touched; this may cause the clinical symptomatology of acute gastric bleeding or chronic anemia as a consequence of blood loss. Myoelectrical activity was recorded from the human antrum on 136 occasions using a monopolar mucosal electrode in preoperative and post-vagotomy patients, and bipolar serosal electrodes in post-cholecystectomy patients, and the incidence of antral arrhythmias observed. Examination of the distribution of vagus nerve fibers to the stomach in 100 cadavers revealed considerable individual variation. No single pattern shown in textbooks or published papers can be accepted as correct for all individuals. The variations encountered are discussed from the viewpoint of the surgeon who wishes to perform highly selective vagotomy.

A report is made of the occurrence of acute idiopathic volvulus of the stomach in a 65 year old Nigerian male patient. To our knowledge, this is the first reported case of this rare condition in a Nigerian. The classification, diagnosis and surgical management of gastric volvulus are discussed and the world literature on this condition reviewed.
