Does Fixation of the Gastric Conduit Reduce the Incidence of Gastric Volvulus After Esophagectomy?

BACKGROUND: Acute volvulus of the gastric conduit is a rare complication after esophagectomy that warrants surgical intervention and is associated with increased morbidity and mortality. The aim of the study is to evaluate whether fixation of the gastric conduit would reduce the incidence of postoperative volvulus following esophagectomy. METHODS: This single-center retrospective analysis of patients who underwent esophagectomy was conducted to determine the incidence of acute postoperative volvulus following a change in practice. All patients who underwent an esophagectomy from September 201

A Rare Cause of Right-Sided Air Bubble on Chest Radiograph: Intrathoracic Gastric Volvulus Related to Morgagni Hernia.

Morgagni hernia is a rare disorder in adulthood, and most of the cases are asymptomatic. Symptomatic cases are extremely rare and present with life-threatening complications. Early diagnosis and surgery are lifesaving. We hereby present an adult case of symptomatic Morgagni hernia. Diaphragmatic herniation of the stomach and mesenteroaxial rotation led to intrathoracic gastric volvulus in this case. A right-sided air bubble on a chest radiogram was the only finding leading to the suspicion of diaphragmatic hernia. Computed tomography in the diagnosis of diaphragmatic hernias is of great import

Gastric outlet obstruction secondary to Morgagni hernia: a case report.

An 89-year old female presented to us with symptoms of gastric outlet obstruction which appeared to be secondary to gastric volvulus on preoperative work-up. On laparoscopy the stomach was found to be incarcerated in a right-sided Morgagni hernia with surrounding adhesions. The hernia was reduced after dissecting the adhesions and the diaphragmatic defect was repaired using a biologic mesh onlay patch (Surgisis GOLD, Cook Biotech Inc.). Her postoperative recovery was uneventful and she was doing well at three months follow-up.

[Distal gastric volvulus as a cause of acute occlusion in paraesophageal hernia. Considerations on a clinical case].

A case of distal volvulus of the stomach as a cause of acute intestinal obstruction in a paraesophageal hernia is presented. The patient, an old woman aged 82, had been suffering from abdominal pain and vomiting for about 48 hours. She successfully underwent emergency operation for the reduction of hernia and plasty of the hiatus anterior the esophagus. On the basis of personal experience and review of literature data, stress is laid on the high incidence of paraesophageal hernia complications and the importance of early diagnosis and surgical repair is underlined.

Delayed Traumatic Diaphragmatic Hernia Presenting With Gastric Volvulus and Necrosis: A Case Report and Literature Review.

Traumatic diaphragmatic hernia (TDH) is an uncommon condition resulting from blunt or penetrating thoracoabdominal trauma and is frequently underdiagnosed due to its nonspecific presentation. Delayed diagnosis may occur months or even years after the initial injury, increasing the risk of serious complications. Left-sided involvement is more common, and defects typically occur in areas of embryologic weakness. The persistent pressure gradient between the abdominal and thoracic cavities promotes progressive herniation of abdominal viscera. The stomach is commonly involved, and its displacement

Adult Bochdalek Hernia with Organo-Axial Gastric Volvulus: Misdiagnosed as Hydropneumothorax.

Bochdalek hernia (BH) in an adult may manifest clinically with a myriad of abdominal or chest symptoms or a combination of them. Diagnosis of an adult BH is usually delayed in view of rarity of the lesion and its varied presentation. A 30-year-old adult gentleman presented to us with a left thoracostomy which was draining pus and ingested food particles. The tube thoracostomy had been performed in another hospital for an apparent left hydropneumothorax before he arrived in our hospital. Computed tomography of Chest and abdomen revealed a left diaphragmatic defect with herniation of stomach, sp

A Massive Type IV Hiatal Hernia With Gastric Volvulus and the Complicated Road to Recovery.

Type IV paraesophageal hiatal hernias are rare but carry a significant risk of life-threatening complications, including gastric volvulus, incarceration, and the involvement of other intra-abdominal organs. Pancreatic herniation and associated pancreatitis are exceedingly uncommon. We report the case of a 74-year-old woman with Barrett's esophagus, chronic obstructive pulmonary disease (COPD), and obesity who presented with nausea, vomiting, and abdominal pain after a recent increase in the dose of tirzepatide. Imaging demonstrated a massive type IV paraesophageal hernia with organoaxial gastr

Cardiac Tamponade and Arrest Secondary to Simultaneous Gastric and Sigmoid Volvulus With Sigmoid Obstruction From an Incarcerated Left Inguinal Hernia.

Sigmoid volvulus is a well-documented cause of large bowel obstruction, often occurring in elderly or neurologically impaired individuals. While typically straightforward in diagnosis, concurrent anatomical abnormalities can obscure the clinical presentation and lead to catastrophic outcomes. This report presents an exceptionally rare case of simultaneous gastric and sigmoid volvulus with incarceration of the sigmoid colon within a left inguinal hernia, resulting in cardiac tamponade-like physiology and cardiac arrest. A 78-year-old man with cognitive impairment and bilateral inguinal hernias

[Diagnostic challenges of a 360° gastric dilatation volvulus in a dog: a case report].

An 8-year-old mixed-breed female dog was presented as an emergency case due to acutely occurring questionable vomiting, a tense abdomen and restlessness. Initial abdominal radiographs suggested a gastric dilatation. On the abdominal radiographs, no typical compartmentalization of the stomach was visible, and the pylorus appeared to be in a physiological position in the ventrodorsal view, making it difficult to differentiate the condition from a simple gastric dilatation. As the patient did not respond to stabilization measures and the stomach re-distended after gastrocentesis, a 360° gastric v

[Gastric volvulus in children: five case reports].

UNLABELLED: Gastric volvulus is an abnormal rotation of all or a part of the stomach around one of its axes. It is a rare cause of intestinal obstruction in children. This anomaly can be primary, due to abnormalities of the gastric ligaments, or secondary to other congenital malformations. We report on the cases of five children treated between January 1994 and December 2011, four boys and one girl, with a medium age of 7 months. Diagnosis was based on clinical features, particularly in the upper gastrointestinal Rx contrast study, which confirmed the diagnosis. Four out of the five children u

Acute oesophageal necrosis syndrome.

An 89-year-old woman with a known hiatus hernia presented to the accident and emergency department with acute onset epigastric pain. CT of the thorax and abdomen revealed a large hiatus hernia with mesentero-axial volvulus but no evidence of strangulation. A large aneurysmal aortic arch and descending aorta were visible with associated mural thrombus. As the pain was worsening, following discussion with the patient, the decision to operate was taken. The hiatus hernia was successfully reduced and the stomach looked healthy. The oesophagus, however, appeared black almost throughout its entire l

[Incarcerated Chilaiditi disease. A rare indication for surgery].

Surgery was performed on a rare case of incarcerated Chilaiditi's syndrome, a syndrome that has been known for 10 years. The patient was in the state of ileus with volvulus of the stomach and hepatodiaphragmatic interposition of the stomach, transverse colon, and omentum majus. The patient was at high risk--77 years old and in bad condition due to hyperemesis and abdominal pain. Preoperative intensive care was necessary. During the operation the intestinal structures were repositioned, the diaphragmatic hernia was resected, and the muscular parts were sewn tight. The right position of the live

Effect of circumcostal gastropexy on gastric myoelectric and motor activity in dogs.

Gastric electrical and contractile activities were assessed in healthy adult dogs on the eighth day after circumcostal gastropexy surgery, using serosal electrodes and strain gauge force transducers. Recordings were analysed to determine gastric slow wave frequency, presence of gastric slow wave dysrhythmias, gastric slow wave propagation velocity, coupling of gastric contractions to slow waves, a gastric motility index based on relative contractile amplitudes, and onset of gastric contractions after a standardised meal. Overall, gastric electrical and contractile activities were relatively un

Biomechanical and histological evaluation of a laparoscopic stapled gastropexy technique in dogs.

The biochemical holding strength and histological characteristics of a laparoscopic stapled gastropexy (LG) adhesion were compared with that of an incisional gastropexy (IG) adhesion. An LG was performed in 14 dogs and an IG was performed in six dogs. During the LG procedure, the abdomen was insufflated with carbon dioxide and three cannulae were placed in the caudal aspect of the right side of the abdomen. A 35 mm laparoscopic stapler was used to staple the gastric antrum to the adjacent right lateral abdominal wall. The IG procedure was performed through ventral midline celiotomy. A 35 mm IG

Plain film findings of gastric volvulus herniating into the chest.

When confronted with a chest radiograph demonstrating two large air-fluid levels, one in the retrocardiac mediastinum and the other beneath the left hemidiaphragm, the possibility of gastric volvulus herniating into the thorax should be strongly considered. These patients should have an immediate contrast study for further evaluation.

Acute gastric dilatation and volvulus in a free-living polar bear.

A large, adult male polar bear (Ursus maritimus) was found dead on a barrier island north of Prudhoe Bay, Alaska (USA), in June 1987. There were no external signs of trauma. A twisted distended stomach, distinctive parenchymal and fascial congestion, and significant difficulty in repositioning the anterior abdominal organs, indicated that gastric dilatation-volvulus (GDV) was the proximate cause of death. Polar bears frequently consume large quantities of food at one time and have large stomachs that are well adapted to periodic gorging. The scarcity of food in winter and early spring, combine
