The toxicity of oral zinc was investigated in patients hospitalized for chronic leg ulcers. Untoward effects were monitored by reference to clinical tolerance, hematological, hepatic and renal parameters, and serum concentrations of copper and iron. The investigation was conducted by comparing two groups of patients who initially did not differ significantly with regard to ulcer area, hemoglobin, leukocytes, copper, iron and zinc. The glomerular basement membrane was subjected to digestion with specific enzymes to determine the chemical nature (sialoglycoproteins, collagenous peptides, or glycosaminoglycans) of the anionic sites previously demonstrated in the laminae rarae. Sulfate substitutes for phosphate in the transitory uncoupling of rat liver mitochondria induced by hydrazine when beta-hydroxybutyrate is the substrate. A high level of sulfate in the absence of added phosphate induces a pseudo state three of the mitochondria. Uncoupling is inhibited by rutamycin. Analyses of sulfated mucopolysaccharides excreted by patients with Hunter, Hurler, Scheie, Sanfilippo A and Sanfilippo B syndromes are reported. Three distinct methods, namely agarose gel electrophoresis, enzymatic degradation and molecular weight determination, were used in an attempt to differentiate the mucopolysaccharidoses by analysis of the urinary mucopolysaccharides. Experiments in passive transfer of nickel allergy require highly sensitive guinea pigs and a reliable and technically usable test method. A comparison was made between two sensitization methods, the guinea pig maximization test of MAGNUSSON and KLIGMAN (MK) and a combined injection and painting method devised by POLAK and TURK (PT). In a 7-year-old boy with acrodermatitis enteropathica, oral administration of zinc sulfate brought about complete relief of symptoms and signs, with clearing of the skin lesions, arrest of diarrhea, and growth of new hair. Considering the dramatic results obtained, we believe that oral treatment with zinc sulfate is an innocuous, inexpensive, and very effective medication for acrodermatitis enteropathica, which can replace the previously used and toxic diiodohydroxyquin-like drugs.
