The external globus pallidus in patients with Parkinson's disease and progressive supranuclear palsy.

Underactivity of the external segment of the globus pallidus is thought to contribute to the generation of parkinsonian hypokinetic symptoms in association with striatal dopaminergic dysfunction and overactivity of the subthalamus. These symptoms can be corrected by neurosurgical techniques aimed at normalizing subthalamic overactivity. The aim of the present study was to compare the amount of neurodegeneration and changes in the calcium-binding protein parvalbumin in the external segment of the globus pallidus in parkinsonian disorders. Cases with progressive supranuclear palsy were compared

Cognitive and magnetic resonance imaging aspects of corticobasal degeneration and progressive supranuclear palsy.

OBJECTIVE: To identify cognitive and MRI features important for the clinical diagnosis of corticobasal degeneration (CBD) and progressive supranuclear palsy (PSP); these diseases share several clinical features and are often difficult to distinguish on clinical grounds. METHODS: Cognitive functions and MRI characteristics were examined in 16 patients with CBD and 28 patients with PSP, all diagnosed according to current clinical criteria (none was examined by autopsy). RESULTS: MRI findings differed significantly between the two groups: 87.5% of patients with CBD but none with PSP had asymmet

Clinical features differentiating patients with postmortem confirmed progressive supranuclear palsy and corticobasal degeneration.

Progressive supranuclear palsy (PSP) and cortocobasal degeneration (CBD) are often clinically confused with each other because they share a rapid disease progression, parkinsonism that responds poorly or transiently to levodopa therapy, and associated signs (e.g., ocular abnormalities, pyramidal signs and cognitive involvement). To improve the accuracy in diagnosing these disorders, this study examined the clinical features of 51 patients pathologically diagnosed with PSP and CBD. Logistic regression analysis identified two sets of predictors (models) for CBD patients, one consisting of asymme

Whipple's disease mimicking progressive supranuclear palsy: the diagnostic value of eye movement recording.

Treatable causes of parkinsonian syndromes are rare; Whipple's disease is one of them. A patient is described who presented with a parkinsonian syndrome and abnormal vertical gaze. Measurement of eye movements showed marked slowing of upward saccades, moderate slowing of downward saccades, a full range of voluntary vertical eye movements, curved trajectories of oblique saccades, and absence of square wave jerks. These features, atypical of progressive supranuclear palsy, suggested the diagnosis of Whipple's disease, which was subsequently confirmed by polymerase chain reaction analysis of inte

Progressive supranuclear palsy with fronto-temporal atrophy and various tau-positive abnormal structures.

An autopsy case of a 67-year-old man with typical clinical features of progressive supranuclear palsy (PSP) characterized by impairment of vertical ocular pursuit movement, pseudobulbar palsy, nuchal stiffness, parkinsonism, and dementia is described. In addition to typical pathological changes of PSP, the present case showed fronto-temporal cortical atrophy, accompanied with various Gallyas/tau-positive neuronal and glial structures such as neurofibrillary tangles, pretangle neurons, glial coiled bodies, astrocytic plaques and argyrophilic threads in the cerebral cortex and subcortical nuclei

[A case with clinical features of progressive supranuclear palsy with apraxia--corticobasal degeneration?].

We report a 65-year-old female who have suffered from progressive gait disturbance for 3 years, followed by disorientation and forgetfulness. Neurological examination revealed dementia, constructional disability, limb kinetic apraxia, supranuclear gaze palsy, especially on downward gaze, symmetrical muscle rigidity and bradykinesia. Involuntary movements were undetectable. Brain MRI showed significant brain atrophy in the left fronto-parietal lobe. The three-dimensional surface display with 131I-IMP demonstrated decreased cerebral blood flow in the left frontoparietal cortex. The diagnosis of

Pick Complex: an integrative approach to frontotemporal dementia: primary progressive aphasia, corticobasal degeneration, and progressive supranuclear palsy.

BACKGROUND: Frontotemporal dementia (FTD) is a new label for clinical Pick's disease (PiD) because the eponymic term is increasingly restricted to the pathologic finding of Pick bodies. This restriction created the impression that PiD is rare and that is it difficult to diagnose. FTD is also a term most often used for behavioral and personality alterations. Primary progressive aphasia (PPA) and corticobasal degeneration (CBD), formerly the extrapyramidal variety of PiD, are also part of the syndrome. Recently, chromosome 17 localization and tau mutations were discovered in familial forms of th

Remote network for cognitive symptoms derived from tau accumulation in progressive supranuclear palsy.

Progressive supranuclear palsy (PSP) is a neurodegenerative disorder with motor and cognitive impairments. Whereas motor symptoms are associated with subcortical tau deposits, the mechanisms underlying the cognitive symptoms remain unclear due to heterogeneous and primarily subcortical distribution of pathological tau deposits. Here, we combined tau-PET (tau positron emission tomography) using a high-contrast probe we developed with a normative connectome in 37 patients with PSP and 48 healthy controls. We found that tau deposition sites functionally connected to a common cortical network that

Unveiling the Distinctive Brain Functional Dynamics Between Parkinson's Disease and Progressive Supranuclear Palsy.

BACKGROUND: Dynamic analysis of resting-state fMRI (rs-fMRI) offers a novel approach to differentiate Parkinson's disease (PD) from progressive supranuclear palsy (PSP) by capturing temporal features of brain network activity, which may shed light on the mechanisms underlying non-motor symptoms. OBJECTIVES: To characterize differences in brain dynamics between PD and PSP using dynamic brain metrics, evaluate their exploratory discriminative performance, and investigate associations with non-motor symptoms. METHODS: Sixty-nine healthy controls, 82 PD patients, and 29 PSP patients underwent st

Domain-specific brain regions are associated with cognitive impairment in progressive supranuclear palsy.

BACKGROUND: Cognitive impairment significantly contributes to the disease burden of progressive supranuclear palsy (PSP), however, the underlying pathophysiologiy is not well understood. OBJECTIVES: To gain a better understanding of the pathophysiology, we identified the brain regions associated with individual domains of impaired cognition. METHODS: We analyzed MRI data from a cohort of 31 patients with PSP (age 71.0 +-7.0 years, range 58-87; 15 females; disease duration 2.9 +- 1.8 years). Cerebral microstructure was approximated with Diffusion Microstructure Imaging and cognitive performan

Dynamic Stability Analysis of Progressive Supranuclear Palsy Affected Gait Using Lyapunov Floquet Theory.

Progressive supranuclear palsy (PSP) is a neurodegenerative disease with severe gait and balance deficits. There are no effective ways to assess dynamic balance during walking in PSP. The Lyapunov Floquet (LF) theory has been utilized to study dynamic balance in healthy and pathologic gait but has not been applied to PSP affected gait. In the current study, the medio-lateral motion of the center of mass during gait for 40 patients with PSP (PSP group) and 33 healthy older adults (Control group) were studied. Metrics from LF theory, such as the maximal Floquet multiplier (FM), maximal long-term

Diffusion Tensor Imaging in Progressive Supranuclear Palsy Versus Other Neurodegenerative Diseases: A Review.

Progressive supranuclear palsy (PSP) is a complex neurodegenerative disorder that is frequently misdiagnosed, largely due to its overlapping clinical features with other neurodegenerative diseases, such as Parkinson's disease and multiple system atrophy. Accurate and early diagnosis remains a significant clinical challenge. In this context, diffusion tensor imaging (DTI), a specialized magnetic resonance imaging technique that measures the directional movement of water molecules in neural tissue, has emerged as a promising biomarker. This narrative review synthesizes current research on the ut

Disproportionately Enlarged Subarachnoid-Space Hydrocephalus on MRI in Pathologically Confirmed Progressive Supranuclear Palsy.

BACKGROUND AND OBJECTIVE: Several studies have shown that idiopathic normal-pressure hydrocephalus (iNPH) can mimic other neurodegenerative disorders, particularly progressive supranuclear palsy (PSP). In this study, we investigated iNPH clinical and neuroimaging features in patients with autopsy-confirmed PSP or Lewy body disease (LBD) by assessing the normal pressure hydrocephalus (NPH) triad of symptoms and imaging features of disproportionately enlarged subarachnoid-space hydrocephalus (DESH) and Evans index (EI) on antemortem MRI scans. METHODS: Among our study participants (N = 190), th

Alexander disease with a novel GFAP insertion-deletion mutation mimicking progressive supranuclear palsy.

This report presents a case of Alexander disease showing clinical characteristics mimicking progressive supranuclear palsy (PSP). A 67-year-old woman complaining of motor disturbance exhibited severe atrophy of medulla, spinal cord, and midbrain tegmentum, as well as periventricular hyperintensity on cerebral MRI. Genetic analysis identified a novel in-frame deletion/insertion mutation in the exon 3 of the GFAP gene. Interestingly, neurological findings and decreased striatal uptake in dopamine transporter SPECT were suggestive of PSP. A novel GFAP gene mutation found in the present case may c
