Extramedullary Motorized Lengthening of the Femur in Young Children.

BACKGROUND: Limb lengthening by distraction osteogenesis is now achievable via motorized intramedullary devices, mitigating many complications of lengthening by external fixation. In young patients, antegrade intramedullary nailing of the femur risks avascular necrosis of the femoral head. A method of extramedullary placement of a motorized expandable intramedullary nail has been employed by the senior author to safely achieve femoral lengthening without the use of an external fixator in young patients. METHODS: Eleven skeletally immature patients with lower limb length discrepancy were revie

Proximal femoral osteotomy in Legg-Calvé-Perthes disease using a monolateral external fixator: surgical technique, outcome, and complications.

UNLABELLED: Although external fixation methods have been described for proximal femoral osteotomy for various etiologies, none are dedicated to a single disease entity. Our study introduces a technique of proximal femoral osteotomy and fixation with a monolateral external fixator system in Legg-Calvé-Perthes disease (LCPD). Twenty-three patients (19 males, four females) with LCPD underwent surgery at our institute between 2004 and 2012. Varus osteotomy (group A, 11 hips) and valgus osteotomy (group B, 12 hips) were performed and the monolateral external fixator system was used. The average age

"Cat's cradle" midfacial fixation in distraction osteogenesis after Le Fort III osteotomy.

Distraction osteogenesis has provided a powerful technique for the treatment of severe midfacial hypoplasia. Skeletal fixation is a critical component of successful distraction osteogenesis. This is a report on the development of a new approach to bone fixation for distraction osteogenesis of the midface, based on circumferential suspension of the entire midfacial skeleton using surgical wires placed through the soft palate. This technique offers the advantages of (1) complete control of the attitude of the midface in terms of a precise dispersion of forces that can determine the desired pitch

Minimal invasive pocket technique for magnet bone implant hearing aid without fixation.

INTRODUCTION: The BAHA (bone-anchored hearing aid) Attract is a magnetic transcutaneous bone conduction device anchored into the temporal bone. The standard surgical technique for BAHA Attract is a multi-tools time-consuming process, which requires a large cutaneous incision. The objective of this study is to describe and test the feasibility of a minimally invasive pocket (MIP) technique for Magnet Bone Implant Hearing Aid (MBIHA) with a modified magnet of BAHA Attract without fixation and without any tissue reduction. We use a 3-cm vertical skin incision and a subperiosteal pocket. METHOD:

Treatment of open tibia fractures in Sub-Saharan African countries : a systematic review.

Open tibia fracture (OTF) treatment is well documented in developed countries. Yet, this fracture pattern remains challenging because it is associated with an increased risk of infection and delayed union, particularly in case of Gustilo III B and C open fractures. Since access to healthcare is limited in Sub- Saharan African countries, this paper explores the results of OTF management in this setting. A systematic review of the literature was conducted using current databases such as MEDLINE, Cochrane, EMBASE, PubMed, ScienceDirect, Scopus, and Google Scholar in order to identify prospective

A prospective comparative study of the compression hip screw and the gamma nail.

Achieving bony union and early weight bearing in peritrochanteric femur fractures poses a continuous challenge for the orthopedic surgeon. Current standards of surgical fixation for the myriad possible fracture patterns include the compression hip screw (CHS), the 90 degrees/95 degrees blade plate, and locked intramedullary nails. The gamma nail (GN) was designed as a "hybrid" of these devices, combining theoretical biomechanical advantages that should allow earlier postoperative weight bearing with surgical technique advantages that should decrease operative morbidity. Because these theoretic

Comparing internal and external stabilization for traumatic elbow instability: a systematic review.

BACKGROUND: Despite surgical reestablishment of the supporting structures, instability may often persist in traumatic elbow injury. In these cases, a temporary internal or external fixator may be indicated to unload the repaired structures and maintain joint concentricity. Aggregate data are needed to characterize the risk of complication between external fixation (ExFix) and the internal joint stabilizer (IJS) when used for traumatic elbow instability. Our objective was to review the literature to compare the complication profile between external fixation and the IJS. METHODS: A database que

Scleral fixation of a dexamethasone intravitreal implant in a case of subluxated bag-intraocular lens complex.

Macular edema is a condition of retinal tissue treated with anti-inflammatory agents including placement of an intravitreal sustained-release dexamethasone device, designed to deliver a controlled amount of the medication for a prolonged time, representing an excellent therapy. Nonetheless, the implantation cannot be carried out without an anatomical barrier, such as the presence of posterior capsular support, lens, or intraocular lens. The absence of these barriers could lead to several complications, due to migration of the device from the vitreous cavity to the anterior chamber, causing cor

Potent antimicrobial activity of bone cement encapsulating silver nanoparticles capped with oleic acid.

Bone cement is widely used in surgical treatments for the fixation for orthopaedic devices. Subsequently, 2-3% of patients undergoing these procedures develop infections that are both a major health risk for patients and a cost for the health service providers; this is also aggravated by the fact that antibiotics are losing efficacy because of the rising resistance of microorganisms to these substances. In this study, oleic acid capped silver nanoparticles (NP) were encapsulated into Poly(methyl methacrylate) (PMMA)-based bone cement samples at various ratios. Antimicrobial activity against Me

Surgical Removal of Hearing Aid Earmold Impression Material in the Middle Ear.

When fitting hearing aids, patients are required to make an earmold impression material for device fixation. It usually causes no problems, although in rare cases, the earmold passes through the middle ear through tympanic membrane perforations.1-3 Foreign bodies may cause a delayed inflammatory reaction and deterioration of aeration, especially in the Eustachian tube. Herein, we report a rare case of earmold impression material as a foreign body in the middle ear that required surgical removal.

A Midterm Review of Lesser Toe Arthrodesis With an Intramedullary Implant.

UNLABELLED: Lesser toe deformities are one of the most common conditions encountered by podiatric surgeons. When conservative treatments fail surgical correction is indicated. Many surgical options have been described to address the complex nature of these deformities but no perfect solution has been reported to date. However, with the continued advancement of internal fixation technology, interphalangeal joint (IPJ) arthrodesis with an intramedullary implant may be a good option. This retrospective study presents patient reported outcomes and complications at 6 months and 3 years following le

Femoral Malunion and Its Correction: A Review.

Background and Objectives: Femoral malunion, defined as healing of a femoral fracture in an anatomically incorrect position, can lead to significant biomechanical and functional impairment despite modern fixation techniques achieving union rates near 99%. The lack of a universal definition and standardized management approach continues to hinder optimal outcomes. This review aims to synthesize the literature on the causes, clinical presentation, radiologic assessment, surgical indications, corrective procedures, and outcomes of femoral malunion to guide clinical decision-making and future rese
