Multifocal pigmented villonodular synovitis in a child: A case report.

INTRODUCTION: Pigmented villonodular synovitis (PVNS) is a rare, benign proliferative disorder of the synovial membrane that typically presents in adults and affects a single joint. Multifocal PVNS is very rare, particularly in childhood. We reported a rare case of multifocal PVNS affecting over 20 joints in a child. CLINICAL PROCEDURE: A 7-year-old female patient had a 6-month history of multifocal joints swelling with mild pain. She was diagnosed as polyarticular juvenile idiopathic arthritis at a local hospital. Naproxen, methotrexate, infliximab, and pavlin were used to treat the patient …

Pigmented villonodular synovitis of the temporomandibular joint with intracranial extension.

Pigmented villonodular synovitis is an uncommon benign tumor-like proliferative lesion with an undetermined origin. Involvement of the temporomandibular joint is uncommon. Although pigmented villonodular synovitis is a benign lesion, it can grow with an aggressive pattern, and it extends extra-articularly in most of the reported cases, about one-third of them exhibiting intracranial involvement. The authors reported an additional case of a 47-year-old woman with intracranial extension, who had a history of joint pain and trismus. The preoperative diagnosis was made with arthroscopy. The lesion…

Arthroscopic Management of Pigmented Villonodular Synovitis of the Knee Joint.

INTRODUCTION: Pigmented Villonodular Synovitis (PVNS) of knee joint is a rare disorder of Synovium. Hip and knee joint are commonly affected joints. The knee PVNS presents as a localized or diffuse form. Diagnosis if often delayed and permanent joint damage occurs with advanced disease. Ultrasound examination shows fluid collection and synovial hypertrophy. Magnetic resonance imaging helps in clinching the diagnosis. Final confirmation of PVNS is done with histopathological examination of synovial tissue removed. Post operative radiation has shown to reduce the rate of recurrent disease. CASE…

[Pseudoseptic arthritis with foreign body synovitis in a patient with gout].

Monoarthritis is a diagnostic challenge for the clinician, as the list of associated conditions is quite long. It is accepted that in patients with a diagnosis of inflammatory joint disease monoarthritis represents exacerbation of the underlying disease. However, ignoring the systematized approach to monoarthritides may lead to omissions and mistaken diagnostic implications. This report describes the approach to a case of pseudoseptic arthritis that mimicked an acute episode of recurrent arthritis due to monosodium urate in a patient with retention of an intraarticular foreign body.

Association of pain in knee osteoarthritis with distinct patterns of synovitis.

OBJECTIVE: To determine possible patterns of synovitis on contrast-enhanced magnetic resonance imaging (CE-MRI) and its relation to pain and severity in patients with radiographic knee osteoarthritis (OA). METHODS: In total, 86 patients (mean age 62 years, 66% women, median body mass index 29 kg/m(2) ) with symptomatic knee OA (Kellgren/Lawrence radiographic score 3) were included. T1-weighted, gadolinium-chelate-enhanced MRI with fat suppression was used to semiquantitatively score the extent of synovitis at 11 knee sites (total score range 0-22). Self-reported pain was assessed with 3 stand…

Value of ultrasonography for diagnosis of synovitis associated with rheumatoid arthritis.

AIM: To investigate the value of ultrasonography (US) for diagnosing synovitis associated with rheumatoid arthritis (RA). METHOD: Bilateral metacarpophalangeal (MCP), proximal interphalangeal (PIP) II-V and wrist joints of 46 RA patients and 35 healthy controls were evaluated by quantitative and semiquantitative US. Wrists on more severely affected sides of 20 of the 46 patients also underwent magnetic resonance imaging (MRI). The MRI and US results were compared. The US cutoff to distinguish pathology was calculated. The two US methods were compared and the correlation between quantitative m…

The impact of platelet-rich plasma on chronic synovitis in hemophilia.

Untreated chronic haemophiliac synovitis leads to the development of haemophilic arthropathy (HA) by affecting the metabolism of chondrocytes. Symptoms are progressive and often surgical intervention is required to prevent total loss of joint function. The focus of our study was to influence the chronic haemophiliac synovitis by means of autologous platelet-rich plasma (PRP) injection. Six patients with hemophilia (PWH), aged between 9 and 45 and manifesting chronic synovitis of the ankle joint on one or on both sides (8 joints in total) were included into the PRP-study. The patients were clas…

Assessment of synovitis to predict bone erosions in rheumatoid arthritis.

Although rheumatoid arthritis (RA) is traditionally considered as the prototype of destructive arthritis, the course of the disease varies c … in RA is the result of the cumulative burden of inflammation, variables reflecting the severity of synovitis and its persistence over time might refine our ability to build early prognostic algorithms. The goal of this article is to review the clinical implications of the assessment of synovitis in relation to radiographic outcomes. Traditional and novel a …

[Effect of platelet-rich plasma on synovitis of rabbit knee].

OBJECTIVE: Platelet-rich plasma (PRP) can promote the repair of soft tissue, wound, and bone defect. To investigate the effect of PRP on synovitis by establishing papain-induced osteoarthritis model of rabbit knee and interfering with PRP. METHODS: Twenty healthy 6-month-old rabbits (male or female, weighing 2.5-3.5 kg) were randomly divided into the experimental group (n = 10) and the control group (n = 10). The whole blood (10 mL) was extracted from the central aural artery and PRP was prepared with the Landesberg's method. Meanwhile, the platelet derived growth factor (PDGF), transforming …

Angiogenesis and the persistence of inflammation in a rat model of proliferative synovitis.

OBJECTIVE: To determine whether blood vessel growth at the onset of resolving synovitis leads to its subsequent persistence and whether inhibiting this angiogenesis at the onset of persistent inflammation leads to its subsequent resolution. METHODS: Inflammation and angiogenesis were induced by injection of 0.03% carrageenan and/or 6 pmoles of fibroblast growth factor 2 (FGF-2) into rat knees. A brief treatment with the angiogenesis inhibitor PPI-2458 (5 mg/kg orally on alternate days) was administered 1 day before and up to 3 days after synovitis induction. Controls comprised naive and vehic…

Pigmented villonodular synovitis of the knee in skeletally immature patients.

INTRODUCTION: Pigmented villonodular synovitis (PVNS) is a benign idiopathic proliferative disorder that results in villous or nodular formation in the joints, tendons sheaths, and bursae. As PVNS is a rare pathology in children, diagnosis is often delayed. In this study, we analyze the therapeutic methods used and results obtained in the treatment of this pathology. MATERIALS AND METHODS: All patients with PVNS of the knee seen between January 1988 and June 2006 were evaluated. We assessed the form of presentation, time to diagnosis, previous diagnosis, type of treatment, relapse, and the ne…

Transient synovitis of the hip: more evidence for a viral aetiology.

OBJECTIVES: Transient synovitis (TS) of the hip is a common cause of limping in children. A link with infection has been suggested but has not been firmly established. We conducted a case-control study to determine whether the TS is associated with infection. METHODS: We enrolled children diagnosed with TS in our emergency department over a 10-week period. For each case, we identified an age-matched and sex-matched child with a fracture. Parents of cases and controls were questioned with regard to recent infectious illness in their child according to a predefined questionnaire. RESULTS: A pa…

Localized pigmented villonodular synovitis of the posterior capsule of the knee.

Localized pigmented villonodular synovitis involving the posterior intra-articular structures of the knee joint is rare. We report a case of localized pigmented villonodular synovitis arising from the posterior capsule of the knee joint in a patient who presented with a history of repeated joint effusions and loss of terminal knee flexion. After successful, complete removal of the mass through the posteromedial portal, the patient was asymptomatic, with no evidence of recurrence during the 26-month follow-up period.

Prophylaxis and treatment of chronic synovitis in haemophilia patients with inhibitors.

Prophylaxis is paramount to try to avoid the development of haemophilic synovitis. The best treatment for synovitis in patients with inhibitors is radioactive synoviorthesis (rhenium for ankle and elbows, yttrium for knees). With both methods (prohylaxis and radioactive synoviorthesis), we can delay the development of severe haemophilic arthropathy, that eventually will require major orthopaedic surgery.
