Episodic memory requires associating items with temporal context, a process for which the medial temporal lobe (MTL) is critical. This study uses recordings from 27 human subjects who were undergoing surgical intervention for intractable epilepsy. These same data were also utilized in Umbach et al. (2020). We identify 103 memory-sensitive neurons in the hippocampus and entorhinal cortex, whose firing rates predicted successful episodic memory encoding as subjects performed a verbal free recall task. These neurons exhibit important properties. First, as predicted from the temporal context model

OBJECTIVE: The ability to imagine future events, episodic simulation (ES), is critical for goal setting and adaptive functioning. Although ES engages many of the same neural substrates as episodic memory (EM) in healthy controls, particularly the hippocampus, its disruption in clinical populations remains poorly characterized. Temporal lobe epilepsy (TLE) is a condition marked by mesial temporal pathology and known EM impairments. However, both the relationships between EM and ES deficits and their hippocampal structural correlates remain unexplored. Here, we tested whether TLE is associated w

OBJECTIVE: Widespread cognitive deficits in temporal lobe epilepsy (TLE) are associated with macroscale brain structural changes. However, the correlation between the changes in the cortical morphological similarity of the TLE and gene expression remains unclear.
METHODS: Structural magnetic resonance imaging (MRI) was collected from 31 TLE patients and 35 healthy controls to construct an individual-level Morphometric INverse Divergence (MIND) network. Changes in cortical morphological similarity attributable to TLE and their cognitive correlates were assessed. Using a public transcriptional

OBJECTIVE: Temporal lobe epilepsy (TLE) is the most common form of medically refractory epilepsy in adults. While patients with concordant noninvasive findings and hippocampal sclerosis (HS) might proceed directly to surgery, those with discordant data, suspected bilateral involvement, or atypical presentation often require stereoelectroencephalography (SEEG). The diagnostic and therapeutic implications of bilateral temporal sampling remain uncertain. The aim of this study was to evaluate the yield and clinical impact of bilateral SEEG in TLE.
METHODS: The authors retrospectively reviewed dat

OBJECTIVE: In temporal lobe epilepsy (TLE), detection of the epileptogenic zone predicts a good surgical outcome. When submitted to 18F-fluorodeoxyglucose positron emission tomography (PET), some patients display lateralized, focal hypometabolism in the temporal lobe (PET+), whereas others appear normometabolic (PET-). However, the mechanism behind this metabolic difference remains unclear. This study aimed to identify differential molecular mechanisms in these patient subtypes.
METHODS: Neocortical and hippocampal biopsies of TLE patients (n = 3 PET+, n = 3 PET-) and nonepileptic postmortem

OBJECTIVES: The present study investigates the explicit and implicit timing abilities of patients with mesial temporal lobe epilepsy (MTLE). Based on previous studies, it was hypothesized that timing abilities were decreased in MTLE patients.
METHODS: The performance of 21 MTLE patients and 21 neurologically healthy probands was tested on two separate tasks. The time bisection task was used to investigate explicit timing and the foreperiod task to test implicit timing.
RESULTS: For the time bisection task, less precise temporal judgements were found in the patient group compared to the contr

Non-invasive neuroimaging is important in epilepsy to help identify cerebral abnormalities. Abnormally reduced fractional anisotropy (FA) in deep white matter (WM) from diffusion-weighted imaging (DWI) is widely reported in large multi-cohort studies across all types of epilepsies. However, abnormalities in FA for superficial WM are rarely investigated in epilepsy. To gain a greater understanding of the nature of WM abnormality at different WM depths, we investigated DWI abnormalities at a range of superficial and deep WM in two separate temporal lobe epilepsy (TLE) cohorts. The first cohort (

Temporal lobe epilepsy (TLE) is the most common pharmacoresistant epilepsy in adults, yet few patients receive curative surgery due to diagnostic and prognostic uncertainty. In a multicenter cohort, we analyzed multimodal MRI and clinical data from 282 TLE patients, 298 healthy controls, and 45 disease controls. Patient-specific deviations from typical lifespan trajectories of intrinsic brain function were mapped using normative modeling. Regional functional alterations were heterogeneous but overlapped most in the mesiotemporal cortex. Connectome-based simulations revealed abnormality spread

Ictal bruxism is a rare motor manifestation in temporal lobe epilepsy with unclear network underpinnings. We report the first case demonstrating SEEG-guided network characterization of ictal bruxism in a 55-year-old man with drug-resistant temporal lobe epilepsy who continued to experience stereotyped seizures with rhythmic teeth grinding after a failed anterior temporal lobectomy. SEEG recorded one spontaneous and two stimulation-induced seizures sampling from the superior temporal gyrus, anterior ventral insula (aVInsula), and nine additional regions of interest. Recordings identified seizur

Encephaloceles are abnormal protrusions of brain tissue, meninges, and cerebrospinal fluid that result from defects in the skull base or cranial vault. These abnormalities can lead to seizure disorders and focal pharmacoresistant epilepsies. However, clinical suspicion and diagnosis are frequently delayed due to the significant challenges in interpreting initial imaging. Interpretation omissions can contribute to false diagnosis as non-lesional epilepsy in patients with encephalocele. This warrants appropriate imaging paradigms and careful interpretation. When clinical suspicion remains, furth

BACKGROUND: Classical type of superficial hemosiderosis (SH) is subpial hemosiderin deposition mainly affecting the cerebellum, brainstem, and spinal cord, which generally presents with cerebellar ataxia and sensorineural hearing disturbance. We here report a rare case of the classical type of SH presenting with temporal lobe epilepsy and perform a literature review on similar cases.
CASE DESCRIPTION: A 63-year-old man with four episodes of impaired awareness and confusion lasting for around 5 minutes after feeling vague uneasiness, suggesting focal impaired awareness seizure, visited a neuro

PURPOSE: Reduced glucose metabolism in the hippocampus is commonly observed in cases of medial temporal lobe epilepsy (MTLE) with hippocampal sclerosis (HS). Glucose metabolism among the various hippocampal subfields has not been thoroughly investigated.
PATIENTS AND METHODS: This study examined 29 patients (18 females; 15-58 years) diagnosed with HS who underwent surgery for drug-resistant epilepsy. FreeSurfer 7.1.1 was used in the processing of MRI data and 18 F-FDG PET scans to derive volumetric data and the FDG SUVr in the whole hippocampus and hippocampal subfields, including the CA1, CA

Temporal lobe epilepsy (TLE) is the most common pharmaco-resistant epilepsy in adults. While primarily associated with mesiotemporal pathology, recent evidence suggests that brain alterations in TLE extend beyond the paralimbic epicenter and impact macroscale function and cognitive functions, particularly memory. Using connectome-wide manifold learning and generative models of effective connectivity, we examined functional topography and directional signal flow patterns between large-scale neural circuits in TLE at rest. Studying a multisite cohort of 95 patients with TLE and 95 healthy contro

OBJECTIVE: Pathological amyloid-β (Aβ) accumulation and hyperphosphorylated tau proteins have been described in resected temporal lobe specimens of epilepsy patients. We aimed to determine cerebrospinal fluid (CSF) Aβ1-42 and p181-tau levels and cerebral Aβ deposits on positron emission tomography (Aβ PET) and correlate these findings with cognitive performance in adults with drug-resistant temporal lobe epilepsy (TLE).
METHODS: In this cross-sectional study, we enrolled individuals with drug-resistant TLE who were 25-55 years old. Each participant underwent 18F-flutemetamol PET, determinatio

BACKGROUND: Hemichorea and other hyperkinetic movement disorders are uncommon presentations of stroke and are usually secondary to deep infarctions affecting the basal ganglia and thalamus. Therefore, temporal ischemic lesions causing hemichorea are rare. We report the cases of two patients with acute ischemic temporal lobe infarct strokes that presented as hemichorea.
CASE SUMMARY: Patient 1: An 82-year-old woman presented with a 1-mo history of involuntary movement of the left extremity, which was consistent with hemichorea. Her diffusion-weighted imaging (DWI) revealed an acute ischemic st

INTRODUCTION: Typical MRI measures of neurodegeneration have limited sensitivity in early disease stages. Diffusion MRI (dMRI) microstructural measures may allow for detection in preclinical stages.
METHODS: Participants had dMRI and either beta-amyloid PET or plasma biomarkers of Alzheimer's pathology within 18 months of MRI. Microstructure was measured in portions of the medial temporal lobe (MTL) with high neurofibrillary tangle (NFT) burden based on a previously developed post mortem 3D-map. Regressions examined relationships between microstructure and markers of Alzheimer's pathology in

Temporal lobe epilepsy (TLE) can cause different types of memory impairments. Here, we report a case of immediate improvement of memory impairment following antiepileptic drug (AED) treatment in a patient with TLE with amygdala enlargement (TLE-AE), who rapidly developed recurrence. The patient was a man in his 60s whose family members complained of his amnesia. Neuropsychological investigations detected obvious recent and remote memory loss and executive function impairments. Our examinations revealed evidence of TLE and bilateral amygdala enlargement without any results suggesting organic di
