Resection of Hibernoma of the Proximal Shin Followed by Foot Drop and Tendon Transfer: A Case Report.

This is the description of the case of a 42-year-old male who presented with a growing, painless lump on his anterior leg. The diagnosis of a rare tumor called hibernoma was suspected after inspection of the radiologic findings. The unusual location of the tumor resulted in superficial peroneal nerve entrapment. The tumor was excised and the diagnosis of hibernoma was confirmed by histopathology. Surgery resulted in foot drop that was successfully treated with a tibialis posterior tendon transfer. Our case illustrates a rare tumor in an unusual location that can be challenging for clinicians t…

The outcome of soft-tissue release and tendon transfer in shoulders with brachial plexus birth palsy.

BACKGROUND: Shoulder involvement in brachial plexus birth palsy is common, and the adduction, internal rotation contracture deformity often requires some form of surgical treatment. There are very few long-term reports on release of contracted muscles and tendon transfers, especially in older children. We are reporting the single-center results of such a surgery with detailed outcome analysis. METHODS: The prospectively collected data from brachial plexus birth palsy cases who had undergone contracture release and tendon transfer were retrospectively studied and examined. The new Mallet and f…

Tendon transfers for radial nerve palsy with extensor carpi ulnaris revival: Technique and results.

BACKGROUND: Nerve repair is the gold standard for treatment of radial palsy. In case of failure or contraindication, palliative techniques using tendon transfers provide good results. However, wrist extension frequently shows radial deviation, impairing grip strength. HYPOTHESIS: Associating extensor carpi ulnaris (ECU) revival avoids radial deviation. STUDY DESIGN: Single-center retrospective study. MATERIAL AND METHODS: The inclusion criterion was radial nerve palsy treated by tendon transfers involving revival of the ECU. Nine patients, with a mean age of 33 years [15-60] were included. …

Upper Extremity Tendon Transfers: A Brief Review of History, Common Applications, and Technical Tips.

Background  Tendon transfer in the upper extremity represents a powerful tool in the armamentarium of a reconstructive surgeon in the setting of irreparable nerve injury or the anatomic loss of key portions of the muscle-tendon unit. The concept uses the redundancy/expendability of tendons by utilizing a nonessential tendon to restore the function of a lost or nonfunctional muscle-tendon unit of the upper extremity. This article does not aim to perform a comprehensive review of tendon transfers. Instead it is meant to familiarize the reader with salient historical features, common applications…

Posterior Deltoid-to-Triceps Tendon Transfer for Elbow Extension in a Tetraplegia Patient: A Case Report.

In tetraplegia patients, activities of daily living are highly dependent on the remaining upper limb functions. In other countries, upper limb reconstruction surgery to improve function has been applied to diverse cases, but few cases have been reported in Korea. The current authors experienced a case of posterior deltoid-to-triceps tendon transfer and rehabilitation in a complete spinal cord injury with a C6 neurologic level, and we introduce the case-a 36-year-old man-with a literature review. The patient's muscle strength in C5 C6 muscles were normal, but C7 muscles were trace, and the Spin…

Management of the recurrent deformity in a flexible foot following failure of tendon transfer: is arthrodesis necessary?

Recurrent deformity in the adult flatfoot following previous tendon transfer represents a challenging treatment dilemma for even the most experienced foot and ankle surgeon. The evaluation must be comprehensive, resulting in a clear understanding of the extent to which previous surgical procedures either failed to address the deformity initially or led to progressive recurrence. Particularly in younger, more high-demand patients, every effort to preserve normal joint mechanics while alleviating pain and restoring functional alignment must be made. LCL coupled with MDCO and a comprehensive medi…

Risk of neurovascular injuries in flexor hallucis longus tendon transfers: an anatomic cadaver study.

BACKGROUND: Flexor hallucis longus (FHL) tendon transfer is a frequently used treatment for both posterior tibial tendon insufficiency and chronic Achilles tendinopathy. We observed difficulties in harvesting the FHL tendon that may arise from cross-attachments with the flexor digitorum longus (FDL) tendon near the knot of Henry. The posterior tibial nerve is located nearby the decussation of these tendons. This study examined whether the difficult harvesting may be the cause of nerve injury. METHODS: A cadaver study was performed on 24 foot specimens. In all feet, we used a double-incision t…

Dynamic procedures for claw-finger correction--dilemma of balancing tension in transferred tendon slips.

The critical step in dynamic claw-finger correction procedures is adjustment of tension on the tendon slips which are being sutured at the new insertion sites to correct finger-clawing. Several methods to balance and adjust the tension have been described ever since these procedures have been in use. Ultimately it is the experience of the operating surgeon that helps to decide as to the tension that is to be kept on each slip so that maximum deformity correction is obtained without compromising the functional capabilities of the hand. An attempt has been made to describe this "experience" in w

The Effect of Extensor Digiti Minimi Tendon (EDM) Transfer on Ulnar Deviation of the Little Finger.

BACKGROUND: The little finger permanent abduction is an annoying deformity usually along with Wartenberg's sign (a consequence of ulnar nerve palsy), but there are several ways to correct this condition in rheumatoid arthritis as well. We aimed to investigate the effect of surgical intervention on patients. METHODS: The current study was a clinical trial which was done at Hazrat Fatima Hospital of Tehran, Iran from 2020-2022, where 15 patients with an age range of 21-48 years were investigated. All these patients had complications of ulna nerve damage. After the intervention (tendon transfer)…

Clinical Application of Anomalous Muscles for Tendon Transfer in the Upper Limb: A Systematic Review of the Literature.

Numerous anomalous muscles of the forearm and hand have been reported in the literature. The majority were encountered in cadaver dissections and some were incidentally detected during tendon transfer surgery. Because of the limited number of motors available for transfer, it may be advantageous if an anomalous muscle with favorable anatomy can function as a potential donor in a suitable clinical environment. Although several authors have illustrated various anomalies and their functional significance, the reports of actual utilization of such muscles as donors for tendon reconstructions are s…

Tendon transfer for treatment of internal rotation contracture of the shoulder in brachial plexus birth palsy.

We retrospectively analyzed 63 patients with internal rotation contracture of the shoulder secondary to brachial plexus birth palsy treated with subscapularis sliding combined with either latissimus dorsi transfer (group A: n = 18) or latissimus dorsi and teres major transfer (group B: n = 45) to the rotator cuff. The mean age at time of surgery was 43 months (SD 21 months; range 8 months to 9 years). We used a modification of the Gilbert shoulder grading system for assessment. All patients showed statistically significant improvement of active shoulder abduction and external rotation without
