Closed rupture of the flexor tendons caused by carpal bone and joint disorders.

We analysed 21 patients with closed rupture of the flexor tendons caused by carpal bone and joint disorders. The tendon that ruptured depended on the location of the bone perforation into the carpal tunnel. Radiocarpal arthrography was performed in 13 patients and capsular perforation was demonstrated by contrast medium leakage into the carpal canal in 11 patients. This proved a useful diagnostic test. The flexor tendon(s) were reconstructed with free tendon graft in 17 patients, cross-over transfer of flexor tendons from adjacent digits in two and buddying to an adjacent flexor tendon in one …

[Reconstruction of the extensor tendons in revision total knee arthroplasty and tumor surgery].

Reconstruction of the extensor mechanism in extended revision after total knee replacement and tumor surgery remains a clinically relevant problem. Due to large tibial bone defects with resection of the extensor insertion area, the specific problem of patella ligament refixation frequently arises. Several biological approaches and augmentation techniques have been published. Most of these are associated with a high rate of revision surgery because of failed replacement of the extensor mechanism and unsatisfactory functional outcome. Surgical reconstruction of these tendon defects is complicate

Ulnar subluxation of the extensor tendons in elderly osteoarthritic females: a neglected diagnosis.

Ulnar subluxation of the extensor digitorum communis tendons at the metacarpophalangeal joints occurs rarely in the absence of rheumatoid disease or a history of trauma. Three elderly women presented with chronic ulnar subluxation of the extensor tendons of spontaneous onset. They did not have rheumatoid arthritis and had suffered no acute injury. Seven extensor tendon relocations were performed. The treatment options for this condition are discussed. Recognition of this condition is important if permanent loss of function and disability is to be avoided.

Multiple tendons of abductor pollicis longus muscle: a cadaveric study with clinical implications.

Abductor pollicis longus (APL) muscle is known to exhibit numerous variations. Variations in the number of tendons of APL muscle may be asymptomatic and are often incidental findings. The present case report, detected in a 42 year male cadaver, describes the APL muscle having three tendons. The presence of accessory tendons may be important in De Quervain's stenosing tendovaginitis. Anatomical knowledge of such variations may be clinically important for surgeons performing reconstructive hand surgeries in the dorsolateral region of the hand. In a wider perspective, variations in the number of …

Functional assessment after acute and chronic complete ruptures of the proximal hamstring tendons.

Nonoperative treatment of acute or chronic complete ruptures of the proximal hamstring tendons leads to functional impairment in sports activities. The objective of the study was to evaluate the functional status after primary and delayed surgical treatment including objective (isokinetic hamstring and quadriceps muscle testing, hamstring flexibility) and subjective parameters (overall satisfaction, postoperative sports level). A total of eight patients (six male, two female) with an average age of 40.0 years (range 23-60) were treated operatively by refixation of the ruptured tendons in anato…

Congenital absence of the juncturae tendini contributing to dislocation of the extensor tendons.

We report a patient with bilateral congenital absence of the juncturae tendini who developed dislocation of the extensor tendons of the long and ring fingers of both hands. Reconstruction of the juncturae tendini restored extensor function. We describe our technique and the results 9 months after surgery.

[An uncommon occupational accident: tuberculous tenosynovitis of the extensor tendons of the hand].

We present a case of tuberculous tenosynovitis of the extensor tendons of the hand. Our patient was a young doctor working in the respiratory medicine department. He was injured on the dorsal aspect of the hand with a needle used for pleural aspiration. The clinical features consisted of gradually swelling, mild pain and stiffness of the metacarpophalangeal joint. The diagnosis was made after synovectomy. Histological and bacteriological examinations revealed tuberculosis. Treatment consisted of synovectomy and appropriate antibiotics. The clinical course was excellent after one year follow-up…

[Evaluation of the function of the flexor and extensor tendons of the hand].

Evaluation of the function of flexor tendons is a complex problem with a lot of controversies. Many propositions have been done since the first quotation of Boyes in 1950. Were are making a presentation and a critical analysis of the most used methods: T.A.M., Kleinert's classification, and the one described by Buck-Gramcko. We are presenting a personal method, useful in every zone, eliminating the action of the intrinsics and showing certain tenodesis effects. This method has been compared to those of Kleinert, Buck Gramcko and Strickland, using the analysis of the results of 165 tendon sutur…

A study of the dynamic anatomy of extensor tendons and implications for treatment.

This study correlates the excursion of the extrinsic finger extensors in zones V, VI, and VII and the extensor pollicis longus tendon in zones T IV and T V ("T" is used to designate thumb zones) with joint motion. Excursion was estimated by review of the literature, biomechanic calculations, and intraoperative observations. A simple equation determines extrinsic extensor tendon excursion in zone V, VI, and VII. This equation is substantiated by Brand's work on radians and by intraoperative studies. Guidelines are suggested for controlled motion for extrinsic finger extensors and the extensor p

[Personal technique for suturing the flexor tendons of the fingers with immediate mobilization].

Immediate mobilization of flexor tendons repairs creates a conflict between muscle strength and suture solidity. Non removable sutures are brittle and require to prevent extension by means of either a cast in flexion of the wrist and of the metacarpo-phalangeal joints or a rubber band traction. Pull-out sutures do not permit early mobilization because of their anchorages. The authors present a new technique they have been using for the last five years. It consists in a simple solid slip-knot which tightening the tendon when the muscle pulls. This knot is fixed at the distal bone insertion of t…

[Echocardiographic features of false tendons: with special reference to phonocardiographic significance (author's transl)].

Echocardiographic features consistent with the findings of false tendons (FTs) were described in five out of 1,000 consecutive cases, and they were studied in order to determine whether FT was responsible for the systolic murmurs. Three had heart diseases including aortic regurgitation, 3 degrees AV-block with aortic regurgitation, and pericarditis and mitral stenosis, and the remainder cases had no heart disease. M-mode echocardiograms showed abnormal linear echoes in the outflow tract of the left ventricle in three cases, and in the left ventricle toward the apex in another two cases. Two-di…

Abnormal flexion of the corono-pedal joint or "contracted tendons" in unweaned foals.

A condition frequently misnamed "contracted tendons" is described in unweaned foals. Various theories regarding its aetiology are examined. Overfeeding and lack of exercise are suggested as being the most likely causes, leading to excessive growth of the long bones. An effective method of treatment is described. "Contracted tendons" in yearlings are also discussed.

Reliability and Repeatability of Diffusion Tensor Imaging in Healthy and Pathological Patellar Tendons.

Patellar tendinopathy and bone-patellar tendon-bone autograft harvest for anterior cruciate ligament reconstruction are tendon injuries that impact long-term knee health. Diffusion tensor imaging (DTI) is a non-invasive magnetic resonance imaging (MRI) based approach with the potential to assess tendon microstructure. This study aimed to determine the inter-rater reliability of segmentations and test-retest repeatability of DTI metrics in pathological and contralateral patellar tendons. Ten participants received two bilateral knee MRI scans within a 7-day period. 3D CUBE proton density weighte…

Aged tendons have impaired mechanosensitivity and lower thresholds for injury under dynamic compression.

Rotator cuff tendinopathy is highly prevalent in aging populations, yet the mechanisms leading to age-dependent tendon degeneration are not well understood. In addition to tensile loading, tendons are subjected to compressive forces at certain anatomical sites (e.g., Achilles, rotator cuff), where altered adaptive responses may contribute to degenerative remodeling. The objective of this study was to investigate age-related differences in tendon responses to dynamic compressive loading using an ex vivo model. Murine flexor tendon explants from young and aged animals were cultured in a biaxial …

Novel approaches to extensor tendons: Strong surgical repairs with flexor tendon repair principles.

Extensor tendons in the dorsal hand especially at or close to the metacarpophalangeal joint area and those of the thumb are solid and round in cross-sections. A survey shows that most surgeons are using less strong surgical repair methods, often with over-protection after surgery. In this article, strong surgical repairs (6-strand repairs) are recommended for these areas based on author's experience. These tendons are treated with surgical methods and principles of current flexor tendon surgery. The finger is protected with short splinting with early active motion or hand use. After the extens…
