Experiences and Views of Dutch Protestant Pastors with Palliative Sedation in Their Pastoral Care at the End of Life.

There is a growing public support for medical interventions at the End of Life (EoL), such as euthanasia and physician assisted suicide (EPAS) and palliative sedation (PS). This paper explores pastors' views and their pastoral care on the place of EPAS and PS and uses a 2.556 pastors' survey. Whereas only 38% were in favour of EPAS, more than 90% could justify PS. This paper analyses the differences in pastors' attitudes regarding EPAS versus PS.

Palliative care in brain tumors.

The diagnosis of a brain tumor is a life-changing event for patients and families. High-grade gliomas (especially glioblastomas) are incurable, and long-term survival is limited. Metastatic brain lesions comprise the majority of intracranial neoplasms and are a significant source of morbidity and mortality in patients with systemic cancer. Response to treatment, prognosis, and survival depends not only on the underlying pathology, but more importantly on recently defined molecular features. Other crucial predictors of survival include age and performance status. Among patients with primary bra

Palliative noninvasive ventilation in patients with acute respiratory failure.

Over the last two decades, the increasing use of noninvasive ventilation (NIV) has diminished the need for endotracheal ventilation, thus decreasing the rate of ventilation-induced complications. Thus, NIV has decreased both intubation rates and mortality rates in specific subsets of patients with acute respiratory failure (e.g., patients with hypercapnia, cardiogenic pulmonary edema, immune deficiencies, or post-transplantation acute respiratory failure). NIV is also increasingly used as a palliative strategy when endotracheal ventilation is deemed inappropriate. In this context, palliative N

End-of-Life Care in the Austere Military Environment.

INTRODUCTION: Throughout the Global War on Terror (GWOT), the U.S. military built a revolutionary capacity to deliver life-saving care-even in austere environments-through rapid evacuation, far-forward providers and resources, and advances in prehospital and trauma-critical care. However, the operational reality of future Large Scale Combat Operations (LSCO), as evidenced by wars in Ukraine, Sudan, and Gaza, will be marked by high casualty rates and limited medical resources. The number of patients with non-survivable injuries because of operational constraints such as inability to evacuate, m

Digital legacy in palliative care: A concept analysis with Walker and Avant's approach.

BACKGROUND: The increasing digitalization of everyday life means that digital artifacts persist after death, presenting unique and complex challenges in palliative care. Lacking clear conceptualization, digital legacy raises clinical, ethical, and legal uncertainties. Conceptual clarity is crucial to support palliative care teams in addressing digital legacy issues at the end of life. AIM: To examine and clarify the concept of digital legacy in palliative care. DESIGN: A concept analysis was conducted using Walker and Avant's eight-step method. DATA SOURCES: Seven electronic databases (CINA

Red blood cell transfusions in advanced cancer near the end-of-life: narrative review.

BACKGROUND: Red blood cell (RBC) transfusions are commonly used to manage anemia-related symptoms, such as fatigue and dyspnea in patients with advanced cancer. Their use in palliative care (PC) remains controversial due to the lack of clear guidelines, potential risks, and ethical considerations. OBJECTIVE: This narrative review aims to evaluate the role of RBC transfusions in patients with advanced cancer and those at the end of life. METHODS AND DATA SOURCES: A comprehensive review of relevant existing literature was performed in various databases, using combinations of relevant keywords.

A Systematic Review of Worldwide Guidelines, Protocols, and Health Care Standards for Provision of Palliative Care Services in Progressive Neurological Disorders.

BACKGROUND: Progressive neurological disorders (PNDs) are life-limiting conditions with complex trajectories. PNDs require holistic, person-centred care that responds to the needs of both individuals and their families, including the provision of palliative and end-of-life (EOL) care. AIMS: This systematic review aimed to identify and synthesize existing international guidelines addressing palliative and EOL care in adult PND populations, with particular attention to their quality and scope. METHODS: We searched databases and gray literature sources for guidelines published between January 2

Expanding End-of-Life Options for Incarcerated Individuals.

The aging prison population in the United States presents urgent challenges for providing appropriate end-of-life (EOL) care. Currently, there are two primary approaches to deliver this in a compassionate manner: medical release programs (MRPs) and prison-based end-of-life care programs (EOLCPs). Using New York State as a case study, we argue expanded MRPs represent the most ethical path forward, while acknowledging the continuing need for prison-based EOLCPs. Our analysis reveals significant barriers to accessing medical release, including lack of knowledge about the application processes, pr

End-of-Life Care of Persons with Alzheimer's Disease and Other Dementias.

End-of-life (EOL) care has been a common option for patients with terminal medical conditions such as cancers. However, the utilization of EOL care in Alzheimer disease and other dementing conditions have become available relatively recently. As the end-stage dementia approaches, the clinicians and caregivers become faced with numerous clinical challenges-delirium, neuropbehavioral symptoms, the patient's inability to communicate pain and associated discomfort, food refusal, and so on. In addition to providing quality clinical care to the EOL patients, clinicians should pay special attention t

Surrogate Wars: The "Best Interest Values" Hierarchy & End-of-Life Conflicts with Surrogate Decision-Makers.

Conflicts involving end-of-life care between healthcare providers (HCPs) and surrogate decision-makers (SDMs) have received sustained attention for more than a quarter of a century, with early studies demonstrating a frequency of HCP-SDM conflict in ICUs ranging from 32-78% of all admissions (Abbott et al. 2001; Breen et al. 2001; Studdert et al. 2003; Azoulay et al. 2009). More recent studies not only acknowledge the persistence of clinical conflict in end-of-life care (Leland et al. 2017), but they have begun to focus on the ways in which these conflicts escalate to verbal or physical violen

Death and Dying in Carceral America: The Prison Hospice as an Inverted Space of Exception.

In America's prisons, long-term incarceration carries fears that one could die in prison alone and abandoned. Death by incarceration looms as inescapable for myriad inmates who are terminally ill or "graying" in prison. These realities inform this study of a prison-based hospice program staffed by male inmate volunteers in a mixed medium/maximum security facility. Of special concern are the experiences of the men who sit by the bedside of others who are dying. I begin with the assumption that prisons loom as states of exception, epitomized by the realities of substandard prison medicine, the d
