Recurrent chylopericardium in a patient with history of left leg sarcoma.

Chylopericardium is a rare condition that is characterized by the presence of chyle in the pericardial space due to idiopathic (primary) or secondary causes. We present a 43-year-old female with hypothyroidism and left lower extremity sarcoma which were treated with resection and adjuvant chemoradiotherapy that was found with moderate pericardial effusion in surveillance tests. The patient was initially treated with tube thoracostomy and conservative management but presented with recurrence. Eventually, she was treated with left thoracotomy, ligation of the thoracic duct and pericardiectomy.

[Chylothorax after blunt thoracic trauma].

The diagnosis of chylothorax following blunt chest trauma is rare, only few cases have been reported. We describe three patients with chylothorax following blunt chest trauma. Conservative treatment consists of drainage, in severe cases mechanical ventilation with PEEP and total parenteral nutrition. In case of persisting and/or increasing chylus production, thoracotomy and ligation of the thoracic duct may be required. In all of our patients thoracostomy was the definite therapeutic modality, no thoracotomy was necessary.

Spontaneous pneumothorax following caesarean section under spinal anaesthesia.

It is unusual for pneumothorax to occur spontaneously during pregnancy. Its occurrence during or following caesarean section is extremely uncommon with only three other cases reported previously. In none of the reported cases, was the caesarean section performed under spinal anaesthesia. We report the successful management of a multigravida female patient, who developed spontaneous pneumothorax following caesarean section, performed under spinal anaesthesia. Tube thoracostomy was required for management and the patient had an uneventful recovery.

[Airway obstruction and massive subcutaneous emphysema].

Airway obstruction caused by massive subcutaneous emphysema (MSE) in the chest and neck region is a rare complication to tension pneumothorax. We describe the condition in a 91-year-old male in whom rapidly progressing MSE caused airway obstruction. As a result of progressing airway obstruction acute intubation was performed prior to completion of tube thoracostomy resulting in immediate improvement of the condition. This case demonstrates that MSE can lead to acute airway obstruction. Rapid intubation can in this situation be required to prevent asphyxiation.

Calcified pleural plaque mimicking a traumatic pneumothorax.

Pneumothorax is a common presentation to the emergency department, arising from traumatic and non-traumatic aetiologies. Diagnosis of non-tension pneumothorax is by a visible lung edge on chest radiography together with the absence of lung markings more peripherally. The Advanced Trauma Life Support (ATLS) system recommends tube thoracostomy as the definitive treatment for traumatic pneumothorax. The case is described of a patient who presented following thoracic trauma with an unusual finding on chest radiography that led to diagnostic confusion by mimicking a pneumothorax.

[The usefulness of thoracoscopic surgery for pleuritis carcinomatosa].

We performed thoracoscopic surgery for pleuritis carcinomatosa, in which re-expansion of the collapsed lung was impossible, by employing tube thoracostomy. One of the treatments used for pleuritis carcinomatosa was drainage of the pleural effusion followed by re-expansion of the collapsed lung. The decortication and biopsy of the pleura and the pleural effusion drainage with the thoracoscopic procedure were performed, allowing control of the pleural effusion and re-expansion of the collapsed lung to be accomplished. We conclude that thoracoscopic surgery is useful for the thoracic treatment of…

Video-assisted thoracic surgery of a caudal mediastinal paraoesophageal abscess in a cat with recurrent pyothorax.

CASE SUMMARY: A 6-year-old spayed female domestic shorthair cat was presented with acute respiratory distress and lethargy. Three months prior, the cat had been medically managed for a bacterial pyothorax. Initial stabilisation included oxygen supplementation, and a point-of-care ultrasound (POCUS) revealed bilateral pleural effusion. A thoracostomy tube was placed and pleural fluid was aseptically collected. Cytological evaluation of the pleural fluid was consistent with a septic neutrophilic exudate, with Fusobacterium russii, Bacteriodes pyogenes and Porphyromonas gingivalis isolated on ana…

Management of traumatic occult pneumothorax.

STUDY OBJECTIVE: Occult pneumothorax (OPTX) is defined as a pneumothorax seen on computed tomography but not apparent on supine plain radiography. Though increasingly common, the acute management of OPTX after trauma remains controversial. This evidence-based review evaluates the existing evidence regarding the safety and efficacy of observation as compared to tube thoracostomy (TT) for management of OPTX in emergency department trauma patients. METHODS: The authors searched MEDLINE, EMBASE, the Cochrane Library, and other databases. INCLUSION CRITERIA: studies of adult or pediatric trauma v…

The diagnosis and treatment of non-cardiac thoracic trauma.

Penetrating and blunt force mechanisms frequently result in thoracic trauma. Thoracic injuries cover the spectrum from trivial to lethal, and more than half are associated with head, abdomen or extremity trauma. Fortunately over eighty percent of injuries can be managed non-operatively utilizing tube thoracostomy, appropriate analgesia and aggressive respiratory therapy. Patients requiring emergency thoracotomy are either in shock or have life threatening injuries and, as expected, have significant mortality and morbidity. Injury to the thorax directly accounts for approximately 25% of trauma …

Conservative versus surgical management of chylothorax.

Chylothorax is a potentially life-threatening disorder that has profound respiratory, nutritional, and immunologic consequences. Treatment for this problem is controversial, and the results have been variable. From 1985 to 1990, 29 patients (16 males, 13 females; mean age: 20.1 years; range: 5 days to 76.8 years) were diagnosed as having chylothorax (18 right, 6 left, 5 bilateral). Etiologies included surgical trauma (26) and nonsurgical trauma (3). Initial conservative treatment consisted of tube thoracostomy drainage (mean duration: 13.3 days; range: 1 to 62 days; mean total volume: 4,030 mL…

Serratus Anterior Plane Block in the Emergency Department: A Case Series.

This is a case series of six emergency department (ED) patients who received an ultrasound-guided serratus anterior plane block (SAPB) for a variety of painful conditions. Our cases illustrate the feasibility and analgesic efficacy of the SAPB in providing pain management in ED patients with a variety of painful syndromes, including those with severe pain from multiple rib fractures, herpes zoster, and tube thoracostomy placement. In addition, we found no adverse events in our case series.

Biliopleural fistula: a rare complication of percutaneous transhepatic gallbladder drainage.

A 79-year-old previously healthy man presented with acute acalculous cholecystitis with obstruction of the biliary tract. He was successfully treated with antibiotics and percutaneous transhepatic gallbladder drainage, but returned to the hospital two days after discharge with a rare complication of this technique, biliopleural fistula. A thoracostomy tube was inserted to drain the pleural effusion, and the patient's previous antibiotics reinstated. After two weeks of drainage and antibiotics, the fistula healed spontaneously without the need for further intervention.

"Pneumo-ptosis" in the emergency department.

The authors report the case of a 16-year-old female who presented with a left tension pneumothorax and a left Horner's syndrome. Chest tube thoracostomy performed to relieve the tension pneumothorax also resulted in the immediate resolution of the patient's ptosis and miosis. The probable mechanism for the patient's focal neurologic signs was traction upon the cervical sympathetic chain secondary to the mediastinal shift of the tension pneumothorax. This case demonstrates that unequal pupils in the presence of a pneumothorax could represent Horner's syndrome.

Management of Complications After Lung Resection: Prolonged Air Leak and Bronchopleural Fistula.

Prolonged air leak or alveolar-pleural fistula is common after lung resection and can usually be managed with continued pleural drainage until resolution. Further management options include blood patch administration, chemical pleurodesis, and 1-way endobronchial valve placement. Bronchopleural fistula is rare but is associated with high mortality, often caused by development of concomitant empyema. Bronchopleural fistula should be confirmed with bronchoscopy, which may allow bronchoscopic intervention; however, transthoracic stump revision or window thoracostomy may be required.

Chylothorax after off-pump coronary artery bypass graft surgery: Management strategy.

Chylothorax is a rare complication after cardiac surgery but is associated with morbidity and mortality. The most common cause of chylothorax is damage to or avulsion of thoracic duct by electrocautery during left internal thoracic artery harvesting for coronary artery bypass graft (CABG) surgery. We describe a case of chylothorax after off-pump CABG, which was successfully treated with thoracostomy tube drainage, withholding of oral intake, total parenteral nutrition and subcutaneous octreotide, a somatostatin analog, and chemical pleurodesis.

Management of Blunt Chest Trauma.

Common mechanisms of blunt thoracic injury include motor vehicle collisions and falls. Chest wall injuries include rib fractures and sternal fractures; treatment involves supportive care, multimodal analgesia, and pulmonary toilet. Pneumothorax, hemothorax, and pulmonary contusions are also common and may be managed expectantly or with tube thoracostomy as indicated. Surgical treatment may be considered in select cases. Less common injury patterns include blunt trauma to the tracheobronchial tree, esophagus, diaphragm, heart, or aorta. Operative intervention is more often required to address t…
