Use of a new vertical traction device for early traction-assisted staged closure of congenital abdominal wall defects: a prospective series of 16 patients.

PURPOSE: Abdominal wall closure in patients with giant omphalocele (GOC) and complicated gastroschisis (GS) remains to be a surgical challenge. To facilitate an early complete abdominal wall closure, we investigated the combination of a staged closure technique with continuous traction to the abdominal wall using a newly designed vertical traction device for newborns. METHODS: Four tertiary pediatric surgery departments participated in the study between 04/2022 and 11/2023. In case primary organ reduction and abdominal wall closure were not amenable, patients underwent a traction-assisted abd…

Use of a Minimally Invasive Traction Repositor versus Conventional Manual Traction for the Treatment of Tibial Fractures: A Comparative Study from a Tertiary Hospital in China.

BACKGROUND: Closed reduction and intramedullary nail fixation of tibial fractures may not utilize a fracture table or reduction aids like a femoral distractor, and only manual traction will help aid the reduction process. This study aimed to describe and further investigate the effectiveness of an originally designed minimally invasive traction repositor (MITR) for the treatment of tibial fractures. METHODS: From January 2018 to April 2021, a total of 119 eligible patients with tibial shaft fractures were included and retrospectively assigned to two groups according to different reduction met…

A comparative evaluation of the efficacy between skeletal traction and skin traction in pre-operative management of femur shaft fractures in Korle Bu Teaching Hospital.

OBJECTIVES: This study is to compare the outcomes of pre-operative skeletal and skin traction in adult femoral shaft fractures awaiting surgical fixation within two weeks of presentation to the Accident Center of Korle Bu Teaching Hospital. METHODS: This study was a clinical trial on 86 recruited patients with closed femoral shaft fractures sustained within 24 hours of presentation grouped into 2 groups. Descriptive and inferential statistics comprising frequency, percentage, Chi-square, independent sample t-test and Mann-Whitney U test were used in analysing the data. RESULTS: Of the total …

Sebaceous Extrusion Sign: A Novel Histopathologic Clue in Traction Alopecia.

Tractional alopecia is a form of hair loss caused by prolonged tension on hair follicles, typically resulting from tight hairstyles. Although histopathological features of TA have been described, differentiation from other alopecias remains challenging. In this case series, we report a novel histopathological finding (Kamyab sign) characterized by the extrusion of sebaceous glands through the follicular infundibulum onto the skin surface.

Pneumocystis Pneumonia Presenting With Peripheral Predominant Consolidation and Traction Bronchiectasis.

Pneumocystis jirovecii pneumonia (PCP) typically presents as a predominant ground-glass opacity (GGO) in the upper lobes. We report a case of a patient with PCP that mimicked organizing pneumonia or nonspecific interstitial pneumonia, showing peripheral predominant consolidation with traction bronchiectasis and peribronchovascular thickening in the lower lobes on high-resolution computed tomography (HRCT). Pneumocystis jirovecii was detected in bronchoalveolar lavage (BAL), and no other pathogens were isolated. After confirmation of a high plasma human immunodeficiency virus (HIV)-RNA titer an…

Optimizing pediatric laparoscopic cholecystectomy: trocar reduction with percutaneous gallblader traction.

OBJECTIVE: Laparoscopic cholecystectomy is typically carried out using four ports. The objective of this study was to describe the percutaneous gallbladder traction technique, which allows the number of ports to be reduced, thus optimizing the procedure, with a similar exposure of the surgical site. MATERIALS AND METHODS: 2 pediatric patients with symptomatic cholelithiasis underwent laparoscopic surgery from January 2021 to January 2024. Cholecystectomy was performed using an umbilical port (5 mm, 30-degree scope) and two accessory ports. A laparoscopic loop with a running knot was used to c…

[Vitreomacular traction: diagnostics, natural course, treatment decision and guideline recommendations].

Vitreomacular traction is a tractive foveolar adhesion of the posterior vitreous limiting membrane, resulting in pathological structural alterations of the vitreomacular interface. This must be differentiated from physiological vitreomacular adhesion, which exhibits a completely preserved foveolar depression. Symptoms depend on the severity of the macular changes and typically include reduced visual acuity, reading problems and metamorphopsia. High-resolution spectral domain optical coherence tomography (SDOCT) imaging enables classification of the sometimes only subtle morphological changes. …

Preoperative cervical traction with Gardner-Wells tongs: who profits most?

BACKGROUND: Preoperative traction with the Gardner-Wells tongs (PTGWT) is a valuable option for cervical spine injuries with malalignment. The aim of this study was to analyze the factors related to the treatment success of PTGWT. METHODS: All consecutive cases with PTGWT due to cervical spine injury with malalignment treated between 01/2010 and 09/2020 were included. Patients' records were reviewed for demographic and clinical characteristics. Treatment success was evaluated upon the angle correction in the sagittal plane using the computed tomography scans before and after the treatment. R…

Counseling About Traction Alopecia: A "Compliment, Discuss, and Suggest" Method.

Traction alopecia (TA) remains a common concern for Black women visiting a dermatologist. Understanding cultural hair care practices, timely diagnosis, and discontinuation of high-risk hairstyles are essential for preventing TA from progressing to scarring alopecia. This dialogue might be difficult for hair-discordant physicians and patients because of a lack of training and shared experiences. We present an empathetic and culturally sensitive method for discussing TA with patients.

Viscoelastic substrate decouples cellular traction force from other related phenotypes.

Survival and maintenance of normal physiological functions depends on continuous interaction of cells with its microenvironment. Cells sense … in Yes-Associated Protein (YAP). Our results show that substrate viscoelasticity decouples cellular traction from other known traction related phenotypes.

Clinical Features of Vitreomacular Traction Syndrome with Peripheral Vitreoretinal Adhesion.

PURPOSE: To retrospectively review and analyze the clinical and imaging data of patients with vitreomacular traction syndrome (VMTS) with strong peripheral vitreoretinal adhesion. SUBJECTS AND METHODS: This retrospective case-series study involved 4 eyes of 4 VMTS patients with vitreoretinal adhesion in both the macular region and the periphery who underwent vitreous surgery. In all 4 cases, preoperative refraction, fundoscopic findings, optical coherence tomography (OCT) findings, and intraoperative findings were evaluated. RESULTS: The preoperative fundoscopy and OCT findings revealed fibr…
