[Factors increasing life threat in patients with lesions of the spinal cord].

Mortality of spinal cord trauma has been analysed. An analysis included 3,486 patients treated early after spinal trauma in 1965-1989. Life threat in patients, who underwent trauma to the spinal cord embraces several factors. The most important is are: the level and degree of the lesion to the spinal cord, certain causes precipitating severe lesions to the nervous system, advanced age, and--to some degree--accompanying lesions, way of therapy, advances in medical technology, and intensive treatment technics. Available data indicate that the life of patients admitted to the hospital with sympto

da Vinci robotic-assisted micro-space dissection and autonomic nerve network preservation technique in the total mesorectal excision procedure for rectal cancer: a single-center, retrospective, observational, real-world study.

Completely preserving the abdominopelvic autonomic nervous system technique during da Vinci robot-assisted rectal cancer surgery remains a challenge for surgeons. Anatomical studies reveal that the abdominopelvic autonomic nervous system resides within the prehypogastric nerve fascia sheath, exhibiting a reticular distribution with accompanying blood vessels surrounding neural fibers. This study proposes a novel autonomic nerve network preservation technique and delineates its technical nuances in robot-assisted total mesorectal excision (TME) for rectal cancer. The enhanced magnification capa

[Adult neurogenesis: from basic research to clinical applications].

Neural stem cells have recently been found in the central nervous system of adult rodents and humans. In defined conditions, these multipotent cells can generate the three major cell types of the nervous system (neurons, oligodendrocytes and astrocytes). These findings raise questions on the functional role of neural stem cells in the adult brain, and point to the possibility of novel therapeutic approaches. We have been investigating the functional consequences of neural stem cells for the adult circuits of the olfactory system. We are currently investigating this unexpected juvenile characte

A Lesson Learned in Managing Traumatic Cervical Neurofibromatosis Type 1.

Neurofibromatosis type 1 (NF-1) is also known as von Recklinghausen disease. It is a rare autosomal dominant disorder that affects the skin and nervous system. The head and neck region is one of the potential areas for the involvement of neurofibromatosis due to the unique anatomical compartmentalization of the central and peripheral nervous systems. Neurofibromas in the head and neck region are primarily found within the soft tissues. NF-1 can be diagnosed during childhood because characteristic cutaneous manifestations are typically present during childhood. In this report, we discuss a case

T-lymphocyte tyrosine hydroxylase regulates TH17 T-lymphocytes during repeated social defeat stress.

Posttraumatic stress disorder (PTSD) is a debilitating psychiatric disorder which results in deleterious changes to psychological and physical health. Patients with PTSD are especially susceptible to life-threatening co-morbid inflammation-driven pathologies, such as autoimmunity, while also demonstrating increased T-helper 17 (TH17) lymphocyte-driven inflammation. While the exact mechanism of this increased inflammation is unknown, overactivity of the sympathetic nervous system is a hallmark of PTSD. Neurotransmitters of the sympathetic nervous system (i.e., catecholamines) can alter T-lympho

Membrane leakage in spinal ganglion nerve cells induced by experimental whiplash extension motion: a study in pigs.

Nerve cells in the cervical and upper thoracic spinal ganglia were examined for possible plasma membrane leakage, as revealed by their ability to exclude a dye-protein complex, after experimentally induced whiplash in a pig model system. The rationale for this approach is found in the fact that the interstitium of spinal ganglia differs from most other parts of the nervous system in that it lacks a barrier, allowing blood constituents to gain access. The dye Evans blue, which rapidly conjugates with blood proteins, is found in the interstitium of normal spinal ganglia after intravenous injecti

Stress-induced hemostasis: mechanisms and implications for health.

Stress, encompassing both psychological and physical dimensions, profoundly affects the hemostatic system, which is responsible for maintaining the delicate balance between blood fluidity and clot formation. The interplay between stress and hemostasis involves intricate mechanisms where stress hormones like cortisol, adrenaline, and noradrenaline interact with coagulation factors and platelets, promoting a hypercoagulable state. This review delves into the various pathways through which stress influences hemostasis, highlighting the critical roles of the sympathetic nervous system, the hypotha

Function-triggering antibodies to the adhesion molecule L1 enhance recovery after injury of the adult mouse femoral nerve.

L1 is among the few adhesion molecules that favors repair after trauma in the adult central nervous system of vertebrates by promoting neuritogenesis and neuronal survival, among other beneficial features. In the peripheral nervous system, L1 is up-regulated in Schwann cells and regrowing axons after nerve damage, but the functional consequences of this expression remain unclear. Our previous study of L1-deficient mice in a femoral nerve injury model showed an unexpected improved functional recovery, attenuated motoneuronal cell death, and enhanced Schwann cell proliferation, being attributed

Sexual function in adults with a history of childhood maltreatment: Mediating effects of self-reported autonomic reactivity.

OBJECTIVE: A growing body of literature documents sexual problems following maltreatment and traumatic experience, but the mechanisms of these effects are poorly understood. The autonomic nervous system coordinates typical and threat-reactive functions throughout the body, including those of reproductive organs. We examined whether relations between adult sexual function problems and childhood maltreatment history could be mediated by an autonomic nervous system retuning with a bias toward maintaining a physiological state that supports defensive strategies. METHOD: Self-reported data on chil

[CENTRALIZED PAIN AND FIBROMYALGIA: WHAT DO WE MEAN WHEN WE SAY "IT'S ALL IN YOUR HEAD"?].

Fibromyalgia is a syndrome characterized by chronic widespread musculoskeletal pain, tenderness, fatigue and associated symptoms which include cognitive difficulties, abdominal symptoms etc. A leading paradigm regarding the pathogenesis of fibromyalgia (and similar functional disorders) focuses on the concept of central sensitization. This concept describes a situation in which there is an increased sensitivity of the central nervous system to the processing and transmission of pain, leading to the development of clinical phenomena such as allodynia and hyperalgesia. Various lines of evidence

Heart rate variability in patients with post-traumatic stress disorder or borderline personality disorder: relationship to early life maltreatment.

Traumatic experiences have severe impact on the autonomous nervous system. Heart rate variability (HRV) is a reliable psychophysiological marker for the autonomous nervous system functioning. Reduced vagally mediated HRV has been found in patients with post-traumatic stress disorder (PTSD) and, in some studies, in patients with borderline personality disorder (BPD). In this study, we compared HRV parameters of patients with PTSD, current BPD, and BPD in remission with healthy volunteers in a 5 min resting-state electrocardiogram recording. 91 unmedicated female participants took part in the st

The clinical value of the palmomental reflex.

The age specific incidence of the palmomental reflex (PMR) has been investigated in persons without any former or present evidence of a neurological or mental involvement. Also excluded were persons with hypertension, diabetes mellitus, thyroid dysfunction, alcohol abuse and other diseases as these could effect the central or peripheral nervous system. From the newborn period up to the age of 20 years the incidence of the PMR was between 3,3% and 20%, the differences being statistically insignificant. After the age of 20 years the incidence of the PMR rises with increasing age, the rise being

Retinal ganglion cell expression of cytokine enhances occupancy of NG2 cell-derived astrocytes at the nerve injury site: Implication for axon regeneration.

Following injury in the central nervous system, a population of astrocytes occupy the lesion site, form glial bridges and facilitate axon regeneration. These astrocytes originate primarily from resident astrocytes or NG2+ oligodendrocyte progenitor cells. However, the extent to which these cell types give rise to the lesion-filling astrocytes, and whether the astrocytes derived from different cell types contribute similarly to optic nerve regeneration remain unclear. Here we examine the distribution of astrocytes and NG2+ cells in an optic nerve crush model. We show that optic nerve astrocytes

Isolated Nerve Grafting for a Young Patient with a Complete Common Peroneal Nerve Palsy Following a Traumatic Knee Dislocation: A case report.

BACKGROUND: Common peroneal nerve (CPN) injury following a knee dislocation is a serious problem, and an optimal treatment is yet to be established. We report a case of complete CPN palsy following a knee dislocation treated with sural nerve grafting. CASE: A 19-year-old man suffered a knee injury during a hurdle race. Diagnosis in a previous hospital revealed a complex ligament injury with CPN palsy. Ten weeks following injury, he was admitted to our institution because of a lack of neurological improvement. Considering the grade 0 results obtained in the manual muscle test (MMT) of tibialis

Review and clinical presentation in reverse shoulder arthroplasty in deltoid palsy.

Deltoid palsy is a classical contraindication for reverse shoulder arthroplasty (RSA). However, in cases associating axillary nerve palsy and rotator cuff tear or glenohumeral arthritis, few options remain. We present a case in which combining RSA with transfer of the pectoralis major and upper and middle trapezius transfer provided satisfactory results in a patient suffering of both an irreparable rotator cuff tear and a deltoid palsy.Level of evidence IV.

Bipolar radio frequency ablation of spinal neoplasms in late stage cancer disease: a report of three cases.

STUDY DESIGN: Case report. OBJECTIVE: To avoid neuronal damage by using the bipolar radio frequency ablation of spinal tumors. SUMMARY OF BACKGROUND DATA: Radio frequency ablation of tumorous masses is an established procedure and is increasingly used as pain therapy of unresectable spine tumors. Ablation of lesions adjacent to vulnerable structures remains a challenging task because flow of current is insufficiently controlled by monopolar probes. Using this technique, a prediction of the induced necrosis accurate to the millimeter is not feasible. METHODS: Three patients with metastases o

Rat sciatic nerve crush injury and recovery tracked by plantar test and immunohistochemistry analysis.

An experimental crush injury to the sciatic nerve, with a crush force of 49.2 N (pressure p=1.98x10(8) Pa), was inflicted in 30 male rats (Wistar). A control group (sham), with the same number of rats, was also operated upon exactly as the experimental group but without the crush injury. We tested the sensory and motor recovery of the sciatic nerve with Hargreaves method, using an apparatus from Ugo Basile, Italy. Testing was continued for both legs of each rat, injured and uninjured, starting preoperatively (0 day), and then 1, 7, 14, 21, and 28 days postoperatively. The same experiment was r

Zone of traction injury of the common peroneal nerve.

The history of functional motor recovery after reconstruction of traction injury to the common peroneal nerve is poor, regardless of technique and regardless of the experience of the surgeon doing the reconstruction. The hypothesis tested is that the failure of functional motor recovery after common peroneal nerve traction injury is because the zone of injury extends beyond the visible region of peroneal nerve in continuity injury and into the muscle entry zone of the motor nerve terminations. The opportunity arose to examine pathologically this suspected distal zone of injury in one patient.

Application of therapeutic hypothermia in the ICU: opportunities and pitfalls of a promising treatment modality. Part 1: Indications and evidence.

OBJECTIVE: Hypothermia has been used for medicinal purposes since ancient times. This paper reviews the current potential clinical applications for mild hypothermia (32-35 degrees C). DESIGN AND SETTING: Induced hypothermia is used mostly to prevent or attenuate neurological injury, and has been used to provide neuroprotection in traumatic brain injury, cardiopulmonary resuscitation, stroke, and various other disorders. The evidence for each of these applications is discussed, and the mechanisms underlying potential neuroprotective effects are reviewed. Some of this evidence comes from animal

Suprascapular nerve injury during spine surgery. A case report.

In long spine surgeries, knee-chest position-associated complications can occur. This report describes a case of suprascapular nerve injury that occurred during the lengthy surgical procedure. It is postulated that excessive scapula, shoulder girdle, and upper arm protraction occurred, resulting in a sling effect compression injury of the suprascapular nerve.
