What has intravascular ultrasound taught us about plaque biology?

Intravascular ultrasound (IVUS) has a defined role in the cardiac catheterization laboratory to assess lesion severity and the procedural success of vascular interventions. However, IVUS has also contributed to our understanding of the biology of atherosclerosis and restenosis. In acute coronary syndromes, IVUS has revealed varying degrees of stenosis, thrombosis, and plaque derangement typical of the plaque disruption seen in many pathologic studies of patients who have died of this condition. IVUS has demonstrated that the culprit lesions of patients surviving acute coronary syndromes also t

[Magnetic-field-based navigation system for ultrasound-guided interventions].

PURPOSE: To describe a new magnetic field-based navigation system for ultrasound-guided interventional procedures. METHODS: The navigation system supports biopsies either in plane or out of plane. To evaluate the accuracy of the system, targets in four standard silicone phantoms were punctured. Six cystic and six solid lesions were present in every phantom. The success of the puncture was controlled by aspiration of the content of the lesions, which was coloured. Furthermore, liver lesions of different sizes (2-6 mm) and depths (3-6 cm) were produced by injecting a viscous fluid and punctured

Assessing the effectiveness of 18F-Fluorocholine PET-CT as the leading imaging technique compared to 99mTc-Sestamibi scans and ultrasonography for diagnosing hyperparathyroidism.

BACKGROUND: Accurate preoperative localization of hyperfunctioning parathyroid tissue is paramount for the success of minimally invasive parathyroidectomy in primary hyperparathyroidism (PHPT). Conventional imaging with 99mTc-sestamibi (MIBI) scintigraphy and cervical ultrasonography (US) has significant limitations in sensitivity and specificity. This study aims to evaluate the diagnostic performance of 18F-fluorocholine PET/CT (FCH-PET/CT) as a preferred imaging modality compared with conventional methods. METHOD: ology: A prospective, non-interventional cohort study was conducted on 82 pat

Computed tomography, ultrasound and fluoroscopy guidance in intra-articular sacroiliac joint injections: A systematic review.

INTRODUCTION: Fluoroscopy and Computed Tomography (CT) are the reference standards for verifying needle placement when undertaking an Intra-articular (IA) Sacroiliac Joint (SIJ) injection. The use of Ultrasound (US) for performing SIJ injections has become a focus in recent years, as it eliminates radiation exposure. This systematic review aimed to compare the efficacy of US, CT, and Fluoroscopy when undertaking SIJ injections. METHODS: A systematic search was conducted using the databases Embase, PubMed, CINAHL, and Scopus. The included studies were published in English (January 2003 to Marc

Myocardial Bridging: An Up-to-Date Review.

Myocardial bridging is a congenital anomaly in which a segment of a coronary artery takes a "tunneled" intramuscular course under a "bridge" of overlying myocardium. This causes vessel compression in systole, resulting in hemodynamic changes that may be associated with angina, myocardial ischemia, acute coronary syndrome, left ventricular dysfunction, arrhythmias, and even sudden cardiac death. While described on autopsy for centuries, technological advances such as coronary computed tomography angiography and intravascular ultrasound have contributed greatly to our understanding of the anatom

Percutaneous image-guided splenic procedures: update on indications, technique, complications, and outcomes.

Percutaneous image-guided splenic procedures are seldom performed due to fear of complications, mainly hemorrhage. Percutaneous splenic invasive procedures are, however, safe, and in most cases, the complication rates are similar to those of other abdominal organs. In most patients, biopsy of a focal splenic lesion establishes the diagnosis. Aspiration and drainage of splenic collections are performed obviating the need for splenectomy. In this review we describe the indications, technique, complications, and outcomes of percutaneous splenic biopsy, aspiration, and drainage.

Complete resolution of an alveolar echinococcosis liver lesion following percutaneous treatment.

Herein we present a 63-year-old male patient with a solid hepatic alveolar echinococcosis diagnosed by surgical biopsy. His liver lesion, which was infected, was drained by percutaneous catheterization. The lesion surprisingly disappeared completely after the treatment. The patient was followed-up without any symptoms for 20 months after the drainage. As alveolar echinococcosis of the liver behaves like a slow-growing liver cancer, the disappearance of our patient's lesion was a very unusual and rare outcome, which, to the best of our knowledge, has never been published in the literature.

Standardized Protocol for Intravascular Ultrasound-Guided Lithotripsy in Severely Calcified Coronary Lesions.

Severe coronary artery calcification poses a clinical challenge during percutaneous coronary intervention, significantly increasing the risk of stent underexpansion and subsequent adverse cardiovascular events. While intravascular lithotripsy (IVL) uses acoustic pressure waves to modify calcified plaques, its use without precise intracoronary imaging can lead to suboptimal balloon sizing and unpredictable anatomical outcomes. The primary goal of the presented protocol is to detail a standardized, step-by-step methodology for utilizing intravascular ultrasound (IVUS) to guide IVL therapy in pat

Short- vs. long-axis ultrasound-guided injections for the proximal and distal parts of the posterior interosseous nerve: a cadaveric study.

PURPOSE: Posterior interosseous nerve (PIN) syndrome is an uncommon neuropathy that may mimic lateral epicondylitis. High-resolution ultrasound aids its diagnosis by revealing nerve swelling, while ultrasound-guided injection is increasingly employed for decompression. Nevertheless, the influence of injection axis on injectate distribution remains unclear. This study aimed to compare the spread characteristics and accuracy of short-axis vs. long-axis ultrasound-guided injections at the proximal and distal PIN using cadaveric validation.Kindly check and confirm the city name is correctly identi

Ultrasound-guided percutaneous cholecystostomy for acute cholecystitis: a systematic review and meta-analysis.

BACKGROUND: Ultrasound-guided percutaneous cholecystostomy (US-PC) is widely used in high-risk surgical patients with acute cholecystitis. Still, success and safety rates specific to US guidance are not always distinguished within mixed 'image-guided' series. OBJECTIVE: To estimate technical success, clinical success, and major complication rates of US-PC. METHODS: Primary studies explicitly reporting US-PC or with separable data were included. PRIMARY OUTCOMES: technical success, clinical success (as defined by the study), and major adverse events (AEs). Single-arm meta-analysis using rand

Ultrasound guided procedures in infectious diseases.

Retained foci of infection can represent an additional risk for patients, as they may not respond to antibiotics and require specific procedures to control the focus. This manuscript provides a practical approach to performing ultrasound-guided interventional procedures to recognise and drain infectious collections. The review details the main indications, technical steps, safety tips and clinical applications for five critical procedures: thoracentesis and chest drainage, paracentesis, arthrocentesis, pericardiocentesis and drainage of intra-abdominal abscesses (hepatic, splenic and renal). Vaginal Endometriosis Resection Guided by an Intra-Operative Transvaginal Ultrasound.

OBJECTIVE: Vaginal endometriosis occurs in 3.5% to 14.5% of patients with endometriosis [1-3]. Complete resection is crucial, as unnecessary colpotomy may increase the risk of rectovaginal fistula [4], and incomplete resection may leave residual disease, raising the likelihood of recurrence. Ultrasound is commonly used in the diagnosis and preoperative assessment of deep endometriosis [5]. This study demonstrates the use of intraoperative transvaginal ultrasound to guide safe and complete vaginal endometriosis resection. SETTING: An academic-affiliated healthcare center.
