Somatoform abdominal pain in surgery: is SD worthy of surgical attention? Case reports and literature review.

Somatoform disorders (SD) or medically unexplained physical symptoms (MUPS) are a group of disorders that represent a group of symptoms that cannot be explained by an organic or physical pathology. These disorders are widely prevalent, and, if unrecognised, SD may lead medical professionals to embark on tests or procedures which may inflict unnecessary iatrogenic complications. Despite the high prevalence, they are only poorly included in medical training curricula, at both undergraduate and postgraduate levels. In this article, we review the literature and present two cases. The first one pre

Differentiating benign and malignant solitary pulmonary nodules through exhaled breath analysis.

BACKGROUND: Lung cancer remains the leading cause of cancer-related deaths worldwide, with early detection significantly improving survival. Lung nodules are a common finding, both as incidental solitary pulmonary nodules (SPNs) and in lung cancer screening programmes. Accurately distinguishing benign from malignant nodules, particularly small ones, remains challenging. Determining which nodules require further investigation is crucial for optimising early lung cancer detection and reducing unnecessary procedures. Therefore, volatile organic compounds (VOCs) in exhaled breath are analysed usin

Assessing the rates of false-positive ovarian cancer screenings and surgical interventions associated with screening tools: a systematic review.

OBJECTIVE: Early detection of ovarian cancer can improve patient outcomes; however, screening tests can yield false-positive results, leading to unnecessary surgical interventions. This systematic review explores the prevalence of false-positive ovarian cancer screenings and subsequent unnecessary surgical interventions. METHODS AND ANALYSIS: Five databases were searched in March 2023 and again in March 2024, encompassing primary literature published between 2003 and 2024. Data collection focused on studies reporting the number of surgical interventions resulting from a false-positive screeni

Peritoneal tuberculosis impersonating ovarian malignancy: A case series from a high tuberculosis burden country.

An uncommon form of extrapulmonary tuberculosis is abdominal tuberculosis (TB). Due to its vague symptoms, abdominal TB remains difficult to diagnose. We report three cases of peritoneal TB with abdominal enlargement in Bandung, Indonesia, which were initially misdiagnosed as ovarian carcinoma and treated with biopsy and laparotomy. To avoid unnecessary invasive procedures that pose risks to patients, we assess the need for a diagnostic strategy to differentiate between ovarian cancer and abdominal TB based on our experiences and existing literature. Peritoneal TB closely mimics ovarian cancer

NLP-Driven Analysis of Pneumothorax Incidence Following Central Venous Catheter Procedures: A Data-Driven Re-Evaluation of Routine Imaging in Value-Based Medicine.

Background: This study presents a systematic approach using a natural language processing (NLP) algorithm to assess the necessity of routine imaging after central venous catheter (CVC) placement and removal. With pneumothorax being a key complication of CVC procedures, this research aims to provide evidence-based recommendations for optimizing imaging protocols and minimizing unnecessary imaging risks. Methods: We analyzed electronic health records from four university hospitals in Salzburg, Austria, focusing on X-rays performed between 2012 and 2021 following CVC procedures. A custom-built NL

Routine Pathology Testing Prior to Bronchoscopy has Limited Clinical Impact With Significant Economic and Environmental Costs.

BACKGROUND: Routine preprocedural pathology testing is no longer recommended for minor and intermediate-risk elective procedures. British Thoracic Society guidelines advise hemoglobin, platelet count, and coagulation studies only in patients with risk factors for bleeding. Despite this, such tests are commonly ordered without a clear clinical indication before bronchoscopic procedures, incurring unnecessary financial and environmental costs. METHODS: We conducted a retrospective review of outpatient bronchoscopic procedures over a 12-month period at an Australian tertiary hospital. Preprocedu

Optimizing Thyroid Nodule Management With Artificial Intelligence: Multicenter Retrospective Study on Reducing Unnecessary Fine Needle Aspirations.

BACKGROUND: Most artificial intelligence (AI) models for thyroid nodules are designed to screen for malignancy to guide further interventions; however, these models have not yet been fully implemented in clinical practice. OBJECTIVE: This study aimed to evaluate AI in real clinical settings for identifying potentially benign thyroid nodules initially deemed to be at risk for malignancy by radiologists, reducing unnecessary fine needle aspiration (FNA) and optimizing management. METHODS: We retrospectively collected a validation cohort of thyroid nodules that had undergone FNA. These nodules

The role of microflow patterns combined with greyscale ultrasound in enhancing diagnostic validity and reducing unnecessary biopsy rate of thyroid nodules.

OBJECTIVE: To explore the value of microflow patterns based on superb microvascular imaging (SMI) combined with greyscale ultrasound in thyroid nodule diagnosis and biopsy recommendation. MATERIALS AND METHODS: Adult patients with thyroid nodules were recruited from May 2023 to February 2024. The greyscale features of nodules were evaluated according to the five ultrasound risk stratification systems (RSSs). The microflow patterns on SMI were used to adjust the category of nodules. The crab claw-like and the root hair-like patterns were malignant signs for upgrading, with the wheel-like and t

A strategy to reduce unnecessary prostate biopsies in patients with tPSA >10 ng ml -1 and PI-RADS 1-3.

We propose a strategy to reduce unnecessary prostate biopsies in Chinese patients with total prostate-specific antigen (tPSA) >10 ng ml -1 and Prostate Imaging Reporting and Data System (PI-RADS) scores between 1 and 3. Clinical data derived from 517 patients of The First Affiliated Hospital of USTC (Hefei, China) from January 2020 to December 2023 who met the screening criteria for the study were retrospectively collected. Independent predictors were identified via univariate and multivariate logistic regression analysis. The diagnostic capacity of clinical variables was evaluated using the r

When less is more: Overdiagnosis and overtesting in pediatric febrile seizures in the adult emergency department.

BACKGROUND: Febrile seizures are a common pediatric condition, affecting about 1 in 25 children. Defined by a fever of 38 °C or higher without central nervous system symptoms or a history of seizures, they typically occur between 6 months and 5 years of age. Published guidelines recommend conservative management for simple febrile seizures (SFS) if the child returns to baseline. There is concern, however, about excess testing in general emergency departments. This study examined the occurrence of such unnecessary testing. METHODS: This was a 10-year retrospective review from three emergency d

A Simple Screening Tool Reduces Unnecessary Preoperative Evaluation for Cataract Surgery.

BACKGROUND AND OBJECTIVES: Cataract surgery is common and low-risk. Preoperative evaluation and preoperative testing have not been shown to improve patient outcomes but do increase cost. Our process improvement aimed to reduce unnecessary preoperative primary care evaluation for cataract surgery. METHODS: We implemented a simple process involving a brief chart review and conversation with the patient to determine the appropriateness of preoperative primary care evaluations. After implementation of the screening tool, we reviewed 100 patient charts, 50 who underwent cataract surgery prior to a

Antibiotic therapy for an ENT specialist.

The study dedicated mainly to general practitioners of ENTs discusses the principles of antibiotic therapy of the most common diseases that this specialty deals with, namely acute tonsillitis and throat inflammation, acute otitis media (AOM), and acute sinusitis. The most common errors in the antibiotic therapy of these diseases are also presented. The basic antibiotic in streptococcal pharyngitis is and remains oral penicillin administered for 10 days. The basic antibiotic, if it is needed, in AOM and acute sinusitis is amoxicillin in high doses. The most common mistake in antibiotic therapy

Expanding the Baveno VI criteria for the screening of varices in patients with compensated advanced chronic liver disease.

UNLABELLED: Patients with compensated advanced chronic liver disease (cACLD) can safely avoid screening endoscopy with a platelet count >150 × 109 cells/L and a liver stiffness measurement (LSM) <20 kPa (Baveno VI criteria). However, the total number of avoided endoscopies using this rule is relatively low. We aimed at expanding the Baveno VI criteria and validating them in additional cohorts. Patients from the Anticipate cohort (499 patients with cACLD of different etiologies) were used to study the performance of different thresholds of platelets and LSM for the identification of patients at
