Postural influences on urinary incontinence in the elderly.

To evaluate the requirement for provocative cystometry in urodynamic investigation in the elderly with urinary incontinence, 100 consecutive patients, who were referred on the basis of defined clinical criteria, were first studied by supine cystometry followed by provocative tests where necessary. The most satisfactory means of performing these in the elderly is discussed and the common incontinence patterns, which may point to their need, are described. Arising from the results, the different causes of acute onset from chronic stress incontinence in elderly female patients are emphasised.

A grading model for stress urinary incontinence.

A classification of stress incontinence is described. The stress test makes it possible to diagnose sphinctero-urethral incompetence and to identify the cases that are objectively negligible. Thus, surgical treatment can be limited to those who require it.

Urodynamics in women with stress urinary incontinence.

Characteristic alterations of urethral pressure and length occur in patients with stress urinary incontinence. Urodynamics in this group of 50 patients revealed a significant decrease in urethral functional length under the stress of bladder filling and change of position from supine to sitting. A decrease in urethral closure pressure was present in individual patients and was significant. All patients with stress urinary incontinence demonstrated a decreased ability to voluntarily increase urethral pressure and also had evidence of pressure equalization on Valsalva maneuver and coughing. Coug…

Treatment of urinary incontinence with the artificial sphincter.

From December 1977 to November 1978, 21 American Medical System 742A artificial sphincters were implanted in 19 male and 2 female patients with a variety of etiologies for incontinence, including post-prostatectomy incontinence, female incontinence following unsuccessful bladder neck suspension and myelodysplasia. Urodynamic evaluation before sphincter placement is reviewed. Eight patients (38 per cent) had excellent or improved results, while 5 (24 per cent) remained unchanged and 5 (24 per cent) suffered urethral erosion. Between January and September 1979, 18 patients had placement of the 7

Diagnosis and management of urinary incontinence and functional fecal incontinence (encopresis) in children.

The ability to maintain normal continence for urine and stools is not achievable in all children by a certain age. Gaining control of urinar … ialist treatment, not only to restore continence but also to preserve renal function. Most forms of urinary incontinence are not caused by an anatomic or physiologic abnormality and, hence, are more difficult to diagnose and their management requires a sound knowledge of bladder and bowel function.

[Percutaneous posterior tibial nerve electrostimulation in urge urinary incontinence and faecal incontinence].

OBJECTIVE: The main aim of this study was to assess the efficacy of percutaneous posterior tibial nerve electrostimulation (P-PTNS) in urge urinary incontinence (UUI) and faecal incontinence (FI) refractory to first-line treatment. A secondary aim was to identify predictors of treatment response. To do this, we performed a 2-year analytical, longitudinal and prospective study in patients with UUI and FI treated with P-PTNS. MATERIAL AND METHODS: We included patients with UUI and FI who were treated with P-PTNS and who completed the bladder/faecal incontinence diary and severity and quality of…

Impact of Urinary Incontinence on Physical Function and Respiratory Muscle Strength in Incontinent Women: A Comparative Study between Urinary Incontinent and Apparently Healthy Women.

Patients with stress urinary incontinence (SUI) may be afraid to increase intra-abdominal pressure to avoid incontinence. This could lead to weak expiratory muscles. The aim of this study was to investigate the association between respiratory muscle strength, physical function, and SUI in patients with SUI. A cross-sectional study was conducted in the Physical Medicine and Functional Rehabilitation Department. Thirty-one incontinent women (IG) and twenty-nine women in a control group (CG) were enrolled in this study. Anthropometric data, respiratory muscle strength (maximal inspiratory pressur…

Validation of the "international consultation on incontinence questionnaire urinary incontinence quality of life".

OBJECTIVE: To Validate the International Consultation on Incontinence Questionnaire Quality of Life for the Portuguese population. Urinary incontinence is a highly prevalent condition with a negative impact on people's quality of life. The International Consultation on Incontinence Questionnaire Quality of Life was adapted in order to have a standardized structure that allows the assessment of the impact of urinary incontinence on quality of life. METHOD: An observational cross-sectional study was carried out with two hundred and twenty participants recruited at the Centro Hospitalar de Vila …

The role of anti-incontinence surgery in management of occult urinary stress incontinence.

Combining anti-incontinence and pelvic organ prolapse surgery for patients with occult urinary stress incontinence is controversial. The concern is that some of these patients may remain continent after vaginal prolapse repair making the addition of anti-incontinence surgery unnecessary. However, this can be explained by the fact that the anterior vaginal repair has a curative effect on stress incontinence. Therefore, these patients are denied the more successful anti-incontinence surgery by treating their incontinence with vaginal repair. Once we are able to detect the true cases of occult ur

Prevalence of anal incontinence in women with symptoms of urinary incontinence and genital prolapse.

OBJECTIVE: To investigate risk factors and prevalence of anal incontinence among women with pelvic floor dysfunctions. METHODS: We evaluated 881 women with symptoms of urinary incontinence and/or genital prolapse. Each completed a bowel questionnaire and underwent a detailed medical, surgical, obstetric, and gynecologic history, and a pelvic examination. Additional testing, when indicated, included office cystometry or multichannel urodynamic evaluation. Multivariable analysis using logistic regression was used to test the overall significance of all variables significantly associated with an…

Stress Urinary Incontinence After Urethral Diverticulum Repair Without Concomitant Anti-Incontinence Procedure.

OBJECTIVE: To review the rates of persistent and de novo stress urinary incontinence (SUI) following urethral diverticulum (UD) repairs performed without concomitant SUI surgical procedures. METHODS: Following IRB approval, charts of women who underwent UD excision by three FPMRS surgeons were reviewed. Data collected from the electronic medical record included demographic information, preoperative symptoms and evaluation (validated questionnaires [UDI-6, QoL]), imaging studies, operative details, post-operative symptoms, and subsequent surgical interventions. Excluded were women with <6 mont…
