The performance of vagotomy and pyloroplasty is followed by the occurrence of dumping symptoms in 10-30% of patients. In a few, these are severe, persistent, and refractory to dietary and medical management. Pyloric reconstruction was performed in nine patients with severe dumping symptoms. Intragastric concentrations of bile acids, lysolecithin, 14C-bile acids, pH-value, bacterial growth and gastric mucosa histology were analysed in 47 patients following selective proximal vagotomy with and without pyloroplasty. All the forms of vagotomy that are used in the treatment of peptic ulcer disease lower the acid secretion by the gastric parietal cells. The preferred operation is the proximal selective vagotomy in which the innervation to the antrum and pylorus is undisturbed. Therefore the necessity for any type of drainage procedure is obviated. The results of a number of clinical trials can be considered highly satisfactory. The undue rate of ulcer recurrence sometimes reported after the operation is possibly attributable to its incorrect performance.

Gastroesophageal reflux was investigated in 80 patients with duodenal ulcer by analysis of symptomatology and the acid reflux test. In 728 patients operated on because of gastroduodenal ulcers there were early complications in 7.6% and a lethality of 0.13%. Primary drainage operation was performed in 9.5%, secondary drainage in 0.9%. One year after operation, reduction of BAO was 60% and of PAO 50%, in cases of recurrence only 35 and 30%. Postoperative recurrence of duodenal ulcer was seen in 3.1% after one year and in 5.5% after two years. Recurrence of gastric ulcer became apparent in 9.3% after one year and in 3.4% after two years. Reoperation for recurrence was performed in 1.3%.

Two to five years after highly selective vagotomy (H.S.V.) for duodenal ulcer the results were similar in patients with high preoperative maximal acid outputs and those with lower acid outputs.
