Value-based price corridor for bepirovirsen in chronic hepatitis B: an early health technology assessment in a cost-compressed setting.

BACKGROUND: We conducted an early health technology assessment (eHTA) of bepirovirsen, an investigational finite-course therapy for chronic hepatitis B. In China, low-cost generic nucleos(t)ide analogues (NAs) create a demanding comparator environment. The value-based price of a potential functional-cure strategy therefore depends on local comparator prices, threshold selection, cure durability, safety, monitoring requirements, and cured-state utility assumptions. METHODS: A lifetime Markov model simulated bepirovirsen 300 mg/week for 24 weeks plus background NAs against comparator-specific s

Veterans Health Administration Models of Community-Based Long-Term Care: State of the Science.

The complex care needs of older adults arising at the intersection of age-related illnesses, military service, and social barriers have presented challenges to the US Department of Veterans Affairs (VA) for decades. In response, the VA has invested in centers that integrate research, education, and clinical innovation, using approaches aligned with a learning health care system, to create, evaluate, and implement new care models. This article presents an integrative review of 6 community care models developed within the VA to manage multimorbidity, complex social needs, and avoid institutional

Health-seeking behavior among hematuria patients in Jordan: the case of the Health Belief Model.

BACKGROUND/OBJECTIVES: Despite broad application of the Health Belief Model (HBM) across a range of clinical contexts, its relevance to patients actively presenting with hematuria remains unexplored. Hematuria may stem from minor self-resolving causes or serious malignancies requiring early intervention. This study aimed to characterize health-seeking behavior in this population and to determine which clinical and belief-based factors predict whether patients pursue medical evaluation. METHODS: We conducted a cross-sectional study on 150 patients presented with hematuria, enrolled by convenie

Costs and cost-effectiveness of a comprehensive tuberculosis case finding strategy in Zambia.

INTRODUCTION: Active-case finding (ACF) programs have an important role in addressing case detection gaps and halting tuberculosis (TB) transmission. Evidence is limited on the cost-effectiveness of ACF interventions, particularly on how their value is impacted by different operational, epidemiological and patient care-seeking patterns. METHODS: We evaluated the costs and cost-effectiveness of a combined facility and community-based ACF intervention in Zambia that utilized mobile chest X-ray with computer-aided reading/interpretation software and laboratory-based Xpert MTB/RIF testing. Progra

A contemporary paradigm for value-based medicine in peripheral artery disease.

OBJECTIVE: Peripheral artery disease (PAD) imposes significant clinical and economic burdens. Current fee-for-service models incentivize volume rather than outcomes, highlighting the need for value-based medicine (VBM) approaches. This review evaluates contemporary strategies for integrating VBM into PAD management, focusing on enhancing patient outcomes, optimizing resource use, and reducing health care expenditures. METHODS: We undertook a narrative review of the English-language literature by searching PubMed, Google Scholar, and professional society websites through April 2025 with the te

When is health adequate for older adults? Optimal standards, patient-defined thresholds, and the weight of inequality.

This perspective distinguishes two standards that are often used as if they were interchangeable. Optimal health is a standard built by medicine and expressed through guideline targets and biomarkers. Adequate health is the threshold of functioning that allows a person to live in a way they themselves experience as sufficient and dignified. Building on the concept of homeodynamic space as the biological substrate of adequate independence, on the older salutogenic tradition that asked what sustains health rather than only what causes disease, and on clinical evidence that aligning care with pat

Model-Based Cost-Effectiveness Analysis of Panitumumab Plus FOLFIRI for the Second-Line Treatment of Patients with Wild-Type Ras Metastatic Colorectal Cancer.

INTRODUCTION: The NCT00339183 trial demonstrated that adding panitumumab to fluorouracil, leucovorin and irinotecan (FOLFIRI) as a second-line therapy of wild-type RAS metastatic colorectal cancer (mCRC) increases the median progression-free survival (PFS). Nevertheless, panitumumab is not yet approved in China, and the costs and outcomes of the therapy are still unclear. We estimated the cost-effectiveness of this intervention from the perspective of Chinese health care systems by constructing two pricing scenarios for panitumumab. Scenario 1: Pricing is based on the price of a similar produc

Modelling the Pan-European Economic Burden of Alzheimer's Disease.

BACKGROUND: Recent advances open the opportunity of altering the course of Alzheimer's disease (AD) through lifestyle-based modifications and novel therapies. Ensuring that society is investing limited budgets in the interventions that have the greatest potential to generate tangible impact will require tools to guide policymakers. OBJECTIVES: To build on previous studies to develop an economic model that estimates the societal burden of AD and evaluates the potential impact of novel interventions in six large European countries. DESIGN: AD progression was modelled using a published Markov s

Structured headache services as the solution to the ill-health burden of headache. 2. Modelling effectiveness and cost-effectiveness of implementation in Europe: methodology.

BACKGROUND: Health economic evaluations support health-care decision-making by providing information on the costs and consequences of health interventions. No universally accepted methodology exists for modelling effectiveness and cost-effectiveness of interventions designed to close treatment gaps for headache disorders in countries of Europe (or elsewhere). Our aim here, within the European Brain Council's Value-of-Treatment project, was to develop headache-type-specific analytical models to be applied to implementation of structured headache services in Europe as the health-care solution to

Assessing Health-Related Quality of Life in Patients With Diabetic Foot Disease: Why Is It Important and How Can We Improve? The 2017 Roger E. Pecoraro Award Lecture.

Patient-reported outcomes (PROs) have become an important subject in the area of diabetes-related foot complications. Self-reported health-related quality of life (HRQOL) surveys can provide a generic measure of overall health (global) and can be disease specific (i.e., diabetes) or even region specific (i.e., lower-extremity function). Analysis of PRO measures utilizing validated instruments allows health care providers to determine whether medical and surgical treatments are providing patients with the highest level of outcome possible and are actually improving HRQOL. The 36-item Short Form

Advancing the measurement of caregiver impact: Data collection priorities aligned with caregiver, health technology assessment, and policy stakeholders.

BACKGROUND: Family caregiving plays a central role in the US health care system, with substantial economic, health, and policy implications. In 2024, the estimated economic value of unpaid caregiving exceeded $1 trillion, with tens of millions of individuals providing care nationwide. However, caregiver impacts are inconsistently measured and rarely incorporated into clinical evidence, economic evaluations, and health technology assessments, limiting their use in coverage, formulary, and policy contexts. As a result, health care decision-making may not fully capture the broader benefits, costs
