Selection of a teicoplanin-resistant Enterococcus faecium mutant during an outbreak caused by vancomycin-resistant enterococci with the vanB phenotype.

Vancomycin-resistant enterococci (VRE) have recently become an increasing problem in hospitals in Poland, being responsible for a growing number of nosocomial outbreaks. In this work, we have analyzed the second outbreak of VRE with the VanB phenotype to be identified in the country. It was caused by clonal dissemination of a single strain of vancomycin-resistant Enterococcus faecalis (VRES) and horizontal transmission of vancomycin resistance genes among several vancomycin-resistant Enterococcus faecium (VREM) strains. Two similar restriction fragment length polymorphism types of the vanB gen…

Analysis of 42 cases of septicemia caused by an epidemic strain of methicillin-resistant Staphylococcus aureus: evidence of resistance to vancomycin.

Recent case reports of vancomycin treatment failures in the United States, Japan, and France have prompted a retrospective analysis of 42 cases of septicemia caused by epidemic methicillin-resistant Staphylococcus aureus strain 15 (EMRSA-15), which is the most prevalent epidemic strain of methicillin-resistant S. aureus in the United Kingdom; all cases occurred in a teaching hospital in Manchester, United Kingdom, between 1994 and 1998. Mortality was lowest (4%) in patients with rifampin-susceptible isolates treated with vancomycin and rifampin. It rose to 38% in patients who were treated with

Decreasing prevalence of isolates with vancomycin heteroresistance and vancomycin minimum inhibitory concentrations ≥2 mg/L in methicillin-resistant Staphylococcus aureus over 11 years: potential impact of vancomycin treatment guidelines.

We evaluated vancomycin MIC (V-MIC) and the prevalence of intermediately susceptible (VISA) and heteroresistant (hVISA) isolates trends in methicillin-resistant Staphylococcus aureus bacteremia among 720 adults (≥ 18 years) inpatients over 4 study periods (2002-2003, 2005-2006, 2008-2009, and 2010-2012). V-MIC (Etest) and the prevalence of hVISA and VISA (determined by population analysis profile-area under the curve) were stratified according to the study period. Mean vancomycin MIC was 1.78 ± 0.39, 1.81 ± 0.47, 1.68 ± 0.26, and 1.54 ± 0.28 mg/L in 2002-2003, 2005-2006, 2008-2009, and 2010-20

Antibiofilm and antibacterial activity of maslinic acid against Vancomycin-Resistant Enterococci (VRE).

The emergence of vancomycin-resistant Enterococcus (VRE) has posed a significant global health threat, especially in healthcare settings where treatment options are increasingly limited due to rising antibiotic resistance. Maslinic acid, a naturally occurring pentacyclic triterpene found in olives, is known for its diverse biological activities, including anti-inflammatory, anticancer, antioxidant, and antimicrobial effects. In this study, we investigated the antibacterial and antibiofilm potential of maslinic acid against VRE. Initial screening identified maslinic acid as a potent hit, with m

The Role of Infection Prevention in Preventing the Spread of Vancomycin-Resistant Enterococci in Healthcare Settings.

We describe the role of infection prevention in Vancomycin-Resistant Enterococci (VRE) in the health care setting. Despite low levels of virulence, VRE is a global challenge due to an increasing trend in resistance, including novel agents. Basic infection control practices to interrupt transmission, such as hand hygiene, environmental cleaning, and antimicrobial stewardship, are crucial in preventing the spread of VRE. Reducing patient-specific risk factors, such as the presence of indwelling devices, is also crucial. The use of active surveillance screening and contact precautions is not show

A rabbit model of acute bacteremia and sepsis caused by vancomycin-resistant Enterococcus faecium.

BACKGROUND: Vancomycin-resistant Enterococcus faecium (VREF) is a leading cause of nosocomial bacteremia, with mortality rates exceeding 60%. Current animal models lack translational relevance due to the divergence in immune systems between rodents and humans. OBJECTIVES: To establish a rabbit model of Enterococcus faecium infection for evaluating therapeutic regimes, specifically monoclonal antibody (mAb) therapy. METHODS: We established a reproducible rabbit model of acute VREF bacteremia using New Zealand White rabbits (n = 20) intravenously challenged with 1 × 109 CFU/g of VREF (strain E

Longitudinal wastewater-based surveillance of vancomycin-resistant Enterococci in tertiary-care hospitals.

OBJECTIVE: Vancomycin-resistant Enterococcus (VRE) is an important cause of healthcare-associated infections. We adapted wastewater-based surveillance as a tool to longitudinally monitor VRE in hospitals through the detection of vancomycin resistance genes vanA and vanB. METHODS: Wastewater from four tertiary-care hospitals (three adult and one pediatric, totaling >2300 inpatient beds) and all three municipal wastewater treatment plants (WWTP) in Calgary, Canada (∼1.8 million) was sampled weekly (March to September 2022) and every other week (September 2022 to March 2023). Wastewater pellets …

Vancomycin rescues sensitivity to isoniazid in clinical isolates of drug-resistant Mycobacterium tuberculosis.

UNLABELLED: Tuberculosis (TB) continues to be a major global health challenge, with a particularly high incidence of antibiotic resistance in high-burden countries. The increasing prevalence of resistance in Mycobacterium tuberculosis (Mtb) to both first-line and second-line TB drugs calls for novel approaches to address this problem. Tackling antibiotic-resistance at its source, with treatment strategies that can reverse resistance or control its new emergence, would be promising in the long run. Our previous work with lab-evolved isoniazid-resistant Msmeg showed that resistant bacteria have

Dissemination of vancomycin-resistant Enterococcus faecalis and Enterococcus faecium between humans and fishes.

Vancomycin-resistant enterococci (VRE) are a major public health concern, yet little is known about their circulation in fish. This study investigated the occurrence, glycopeptide resistance genotypes, virulence characteristics, and sequence types (STs) of VRE isolated from diseased fishes and humans. Isolates were identified using multiplex polymerase chain reaction (PCR) assay and matrix-assisted laser desorption/ionization time-of-flight mass spectrometry (MALDI-TOF MS), and tested for antimicrobial susceptibility. VRE isolates were screened for the presence of glycopeptide resistance genes

Prophage ϕSA169 Enhances Vancomycin Persistence in Methicillin-Resistant Staphylococcus aureus (MRSA).

Background: Persistent methicillin-resistant Staphylococcus aureus (MRSA) endovascular infections present a significant clinical therapeutic challenge. Prophages are increasingly recognized as important genetic factors influencing the pathogenicity of S. aureus, yet their role in antibiotic persistence in MRSA remains underexplored. Our previous work demonstrated that prophage ϕSA169 promotes vancomycin (VAN) persistence in an experimental model of endocarditis caused by MRSA strains with a clonal complex (CC) 45 genetic background. However, it is unknown whether this persistence-promoting eff

Genomic and phenotypic adaptations of methicillin resistant Staphylococcus aureus during vancomycin therapy.

Methicillin-resistant Staphylococcus aureus (MRSA) poses a significant global health challenge, particularly associated with serious infections such as bacteremia. Lipoglycopeptide antibiotics, including vancomycin, dalbavancin, and daptomycin, are critical in MRSA treatment. In this study, we analyzed two MRSA isolates (XF1 and XF2) from a bacteremia patient treated with vancomycin. Antimicrobial susceptibility testing revealed that XF1 was sensitive to vancomycin, dalbavancin, and daptomycin, whereas XF2 exhibited 8- to 16-fold higher minimum inhibitory concentrations for these antibiotics,

Management of vancomycin-resistant Enterococcus faecium in Dutch healthcare institutes: a nationwide survey.

BACKGROUND: Vancomycin-resistant Enterococcus faecium (VREfm) is an opportunistic pathogen, which can cause outbreaks in hospitals. In the Netherlands, several national guidelines and guidance documents on different aspects of VREfm management are available. Most available guidelines are written towards the hospital setting and only few on long-term care facilities (LTCFs). Moreover, not all aspects of VREfm management are covered, recommendations differ and the level of compliance to these guidelines is unknown. The aim of this study was to get insight into the routine VREfm policies in Dutch

Pangenome profiling of novel drug target against vancomycin-resistant Enterococcus faecium.

Enterococcus faecium is a frequent causative agent of nosocomial infection mainly acquired from outgoing hospital patients (Hospital Acquired Infection-HAIs). They are largely involved in the outbreaks of bacteremia, UTI, and endocarditis with a high transmissibility rate. The recent emergence of VRE strain (i.e. vancomycin resistant enterococcus) turned it into high priority pathogen for which new drug research is of dire need. Therefore, in current study, pangenome and resistome analyses were performed for available antibiotic-resistant genomes (n = 216) of E. faecium. It resulted in the pre
