Outcomes of Early and Late Muscle Flap Salvage of Complicated Lower Extremity Vascular Grafts.

Introduction: Muscle flaps are commonly used to protect high-risk vascular grafts in the lower extremities. This study aimed to identify differences between early and late salvage procedures. Methods: Patients undergoing lower extremity vascular reconstruction with muscle flap coverage at a tertiary medical center from 2017 to 2023 were identified. Data included demographics, procedure details, and complications. The study compared early (placed at index or ≤7 d after vascular surgical procedure) versus delayed (>7 d) flap inset. Primary outcomes included rates of infection, hematoma, seroma,

Intravascular Lithotripsy in the Aorta and Iliac Vessels: A Literature Review of the Past Decade.

Background/Objectives: Nowadays, intravascular lithotripsy (IVL) has emerged as a novel technique for treatment of vascular calcifications, first in coronary and then in peripheral arteries. In the current literature there is little evidence that describes IVL as an effective and safe solution in treating severe aortic and aorto-iliac calcifications. The aim of this study is to report current available data about the use of IVL in treating aortic and aorto-iliac calcified lesions and its application in facilitating other endovascular procedures. Methods: the present review was conducted and re

Branch Vessel Patency after Thoracic Endovascular Aortic Repair for Type B Aortic Dissection.

BACKGROUND: Thoracic endovascular aortic repair (TEVAR) for type B aortic dissections is used to promote false lumen (FL) thrombosis and favorable aortic remodeling, but its impact on occlusion of FL origin branch vessels has not been widely described. We compare FL versus true lumen (TL) branch vessel patency after TEVAR. METHODS: Patients treated by TEVAR for type B aortic dissection in zones 2-5 in the Vascular Quality Initiative from 2009 to 2018 were evaluated. The primary outcome was postoperative branch patency. Secondary outcomes were need for branch vessel intervention, preoperative

Infectious complications and outcomes after limb salvage surgery with vascular reconstruction: A retrospective analysis from a lower middle-income country.

OBJECTIVE: Soft tissue sarcomas can involve major blood vessels, historically leading to amputation as the primary treatment. Advances in surgical and vascular reconstruction techniques, along with multidisciplinary care, now allow for limb preservation in selected cases. However, outcomes data from resource-limited countries remain scarce. Therefore, the aim of this study was to assess the infectious, oncological, and functional outcomes of limb salvage surgery with vascular reconstruction in patients with extremity sarcoma. METHODOLOGY: A retrospective review was conducted at Aga Khan Unive

Resistance Index as a Prognostic Factor for Patency in Distal Lower Limb Arterial Revascularization.

BACKGROUND: Critical limb ischemia (CLI) of the lower limbs (LLs) is a severe condition caused by peripheral arterial obstructive disease (PAOD). Surgical planning for CLI requires the study of limb circulation, and the vascular resistance index (RI) measured by Doppler ultrasound (US) has been used to aid in surgical decision-making. The objective is to correlate the RI of recipient arteries with the patency of LL revascularization surgeries in patients with CLI. METHODS: Prospective cohort study conducted from July 2021 to March 2023. The RI of the recipient arteries was evaluated preoperat

Balloon-expandable covered stent therapy of complex endovascular pathology.

The current study was designed to investigate our hypotheses that balloon-expandable covered stents display acceptable function over longitudinal follow-up in patients with complex vascular pathology and provide a suitable alternative for the treatment of recurrent in-stent restenosis. All stents were Atrium iCast, which is a balloon-mounted, polytetrafluoroethylene-covered stent with a 6F/7F delivery system. A retrospective review was performed of 49 patients with 66 stented lesions. Data were analyzed with life tables and t-tests. The most commonly treated vessels were the iliac (61%) and re

Revision eDCR using a superior pedicled mucosal flap.

BACKGROUND: Endoscopic dacryocystorhinostomies (eDCRs) show patency rates between 81% and 94%. However, dacryocystorhinostomy (DCR) failure and the need for revision remain a significant challenge. One of the principal challenges in revision eDCR is the need to surgically identify the correct osteotomy site and maintain long-term patency in the setting of previously instrumented and potentially scarred tissue. At the same time, the surgeon must assume that the blood supply to the commonly described anterior and posteriorly pedicled flaps has been compromised. OBJECTIVE: The objective of the s

Graft patency and clinical outcome of femorodistal arterial reconstruction in diabetic and non-diabetic patients: results of a multicentre comparative analysis.

OBJECTIVE: in diabetic patients with critical limb ischaemia (CLI) an inferior success rate following infrainguinal bypass surgery is quite often suggested. The aim of this retrospective analysis was, therefore, to evaluate the graft patency and, particularly, the clinical outcome at 1 year in diabetic compared with non-diabetic patients. MATERIAL AND METHODS: two hundred and eleven patients (diabetics 94; non-diabetics 117) with femorodistal reconstruction for CLI were studied. Groups were comparable with regard to the Fontaine classification, the distribution of vascular risk factors, graft

Outcomes of Lower-Extremity Hemodialysis Vascular Access: A Systematic Review and Meta-Analysis.

BACKGROUND: Lower-extremity vascular access is used when conventional upper-extremity options for hemodialysis have failed or are anatomically unsuitable; however, its durability and complication profile remain uncertain. This systematic review and meta-analysis evaluated patency, graft survival, complications, mortality, and access-class differences after lower-extremity hemodialysis access creation. METHODS: PubMed, Scopus, Web of Science, and MEDLINE were searched from inception to 30 April 2026. Studies of surgically created lower-extremity hemodialysis access in adults were included. Ran

Endoscopic versus open saphenous vein graft harvest for lower extremity bypass in critical limb ischemia.

OBJECTIVE: Endoscopic vein harvest (EVH) has been demonstrated to improve early morbidity when compared with conventional open vein harvest (OVH) technique for infrainguinal bypass surgery. However, recent literature suggests conflicting results regarding mid- and long-term patency with EVH. The purpose of this study is to compare graft patency between harvest techniques specifically in patients with critical limb ischemia. METHODS: This retrospective study compared two groups of patients (EVH = 39 and OVH = 49) undergoing lower extremity revascularization from January 2009 to December 2011.

Effect of warfarin anticoagulation on below-knee polytetrafluoroethylene graft patency.

When polytetrafluoroethylene (PTFE) must be used for below-knee bypass to achieve limb salvage, effective anticoagulation with warfarin may improve graft survival. We analyzed our practice of routinely using oral anticoagulation to improve graft patency rates for PTFE grafts to below-knee popliteal and crural vessels in limb salvage procedures. We reviewed our established vascular database from February 1999 through April 2003 to identify those patients who required below-knee and tibial artery bypass with PTFE for critical limb ischemia. All patients were initiated on warfarin anticoagulation

External Iliac Occlusion Does Not Preclude Endovascular Management of Aortoiliac Disease-Technique and Evolution of Therapy.

BACKGROUND: Controversy remains over how to best manage chronic total occlusions of the aortoiliac (AI) system. Uncovered stents are the traditional choice but offer less durability in theory with the risk of stent ingrowth. External iliac artery (EIA) occlusions are challenging due to access difficulty. METHODS: We performed a retrospective study of patients who had undergone endovascular AI intervention between December, 2014 and March, 2017 for Trans-Atlantic Inter-Society Consensus D lesions. The primary study end point was overall survival. Secondary end points included primary assisted

Effectiveness and safety of balloon dilatation in radial artery with autogenous arteriovenous fistula anastomosis for hemodialysis patients: A meta-analysis.

BACKGROUND: Achieving successful vascular access for hemodialysis is critical for patients with end-stage renal disease. The creation of an autologous arteriovenous fistula (AVF) is the preferred approach; however, patients with small-caliber radial arteries often face challenges, leading to high rates of AVF failure. Balloon dilation-assisted AVF (BDA-AVF) creation has emerged as a promising technique to address these limitations. This meta-analysis evaluates the effectiveness of BDA-AVF creation in improving primary patency rates at 6 and 12 months in patients with small-caliber radial arter

Comparison of graft patency, limb salvage, and antithrombotic therapy between prosthetic and autogenous below-knee bypass for critical limb ischemia.

BACKGROUND: The autogenous vein is the preferred conduit in below-knee vascular reconstructions. However, many argue that prosthetic grafts can perform well in crural bypass with adjunctive antithrombotic therapy. We therefore compared outcomes of below-knee prosthetic versus autologous vein bypass grafts for critical limb ischemia and the use of adjunctive antithrombotic therapy in both settings. METHODS: Utilizing the registry of the Vascular Study Group of New England (2003-2009), we studied 1227 patients who underwent below-knee bypass for critical limb ischemia, 223 of whom received a pr

[Progress and prospect of the treatment of lower extremity arteriosclerosis obliterans].

In the past five years, both advancements and new problems were seen in the treatment of lower extremity arteriosclerosis obliterans. The Global Vascular Guidelines published in 2019 have given us comprehensive suggestions for the diagnosis and treatment of critical limb threatening ischemia(CLTI),but the grading and treatment suggestions for CLTI should be generalized. As to endovascular treatment, drug coated balloons have been found to be effective for limb salvage and graft patency in femoropopliteal and infra-popliteal artery occlusive diseases. As to surgical revascularization, persistent educ

Vascular reconstruction in Buerger's disease: is it feasible?

PURPOSE: Thromboangiitis obliterans (Buerger's disease) is a clinical syndrome characterized by segmental occlusions of the distal vessels. Although a cessation of using nicotine products usually helps, nevertheless a surgical revascularization may be needed in cases of stage III and IV limbs. Because of the distal and segmental nature of the disease, these procedures are rarely feasible. This article focuses on the feasibility of performing a vascular reconstruction in thromboangiitis obliterans. METHODS: Thirty-six of 94 patients (38.3%) who were followed by the Peripheral Vascular Unit of

Transit time flow measurement in off-pump surgery: is it a functional index of the biological graft integrity?

OBJECTIVE: Transit-time flow measurement (TTFM) represents a valid tool in the assessment of the quality of the anastomosis during coronary artery bypass graft (CABG). Nevertheless, a high variability limits its standardized use, lacking univocally accepted cut-off flow values. Our study analyzes TTFM data collected from a study population that underwent off-pump CABG (OP-CABG), with the aim to differentiate into subgroups according to the presence of diabetes mellitus. PATIENTS AND METHODS: Patients referred to the Cardiovascular Unit of S. Michele Hospital (Caserta, Italy) for coronary arte
