Mid-term differences in right ventricular function in patients with congenital diaphragmatic hernia compared with controls.

BACKGROUND: Patients with congenital diaphragmatic hernia (CDH) may have abnormal lung development, which may cause detrimental effects on right ventricular (RV) function. This study aimed to determine if there are persistent echocardiographic differences in RV function in patients with CDH years after repair versus control patients. METHODS: Patients who underwent repair for CDH were recruited. RV function was evaluated by strain analysis and tissue Doppler imaging (TDI). Wilcoxon's rank-sum test was used for analysis. RESULTS: Seven CDH patients and 16 control patients were studied. There

Is right ventricular systolic function reduced after cardiac surgery? A two- and three-dimensional echocardiographic study.

AIMS: A reduction in tricuspid annular plane systolic excursion (TAPSE) and peak systolic velocity (PSV) of tricuspid annulus after cardiac surgery is a well-known phenomenon, even though its origin is not well established. Recently, a new three-dimensional (3D) echocardiographic software adapted for right ventricular (RV) analysis has been validated. Aims of this study were to evaluate RV function in patients with mitral valve prolapse undergoing surgical valvular repair and to compare and correlate 3D RV ejection fraction (RVEF) with TAPSE and PSV before and after surgery. METHODS AND RESUL

Impaired right ventricular systolic function demonstrated by reduced atrioventricular plane displacement in adults with Marfan syndrome.

AIMS: The right ventricle (RV) ejects the same volume of blood at the same rate as the left ventricle (LV). Mild LV dysfunction has been demonstrated in Marfan syndrome (MFS). However, little attention has been paid to the functioning of the RV. The aim of this study was to assess RV function in unoperated adult MFS patients. METHODS AND RESULTS: In 66 unoperated (15-58 years) MFS patients and 61 controls, rate of pressure rise (dp/dt) in RV, and tricuspid annular motion (TAM) were studied using conventional echocardiography and tissue Doppler imaging (TDI). When compared with controls, MFS p

Resting right ventricular function in patients with coronary artery disease: pressure volume analysis using conductance catheters.

Right ventricular pressure volume cycles from patients with coronary artery disease were created in order to assess their characteristics and the effects of peak ventricular pressure and right and left artery coronary disease. Thirty-three patients undergoing diagnostic catheterisation for ischaemic heart disease underwent right ventricular catheterisation with a micromanometer and a conductance catheter. Simultaneous pressure and volume signals were recorded and analysed as functions of time, and of each other, forming pressure volume cycles. A total of 19/33 (58%) patients had an abnormal pr

Right ventricular function after extracardiac conduit repair for tetralogy of Fallot. A comparison among surgical approaches.

Right ventricular volume measurements and hemodynamic studies were performed in 20 patients after total correction of tetralogy of Fallot. There were 6 patients with an extracardiac conduit and Hancock xenograft (G-1), 8 with an outflow tract patch (G-2) and 6 without or with a minimal right ventriculotomy and repair of pulmonary valve (G-3). The age at the time of operation was over 3 years in each patient and averaged 5 +/- 2 (mean +/- SD) years. Right ventricular systolic pressure averaged 81 +/- 22, 55 +/- 22 and 58 +/- 30 mmHg in G-1, G-2 and G-3, respectively. Right ventriculography show

Supra-coronary aortic banding improves right ventricular function in experimental pulmonary arterial hypertension in rats by increasing systolic right coronary artery perfusion.

AIM: Pulmonary arterial hypertension (PAH) results in right ventricular (RV) dysfunction owing, in part, to RV ischemia. The relative contribution of RV microvascular rarefaction vs reduced right coronary artery perfusion pressure (RCA-PP) to RV ischemia remains unknown. We hypothesize that increasing RCA-PP improves RV function in PAH by increasing RV systolic perfusion. METHODS: Supra-coronary aortic banding (SAB) or sham surgery was performed on male Sprague-Dawley rats. Seven to ten days later, rats received either monocrotaline (MCT; 60 mg/kg) or saline. After 1 month, echocardiography,

Lung recruitment maneuver improves right and left ventricular function in patients with acute respiratory distress syndrome.

BACKGROUND: Lung recruitment maneuvers (LRM) and high positive end-expiratory pressure (PEEP) may benefit some patients by reopening non- or poorly aerated alveoli. However, the effects of opening the lung with LRM on hemodynamics remain uncertain. This study aimed to evaluate the direct impact of LRM on cardiac function in patients with moderate-to-severe acute respiratory distress syndrome (ARDS). METHODS: This post-hoc analysis included 34 patients with moderate-to-severe ARDS from two prospective cohort studies. The LRM consisted in a gradual increase in PEEP, starting from 25 cmH2O (PEEPp

Assessment of right ventricular function in patients after coronary artery bypass graft: a single center study.

Right ventricular dysfunction is a major risk factor in coronary artery disease. In patients undergoing revascularization for left ventricular ischemia, the incidence of RVD is reported in about 20%. This study aimed to assess right ventricular function as a possible risk factor in patients undergoing coronary artery bypass graft. This prospective study was conducted on 77 patients who underwent coronary artery bypass grafting. All cases were subjected to detailed medical history, full physical examination, electrocardiogram, routine laboratory tests including echocardiography or STE, and all

Chronic Thromboembolic Pulmonary Disease: Right Ventricular Function and Pulmonary Hemodynamics in a 4-Year Follow-Up.

Chronic thromboembolic pulmonary disease (CTEPD) with or without pulmonary hypertension (PH) is characterized by persistent perfusion defects and progressive pulmonary vascular dysfunction after acute pulmonary embolism (PE), despite adequate anticoagulant therapy. We aimed at assessing clinical, hemodynamic, and functional evolution in patients screened for CTEPD with persistent lung perfusion scintigraphy (Q-scan) defects to identify non-invasive predictors of right ventricular (RV) impairment and development of exercise-induced pulmonary hypertension (ExPH). We analyzed 55 patients with a h

Right ventricular echocardiographic function in patients with breast cancer undergoing anthracycline-based chemotherapy: A prospective study.

BACKGROUND: Chemotherapy regimens with anthracyclines, widely used in treating breast cancer and lymphoma, are associated with significant cardiac toxicity. While previous studies have primarily focused on left ventricular (LV) function, limited research exists on right ventricular (RV) function. This study aimed to evaluate RV echocardiographic function in breast cancer patients undergoing anthracycline-based chemotherapy. METHODS: A cohort of 72 breast cancer patients receiving anthracycline treatment at Ghazi Tabatabai and Madani Hospitals from April to March 2022 participated in this stud

Inhaled nitric oxide therapy is effective in improving right ventricular function in patients receiving ECPELLA support.

OBJECTIVES: ECPELLA, which uses veno-arterial extracorporeal membrane oxygenation (V-A ECMO) and the Impella percutaneous ventricular assist device, is effective for patients with cardiogenic shock. However, patients with severe right heart dysfunction frequently have difficulty weaning off V-A ECMO. Inhaled nitric oxide (iNO) therapy may induce pulmonary artery pressure while improving right ventricular function. The goal of this study is to determine whether iNO improves right and left ventricular function in patients on ECPELLA. METHODS: This retrospective study, conducted at a single-cent

Validation of noninvasive indices of right ventricular diastolic function. Simultaneous echocardiography and pressure-volume catheterization studies.

BACKGROUND: The reliability of the recommended echocardiographic methods for assessing RV diastolic function has been questioned. We aimed to validate noninvasive indices of RV diastolic function, derived from tricuspid Doppler and myocardial deformation metrics, against intrinsic diastolic chamber properties and filling pressures. METHODS: We obtained simultaneous high-fidelity pressure-volume loops and echocardiographic data in separate animal and clinical settings: (1) a porcine model of acute hemodynamic interventions (n = 13), and (2) patients with Fallot tetralogy and pulmonary hyperten

Changes in right ventricular function on three-dimensional speckle tracking echocardiography after transcatheter pulmonary valve replacement.

BACKGROUND: Pulmonary regurgitation (PR) is common after tetralogy of Fallot (TOF) repair and closely associated with adverse outcomes. Transcatheter pulmonary valve replacement (TPVR) is a novel treatment for severe PR or mixed pulmonary stenosis (PS) and regurgitation. The aim of this study was to evaluate the changes in right ventricular (RV) function in patients after TPVR on two-dimensional (2D) and three-dimensional (3D) speckle tracking echocardiography and to identify the predictors of RV function and New York Heart Association (NYHA) functional class at 6 months after TPVR. METHODS:

3D Echocardiographic and CMR Imaging for the Assessment of Right Ventricular Function and Tricuspid Regurgitation Severity.

BACKGROUND: Tricuspid regurgitation (TR) is associated with increased mortality and is often underdiagnosed due to limitations in imaging modalities. While routine 2-dimensional echocardiography (2DE) demonstrates frequent disagreement with cardiac magnetic resonance imaging (CMR) in classifying TR severity, the incremental value of 3-dimensional echocardiography (3DE) remains unknown also due to the lack of a generalizable grading scheme across imaging modalities. Therefore, this study provides an intermodality comparison of all 3 imaging modalities (2DE, 3DE, and CMR) in evaluating TR severi

Assessment of right ventricular function in healthy Great Danes and in Great Danes with dilated cardiomyopathy.

INTRODUCTION: Right ventricular (RV) dysfunction is a significant negative prognostic indicator in human dilated cardiomyopathy (DCM). Many RV indices are weight-dependent, and there is a lack of reference values for the right heart in giant breed dogs (over 50 kg), including Great Danes (GDs). This study aimed to compare indices of RV function in echocardiographically normal GDs, those with preclinical DCM (PC-DCM), and those with DCM and congestive heart failure (DCM-CHF). ANIMALS: A total of 116 client-owned adult GDs: 74 normal, 31 with PC-DCM, and 11 with DCM-CHF. METHODS: A retrospecti

Asymmetric right ventricular myocardial work correlates with gold standard measurements of cardiac function in pulmonary hypertension.

Right ventricular (RV) (dys)function determines outcomes in pulmonary hypertension (PH). We previously found that asymmetric RV myocardial work (MW) corresponds with inefficient RV function in experimental PH models. We therefore aimed to investigate regional distribution of RV MW and its correlation with catheter hemodynamics in children with PH. RV MW was calculated by longitudinal strain and simultaneous catheter pressure measurements in 14 patients with PH. Wasted MW was defined as the ratio of inappropriate myocardial lengthening to favorable shortening work. Segment-wise and averaged MW

The added value of abnormal regional myocardial function for risk prediction in arrhythmogenic right ventricular cardiomyopathy.

AIMS: A risk calculator for individualized prediction of first-time sustained ventricular arrhythmia (VA) in arrhythmogenic right ventricular cardiomyopathy (ARVC) patients has recently been developed and validated (www.ARVCrisk.com). This study aimed to investigate whether regional functional abnormalities, measured by echocardiographic deformation imaging, can provide additional prognostic value. METHODS AND RESULTS: From two referral centres, 150 consecutive patients with a definite ARVC diagnosis, no prior sustained VA, and an echocardiogram suitable for deformation analysis were included
