Fetal macrosomia and its associated factors among pregnant women delivered at national referral hospital in Uganda, a case-control study.

BACKGROUND: The delivery of macrosomic newborns (newborns>4000gm) is increasing steadily worldwide and yet it is associated with many complications. The risk factors for fetal macrosomia include weight at first antenatal visit, previous delivery of a large newborn, newly diagnosed diabetes in pregnancy, increasing number of deliveries, a male fetus, and many others. There is paucity of data on fetal macrosomia in Uganda and the objective of this study therefore was to determine the risk factors for fetal macrosomia among women who delivered at a National Referral Hospital in Kampala, Uganda..

Prediction of Neonatal Respiratory Morbidity Assessed by Quantitative Ultrasound Lung Texture Analysis in Twin Pregnancies.

The objective of this study was to evaluate the performance of quantitative ultrasound of fetal lung texture analysis in predicting neonatal respiratory morbidity (NRM) in twin pregnancies. This was an ambispective study involving consecutive cases. Eligible cases included twin pregnancies between 27.0 and 38.6 weeks of gestation, for which an ultrasound image of the fetal thorax was obtained within 48 h of delivery. Images were analyzed using quantusFLM® version 3.0. The primary outcome of this study was neonatal respiratory morbidity, defined as the occurrence of either transient tachypnea o

Fetal anatomy of peripheral lymphatic vessels: a D2-40 immunohistochemical study using an 18-week human fetus (CRL 155 mm).

We demonstrated fetal peripheral lymphatic vessels (LVs) using D2-40 immunohistochemistry in a whole female fetus (18 weeks of gestation, CRL 155 mm) except for the head. There were abundant LVs in the thyroid gland, lung, stomach, small intestine, rectum and pancreas, whereas no LVs were seen in the parathyroid gland, spleen and adrenal cortex. In the liver, except for the gallbladder bed, LVs were still restricted to around hilar thick portal veins and around the hepatic vein terminals. Subcutaneous LVs were well developed throughout the body even in areas where no or few perforating LVs con

Atherosis revisited: current concepts on the pathophysiology of implantation site disorders.

There are two distinct histological manifestations of impaired placental implantation in humans--incomplete trophoblastic vascular invasion and atherosis. Both have been described to occur in pregnancies affected by a variety of disorders such as preeclampsia, fetal growth restriction, systemic lupus erythematosus, and diabetes. Our purpose was to integrate recent developments in the understanding of implantation site disorders into a pathophysiological scenario that interrelates these placentation disorders and associated pregnancy complications. Sources were identified from a MEDLINE search

Consuming non-alcoholic beer and other beverages during pregnancy and breastfeeding.

QUESTION: An increasing number of my patients are asking about the safety of consuming non-alcoholic beer and other alcohol-free versions of alcoholic beverages during pregnancy and breastfeeding, as they believe that these drinks might be a "safer" alternative to regular alcoholic beverages. What are Motherisk's recommendations regarding these products? ANSWER: Such drinks might contain higher ethanol levels than what is indicated on their labels. As there is no known safe level of alcohol intake in pregnancy, abstinence from non-alcoholic beverages would eliminate any risk of fetal alcohol

[Version by internal maneuvers: alternative to cesarean section under precarious conditions. The experience of the University Clinic of Gynecology and Obstetrics].

Obstetrics practised in African University Hospitals is beset with specific constraints. The emergency admission of a woman whose fetus is dead or severely compromised, after an extended labor and an obvious ovular infection, leads to fear an hysterotomy as well as an hysterectomy. The authors specify, under their work conditions, the indications of internal version maneuvers, from 50 cases collected in one year. The fetal prognosis is not taken into account since it concerns dead or very threatened children. As for the maternal prognosis, it is excellent since the average stay is of 48 hours

Methods and Software to Analyze Gene-Environment Interactions under a Case-Mother-Control-Mother Design with Partially Missing Child Genotype.

INTRODUCTION: The case-mother-control-mother design allows to study fetal and maternal genetic factors together with environmental exposures on early life outcomes. Mendelian constraints and conditional independence between child genotype and environmental factors enabled semiparametric likelihood methods to estimate logistic models with greater efficiency than standard logistic regression. Difficulties in child genotype collection require methods handling missing child genotype. METHODS: We review a stratified retrospective likelihood and two semiparametric likelihood approaches: a prospecti

The magnitude of neonatal near miss and associated factors among live births in public hospitals of Jimma Zone, Southwest Ethiopia, 2020: A facility-based cross-sectional study.

BACKGROUND: Neonates with severe complications at birth or during the neonatal period who nearly died but survived constitute neonatal near miss (NNM) cases. Identifying NNM cases and correcting contributing factors are of the utmost importance to get relevant controls for neonatal deaths. However, limited studies are assessing the prevalence of NNM and associated factors with NNM cases in Ethiopia. So, this study is aimed at assessing the magnitude of neonatal near miss and associated factors among live births in public hospitals of Jimma zone, southwest Ethiopia, 2020. METHODS: A facility-b

Proportion and Associated Factors of Nonreassuring Fetal Heart Rate Patterns in Finote Selam Primary Hospital, North West Ethiopia.

INTRODUCTION: Nonreassuring fetal heart rate patterns (NRFHRP) suggest fetal conciliation or a deteriorating ability to handle the stress of labor. Nearly half of stillbirths occurring worldwide are due to hypoxia which is primarily manifested by NRFHRP. Hence, this study assessed the proportion and associated factors of NRFHRP in the Finote Selam primary hospital, North West Ethiopia. METHODS: An institution-based retrospective cross-sectional study was conducted from March 1 to April 1, 2019, on 364 charts of mothers who gave birth from January 2017 to January 2018 at the Finote Selam prima

Incidence of gestational diabetes mellitus among Indian women.

Gestational diabetes mellitus (GDM) is a growing public health concern due to its association with adverse maternal and fetal outcomes, yet its risk factors remain under-investigated in many populations. A prospective observational study of 500 pregnant women who presented on regular antenatal visits to CIMS was completed. During 24-28 weeks of gestation, GDM screening was done through a 75-gram Oral Glucose Tolerance Test (OGTT). Regarding socio-demographic, clinical and lifestyle information, standardized questionnaires were used to collect the relevant data and the risk factors were identif

The impact of manual rotation of the occiput posterior position on spontaneous vaginal delivery rate: study protocol for a randomized clinical trial (RMOS).

BACKGROUND: The frequency of posterior presentations (occiput of the fetus towards the sacrum of the mother) in labor is approximately 20% and, of this, 5% remain posterior until the end of labor. These posterior presentations are associated with higher rates of cesarean section and instrumental delivery. Manual rotation of a posterior position in order to rotate the fetus to an anterior position has been proposed in order to reduce the rate of instrumental fetal delivery. No randomized study has compared the efficacy of this procedure to expectant management. We therefore propose a monocentri

Impact of hydroxychloroquine on pregnancy outcomes in systemic lupus erythematosus: A 25 years retrospective cohort study from Asia.

BackgroundSystemic lupus erythematosus (SLE) is an autoimmune disorder that affects various organs and follows a pattern of remission and relapse. Hydroxychloroquine (HCQ), an antimalarial drug, has recently become widely used in pregnant women with SLE due to its multi-level immune anti-inflammatory mechanisms and potential endothelial protective and thromboprophylaxis properties. While several studies have evaluated the impact of HCQ on SLE activity and the occurrence of neonatal lupus, its effects on improving early delivery, preeclampsia, and intrauterine growth restriction (IUGR) remain c

Factors Leading To Meconium Aspiration Syndrome in Term- and Post-term Neonates.

Background Meconium aspiration syndrome (MAS) is considered a major cause of respiratory morbidity. It is a common issue encountered in the delivery room and newborn nursery. There is a need to identify the factors that lead to MAS to develop strategies to screen such patients at an early stage to decrease the mortality and morbidity. The objective of this study was to determine the factors leading to MAS in neonates delivered at ≥37 weeks of gestational age. Methods A cross-sectional study was conducted through non-probability consecutive sampling technique at Liaquat University Hospital, Hyd

Comparing birth experience and birth outcome of vaginal births between induced and spontaneous onset of labour: a prospective study.

PURPOSE: In developed countries, around 25% of all births involve labour induction. Studies have indicated that labour induction negatively influences the birth experience as well as the birth process. However, the impact of labour induction when only considering vaginal deliveries has not been studied yet, which was the purpose of the present study. METHODS: 186 women who gave birth vaginally took part in this prospective study. 95 women gave birth after spontaneous onset of labour (SL-group) and 91 women received a labour induction (LI-group). Eight to 72 h after birth, the women filled in

Androgen levels in the fetal cord blood of children born to women with polycystic ovary syndrome: a meta-analysis.

INTRODUCTION: Polycystic ovary syndrome (PCOS) is one of the most common endocrine disorders in reproductive-aged women. It is reported that intrauterine exposure to hyperandrogenism may induce the development of PCOS and associated complications in later life. To analyze the intrauterine androgen levels in infants born to PCOS mothers, we evaluated the androgen levels in fetal cord blood through a meta-analysis of observational studies. MATERIAL AND METHODS: The following online databases were systematically searched: PubMed, EMBASE, Cochrane library databases and Web of Science up to Decemb

Fetal pulse oximetry for fetal assessment in labour.

BACKGROUND: The use of conventional cardiotocographic (CTG) monitoring of fetal well-being during labour is associated with an increased caesarean section rate, compared with intermittent auscultation of the fetal heart rate, resulting in a reduction in neonatal seizures, although no differences in other neonatal outcomes. To improve the sensitivity of this test and therefore reduce the number of caesarean sections performed for nonreassuring fetal status, several additional measures of evaluating fetal well-being have been considered. These have demonstrated some effect on reducing caesarean
