Meningioma of the internal auditory canal with extension into the vestibule.

Meningiomas account for approximately 18 to 19 per cent of all brain tumours. Although they can arise in numerous locations, meningiomas of the internal auditory canal (IAC) are rare. Most tumours that originate in the IAC are schwannomas of the VIIIth cranial nerve (acoustic neuromas). We report a case of a meningioma which appears to originate from the IAC and extends into the vestibule. The clinical findings and the radiographical features of meningiomas of the IAC are similar to those of acoustic neuromas. Pre-operative differentiation between acoustic neuromas and meningiomas of the IAC m

Calcium antagonists as a peripherally acting labyrinthine suppressant in humans.

A total of 908 patients with peripheral vestibular disorders were treated with calcium antagonists for periods of 10 to 120 days in a series of controlled clinical trials using a placebo or other medication for comparison. The cumulative analysis of the results indicates that cinnarizine, flunarizine and bencyclan have produced good clinical results, particularly in patients with vascular problems. A comparison between different doses used in different trials revealed a correlation of good clinical results to smaller doses of some of these medications.

Hearing thresholds of normal and fenestrated deaf pigeons. A behavioural study on hearing with the vestibular organ.

Hearing thresholds were measured for 5 normal pigeons with the heart-rate conditioning method. After extirpation of the membranous cochlea on both sides and fenestration of the lateral canals, hearing thresholds were measured again with the same method at 250, 750 and 1500 Hz. Hearing thresholds found in this way for deaf fenestrated pigeons correspond very well to threshold values for the "Tullio effect". Electrical activity in the vestibular nerve evoked by sound is thought to be the origin of these phenomena.

The vertical vestibulo-ocular reflex and ocular resonance.

A technique is described for the measurement of retinal image motion under conditions of forced angular vibration of the head in pitch at frequencies between 10 and 100 Hz. The experimental technique has been used in nine normal subjects, and in two subjects with vestibular dysfunction; one with unilateral, the other with bilateral deficit. Eye movements compensatory in phase were recorded at frequencies between 10 and 25 Hz in all normal subjects. The ratio of angular retinal image motion to angular head motion (ocular gain) over these frequencies was in the range 0.82-0.94. When the head was

[Expert testimony in vibration disease in inland navigation employees].

110 inland navigation workers, exposed to vibration and noise, have undergone electronystagmographic examinations. 61.7% subjects have developed disturbances of vestibular organ. The occurrence of those disturbances has been found to vary with the duration of vibration - and - noise exposure i.e. with the employment system. The 24 - hr - system poses a greater risk of vestibular organ disturbances than the 12 - hr - system. The investigation should promote setting - up some certification of the vibration disease in water transport workers.

Variability and habituation of nystagmic responses to hot caloric stimulation of normal subjects. Evidence that this test may be inapplicable to monitoring drug-induced vestibular toxicity.

Experiments were performed on 25 otoneurologically "normal' subjects to evaluate the hot caloric test as a screening test for aminoglycoside vestibular toxicity. Using portable equipment under non-ideal conditions, it was found that there was a large inter-subject variability in nystagmic response and that, instead of a random test-retest variability, a systematic variation in response occurred on repeated caloric stimulation with water at 44 degrees C. A response decline (habituation) evident in both the maximum slow phase velocity and the maximum frequency occurred at second test, although t

Brainstem auditory evoked response in the diagnosis of inner ear injury in the horse.

Brainstem auditory evoked response (BAER) testing was done to evaluate inner ear/VIIIth cranial nerve (CN8) function in the horse. The BAER test consisted of stimulating the auditory system with clicks and recording far-field responses of the brainstem auditory components via cutaneous electrodes and a signal averaging system. The normal response was shown to be a series of waves occurring within the first 10 msec after the stimulus click. Functional loss of the auditory receptor organ (cochlea) or CN8 results in loss of the entire response on the side of the injury. Because of the anatomic re

Neurologic manifestations of Cogan syndrome.

Cogan syndrome is a multisystem inflammatory vascular disease, characterized by nonsyphilitic interstitial keratitis and vestibuloauditory symptoms. Recent reports have directed attention to involvement of other organ systems. Respiratory, cardiovascular, gastrointestinal, and musculoskeletal problems are common, as are laboratory abnormalities and general symptoms such as fever, chills, and weight loss. Prominent neurologic problems in two patients prompted a review of 79 cases of Cogan syndrome. More than half had nervous system involvement, including electroencephalographic or spinal fluid

Post-operative hearing among patients with labyrinthine fistula as a complication of cholesteatoma using "under water technique".

INTRODUCTION: During surgery in patients with labyrinthine fistula the mandatory complete removal of the cholesteatoma while preserving inner ear and vestibular function is a challenge. Options so far have been either the complete removal of the cholesteatoma or leaving the matrix on the fistula. We evaluated an alternative "under water" surgical technique for complete cholesteatoma resection, in terms of preservation of postoperative inner ear and vestibular function. METHODS: From 2013 to 2019, 20 patients with labyrinthine fistula due to cholesteatoma were operated. We used the canal wall

Sudden vertigo of central origin.

Intense vertiginous attacks with sudden onset and of several months' duration were observed in a group of patients following viral infection. Neurotological examination revealed different types of signs of involvement of the central vestibular apparatus. All of these patients became dizzier when medicated with vestibular depressor drugs, and could obtain symptomatic relief with corticosteroids. They became asymptomatic after a period of 3-4 months from the onset of the disease, but the signs of central vestibular involvement persisted for 6-12 months.

Neuro-otological manifestations of migraine.

Vestibulocochlear derangements have been studied in three groups of patients: 200 unselected patients with migraine (Series I), 80 migrainous patients referred because of their symptoms for full neuro-otological examination (Series II), and 116 patients with tension headache who served as controls (Series III). Significant differences were established between tension headache and migraine in respect of incidence and severity of symptoms and their time of onset in relation to the headache. In migraine, vestibulocochlear disturbances can occur as an aura, accompanying the headache or during head

Diagnosis and management of drop attacks of vestibular origin: Tumarkin's otolithic crisis.

Erroneous signals of vestibular origin can cause sudden falls without warning. Drop attacks of vestibular origin in our experience most commonly occurred in patients with late or end-stage endolymphatic hydrops (usually idiopathic, ie, Meniere's disease). Eponymically termed Tumarkin's otolithic crisis, drop attacks of vestibular origin must be distinguished from drop attacks due to cardiovascular abnormalities, seizure disorders, vertebral basilar arterial insufficiency (transitory brain stem ischemia), multiple sclerosis, and drug-induced motor control disturbances. The treatment of choice f
