Management of snake-bite in rural Maharashtra: a 10-year experience.

BACKGROUND: A high incidence of snake-bite envenomation has been reported from Marathwada, Maharashtra. This study analysed the pattern of snake-bites and their management in a rural area of India over a 10-year period. METHODS: A total of 633 patients with snake-bite admitted to the Rural Community Centre and Punde Hospital in Mukhed taluka, Nanded district (Marathwada) of Maharashtra, between 1992 and 2001, were analysed retrospectively. The local and systemic manifestations of snake-bite, response to antisnake venom, atropine and neostigmine, the treatment of complications and the outcome

Toxicity of South American snake venoms measured by an in vitro cell culture assay.

Cytotoxicity of venoms from eight medically important South American Crotalidae snakes (Bothrops and Lachesis genera) was determined, based on a procedure originally described for the screening of cytotoxic agents in general. The assay, the conditions of which were adapted to snake venoms, determines the survival of viable cells in monolayer culture upon exposure to the toxic agent. Snake venom toxicity was expressed as the venom dose that killed 50% of the cells (CT(50)) under the assay conditions. Bothrops neuwieddi mattogrossensis (CT(50)=4.74+/-0.35 microg/ml) and Bothrops leucurus (CT(50)

Pediatric Viper Envenomation Complicated by Compartment Syndrome.

Snake envenomations can cause both severe systemic and local complications. Acute compartment syndrome (ACS) is a rare but severe limb-threatening complication. Due to limited communicability and anticipated envenomation-related swelling, it is often subtle and difficult to diagnose in pediatric populations. A previously healthy three-year-old female presented one hour after sustaining a bite to the dorsum of her left foot by a saw-scaled viper (Echinus carinatus). A common viperidae in the region of Oman, it is responsible for a significant amount of annual envenomations leading to coagulopat…

Domain loss facilitates accelerated evolution and neofunctionalization of duplicate snake venom metalloproteinase toxin genes.

Gene duplication is a key mechanism for the adaptive evolution and neofunctionalization of gene families. Large multigene families often exh … (SVMPs) as a model system. Our results reveal that the evolutionary history of viperid (Serpentes: Viperidae) SVMPs is repeatedly punctuated by domain loss, with the single loss of the cysteine-rich domain, facilitating the formation of P-II class SVMPs, occurring prior to the convergent loss of the disintegrin domain to form multiple P-I SVMP structures. …

Characterization of hyaluronidase isolated from Agkistrodon contortrix contortrix (Southern Copperhead) venom.

Snake venoms are a rich source of enzymes including many hydrolytic enzymes. Some enzymes such as phospholipase A2, proteolytic enzymes, and phosphodiesterases are well characterized. However many enzymes, such as the glycosidase, hyaluronidase, have not been studied extensively. Here we describe the characterization of snake venom hyaluronidase. In order to determine which venom was the best source for isolation of the enzyme, the hyaluronidase activity of 19 venoms from Elapidae, Viperidae, and Crotalidae snakes was determined. Since Agkistrodon contortrix contortrix venom showed the highest…

Control of venom production and secretion by sympathetic outflow in the snake Bothrops jararaca.

Many studies have examined the morphological and biochemical changes in the secretory epithelium of snake venom glands after a bite or milking. However, the mechanisms of venom production and secretion are not yet well understood. The present study was undertaken to evaluate the role of the sympathetic nervous system in the control of venom production and secretion. Venom glands were obtained from Bothrops jararaca (Viperidae) snakes, either unmilked previously or milked 4, 7 or 15 days before they were killed. Levels of tyrosine-hydroxylase-like immunoreactivity were higher in venom glands co…

cDNA cloning and sequence analysis of a lysine-49 phospholipase A2 myotoxin from Agkistrodon contortrix laticinctus snake venom.

A cDNA clone (ACLPREMT1) for a K49 phospholipase A2 (PLA2) myotoxin from Agkistrodon contortrix laticinctus snake venom was isolated from a venom gland library and sequenced. The ACLPREMT1 cDNA is 734 bp in length and has an open reading frame of 414 bp. It codes for a K49 phospholipase A2 with 121 amino acid residues. The sequence of the first 20 amino acid residues of the predicted mature protein matches exactly with the N-terminal sequence of the purified myotoxin. Comparison of the ACLPREMT1 cDNA sequence with PLA2 cDNAs from Viperidae snakes shows that it has a similar organization: highl…

Snake bite associated with acute kidney injury.

Acute kidney injury (AKI) is a well-known life-threatening systemic effect of snake envenomation which commonly happens secondary to snake bites from families of Viperidae and Elapidae. Enzymatic toxins in snake venom result in injuries to all kidney cell types including glomerular, tubulo-interstitial and kidney vasculature. Pathogenesis of kidney injury due to snake envenomation includes ischaemia secondary to decreased kidney blood flow caused by systemic bleeding and vascular leakage, proteolytic degradation of the glomerular basement membrane by snake venom metalloproteinases (SVMPs), dep…

The hills are alive with the sound of venom: Clinical implications of intraspecific variations in Factor Va mediated activations of Factor VII and Factor X in Swiss populations of Vipera aspis (Asp Viper).

This study investigated the individual variation in the procoagulant toxicity of Vipera aspis venoms collected from multiple Swiss localities, in addition to the neutralisation capacity of three currently available antivenoms. Functional coagulation assays revealed quantifiable geographic differences in venom potency and clotting kinetics. Mechanistic analyses demonstrated that V. aspis venoms exert their procoagulant effects primarily through Factor Va-dependent action of the coagulation cascade, leading to accelerated thrombin generation and clot formation. These findings highlight the compl

Point-of-Care Ultrasound in Japanese Mamushi (Gloydius blomhoffii) Bites: Presence of Deep Subcutaneous Hypoechoic Bands.

Japanese mamushi (Gloydius blomhoffii) causes ∼3000 snakebites annually in Japan, but specific ultrasonographic findings have not been well characterized. Current severity assessment relies on visual grading systems based on local swelling and erythema, which may not provide sufficient information for timely and accurate treatment decisions. We present 3 pediatric patients (ages 7-9 y) with clinically diagnosed mamushi bites to the fingers, all presenting with Grade III envenomation (ie, swelling extending to the elbow). Point-of-care ultrasound examination revealed a remarkably consistent fin

Acute Cerebral Artery Occlusion Following Levantine Viper (Macrovipera lebetina) Bite: Gács Sign in Snakebite.

Acute cerebral thrombotic stroke following a viper snakebite is a rare occurrence. There have been a few documented cases of cerebral infarctions resulting from envenomation by various viper species. However, none of these reports have specifically detailed instances of stroke induced by Macrovipera lebetina envenomation or vessel occlusion without concurrent cerebral infarction. In this study, we present a case of middle cerebral artery (MCA) occlusion in a 56-year-old man who was bitten by a Macrovipera or Vipera lebetina snake. The individual experienced a snakebite on his right foot, leadi

Compromised Regeneration, Damage to Blood Vessels and the Endomysium Underpin Permanent Muscle Damage Induced by Puff Adder (Bitis arietans) Venom.

The puff adder (Bitis arietans) is a clinically relevant viper species found throughout Africa, and it is responsible for a greater incidence of health-related envenomations than all other snake species on the continent combined. Unresolved skeletal muscle damage is a common consequence of B. arietans envenomation that can result in long-term morbidity and even death. Antivenom treatment can mitigate the systemic effects of the venom but offers little protection against local tissue damage. Identifying the mechanisms through which B. arietans venom induces tissue damage and impedes skeletal mu
