The ocular findings in Kniest dysplasia.

We studied seven patients who had Kniest dysplasia. All of the patients had congenital severe myopia and vitreoretinal degeneration. Four patients developed rhegmatogenous retinal detachments. Early surgery on one patient resulted in successful reattachment and 20/50 vision at a three-year follow-up visit. Other ocular findings included cataracts in two patients, dislocated lenses in one patient, and blepharoptosis in one patient.

Heavy trypan blue staining of epiretinal membranes: an alternative to infracyanine green.

BACKGROUND: By using dyes, it is easier to identify the extent of an epiretinal membrane (ERM) or the inner limiting membrane (ILM) during surgery. Trypan blue (TB) stains ERM and ILM weakly, but with less apparent toxicity than other intraocular dyes. Its main drawback in vitreoretinal surgery is the requirement of an air-fluid exchange (AFX) before its use. AIM: To propose a modified form of TB denser than water, thus obviating the need for an AFX. DESIGN: A prospective, consecutive trial with heavy trypan blue in vitreoretinal surgery. METHODS: A consecutive group of patients with ERMs w…

Use of 30 gauge needle to prevent hypotony during silicone oil removal.

Silicone oil (SO) is used as an intraocular tamponade after complex vitreoretinal surgeries because of its properties such as transparency, inertness, high surface tension, and interfacial tension with water. The only disadvantage of SO tamponade is the need for a second surgery for its removal. However, there is a risk of ocular hypotony soon after the removal of the infusion cannula, especially in retinal vascular conditions, such as diabetic retinopathy. We hereby present a silicone oil removal (SOR) technique using a 30 G needle that prevents intraocular hypotony during SOR.

[Anatomical and functional outcomes of full-thickness macular hole treatment using the optimized method of autologous retinal transplantation (preliminary report)].

The introduction of autologous neurosensory retinal transplantation (ANRT) into vitreoretinal surgery has significantly improved the success rates of closure of refractory full-thickness macular holes (FTMH). In recent years, the technique has gained wide acceptance and its indications have expanded; however, certain aspects remain debatable - particularly the optimal graft size to ensure the best anatomical and functional outcomes. To address this issue, the study proposes a surgical technique for treating FTMH using ANRT that involves precise marking of the neurosensory retinal graft. The ef…

Management of antithrombotic therapies in patients scheduled for eye surgery.

The large majority of patients undergoing ophthalmic surgery are elderly and take systemic medications on a regular basis, including antiplatelet and anticoagulant treatments. It is current practice for many physicians to discontinue antithrombotic treatment prior to surgery to reduce bleeding complications that may lead to retrobulbar haemorrhage and, ultimately, to loss of vision. However, discontinuation of antithrombotic treatment in such patients may lead to thromboembolic events with serious consequences. The present narrative review highlights the risk of thrombosis when discontinuing a

Decreased angiogenin concentration in vitreous and serum in proliferative diabetic retinopathy.

Diabetic retinopathy is the most common cause of vision loss in young adults in developed countries. The disease therapy with anti-vascular endothelial growth factor (VEGF) agents gives some positive results, but is associated with retinal ischemia and vasoconstriction. Therefore, determination of factors involved in the physiological and pathological angiogenesis in the diabetic eye is of great importance for understanding of the pathogenesis of diabetic retinopathy and its effective treatment. Previously, we found that diabetic patients were characterized by increased serum concentration of

Retinal vascular occlusion after vitrectomy with retrobulbar anesthesia-observational case series and survey of literature.

BACKGROUND: Severe postoperative loss of vision has been occasionally reported as a rare complication of retrobulbar anesthesia, and several possible causes have been proposed in the literature. In this work, our own and other investigators' experiences with these complications are surveyed with a view to identifying its pathophysiology. PATIENTS: This observational case series refers to six patients who presented during a 3-month period with occlusion of either the central artery itself (n = 3) or a branch thereof (n = 3) 2-14 days after uneventful vitreoretinal surgery following retrobulbar…

Microsurgical management of macular epiretinal membranes (macular pucker).

Surgical management of selected macular epiretinal membranes is possible through the use of microsurgical pars plana vitreoretinal membranectomy. Visual improvement can be significant with resolution of traction retinal detachment and retinal distortion. Complications are similar to other forms of vitreous surgery. In a series of 9 selected cases with severe reduction of visual acuity secondary to macular pucker, microsurgical membranectomy was performed with encouraging results.

Cross Domain Self-Prompting SAM2 for Intraoperative OCT Video Segmentation.

Retinal procedures require precise instrument manipulation to avoid iatrogenic injuries. Intraoperative Optical Coherence Tomography (iOCT) delivers realtime cross-sectional views of tissue-instrument interactions, enabling safer surgical decision-making. Accurate iOCT segmentation would allow the automated estimation of the distance between surgical instruments and retinal surfaces, and the development of safety alerts for both manual and robotic vitreoretinal procedures. Although existing deep learning models have advanced iOCT segmentation, they operate frame-wise without temporal modeling
