Efficacy and safety of modafinil for improving daytime wakefulness in patients treated previously with psychostimulants.

OBJECTIVES: To assess the efficacy and safety of modafinil for improving wakefulness in narcolepsy patients treated previously with psychostimulants. BACKGROUND: Modafinil has become a standard therapy for improving daytime wakefulness in narcolepsy patients and may be a useful therapeutic alternative to psychostimulants used to improve waking function in other medical conditions. Modafinil is chemically dissimilar to and has a pharmacological profile that differs from the psychostimulants. Modafinil has a low abuse potential and is well tolerated. METHODS: Patients (N=151) with narcolepsy w…

Microsleep during a simplified maintenance of wakefulness test. A validation study of the OSLER test.

Daytime somnolence is both a symptom in many patients and a prevalent complaint in the general population, but its objective assessment remains elusive. The current available tests are technically complex and thus inadequate for routine clinical use or epidemiological studies. A simplified behavioral maintenance of wakefulness test (OSLER test) has been recently described that could allow for widespread availability of objective measurements of this symptom. We verified the occurrence of (micro)sleep (episodes of sleep of > or = 3 s duration) during the performance of the OSLER test in 10 norm…

Intracerebral microdialysis in clinical practice: baseline values for chemical markers during wakefulness, anesthesia, and neurosurgery.

OBJECTIVE: The study was undertaken to measure baseline values for chemical markers in human subjects during wakefulness, anesthesia, and neurosurgery, using intracerebral microdialysis. METHODS: Microdialysis catheters were inserted into normal posterior frontal cerebral cortex in nine patients who were undergoing surgery to treat benign lesions of the posterior fossa. The perfusion rate was 1.0 microl/min during anesthesia/neurosurgery and the early postoperative course and 0.3 microl/min during the later course. Bedside biochemical analyses of glucose, pyruvate, lactate, glycerol, glutamat…

Fast-CT evaluation of the upper airway during wakefulness in patients with obstructive sleep apnea.

Fast-CT scanning was used to study the dynamic changes in the upper airway (UA) during quiet tidal ventilation (VT) in 25 patients with obstructive sleep apnea (OSA). Ten fast-CT scans were sequentially obtained over one respiratory cycle from the level of the hard palate to the hypopharynx with the patients awake in the supine position. The patients were 44 +/- 2 years old, weighed 104 +/- 5 kg, and had moderately severe OSA with an apnea plus hypopnea index (AHI) of 59 +/- 7 per hour. Maximum and minimum UA cross-sectional areas (Amax -Amin) were determined for each UA level and percent redu

Bridging dream and wake states: a new tool for consciousness research.

In this study, the Consciousness States Comparison Scale: REM Version (CSCS-REM) was developed as a self-report instrument to assess dream consciousness during REM sleep in comparison with waking consciousness. Existing instruments capture only limited aspects of phenomenology and provide few opportunities for systematic cross-state evaluation. By anchoring waking consciousness as a reference point, the CSCS-REM quantifies relative differences in phenomenological experience and serves as a tool for empirically testing the predictions of the continuity and discontinuity hypotheses in consciousn

Autonomic regulation across sleep and wake during an Antarctic overwintering.

Isolated, confined and extreme environments like Antarctic overwinterings present significant challenges to human psychophysiological adapta … 24-hour periods every two months. Analysis revealed a decrease in parasympathetic regulation during wakefulness and an increase during sleep, in association with the increasing duration of isolation. At the same time, parasympathetic activity during sleep decreased during the polar night, suggesting a distinct seasonal effect. These findings offer novel insig …

Temporal drift of sleep-wake representations in hypothalamic neuronal ensembles.

Correlative and causal evidence implicates distinct genetically-defined and evolutionary-conserved hypothalamic neurons in regulating wakefulness, non-rapid eye movement (NREM), and rapid eye movement (REM) sleep. The prevailing view is that these circuits govern sleep-wake states by recruiting stable, invariant neuronal substrates, yet, this remains unknown. Here, we show that inhibitory, excitatory, hypocretins/orexins-, and melanin concentrating hormone-expressing neurons in hypothalamus did not exhibit stable state-specific activities using longitudinal single cell calcium imaging in freel…

Sleep-wake problems and fatigue in retinitis pigmentosa: A scoping review.

Retinitis pigmentosa (RP) is a group of hereditary retinal disorders causing lifelong visual decline. Sleep-wake problems, fatigue, and circadian disturbances may represent modifiable contributors to disease burden, but the evidence remains fragmented and has not yet been systematically synthesised. This scoping review aimed to map current evidence and identify knowledge gaps to guide future research. A systematic search was conducted across PubMed, Embase, Web of Science, and Cochrane Library to identify publications addressing sleep-wake problems, fatigue, and circadian rhythm disturbances i

The resurgence of malaria in Northern Iran in 2023: a wake-up call.

BACKGROUND: Control and elimination programs for malaria in Iran have consistently faced significant challenges due to various factors, including the presence of neighboring malaria-endemic countries such as Afghanistan and Pakistan. In recent years, Golestan Province in northern Iran has recorded few cases of imported malaria, with a sudden increase of 22 cases in 2023. This article provides an overview of the malaria situation in northern Iran, along with a detailed report of these 22 cases. METHODS: The study population comprises all individuals exhibiting symptoms suspected of malaria (n

Methylphenidate leads to disruptions in rest/wake patterns after discontinuation.

Children with attention-deficit/hyperactivity disorder (ADHD) experience more sleep problems than their peers. As stimulant medications, particularly methylphenidate (MPH), are the most common treatment for pediatric ADHD, there has been growing interest in whether such medications contribute to sleep disturbances and the development of sleep disorders later in life. Despite ongoing interest, evidence related to MPH on sleep functioning in children remains mixed. The present study investigated the effects of MPH on 24-hour rest/wake activity patterns and circadian rhythms in adolescent versus

Mood Disorders in the Wake of Traumatic Brain Injury: A Systematic Review.

Traumatic brain injury (TBI) frequently leads to a myriad of long-term consequences, among which mood disorders present a significant challenge. This systematic review delves into the complex interplay between TBI and subsequent mood disorders, focusing on research studies conducted over the past decade. Encompassing an age range from 12 years old to older adults (60+ years), our review aims to elucidate the epidemiological patterns, neurobiological mechanisms, and psychosocial factors that contribute to the development of mood disorders following TBI. By synthesizing the current literature, w

Morningness-Eveningness Preference and Motor Wake-Sleep Inertia in Adolescents.

The aim of the present work was to analyze possible differences in the wake-sleep and sleep-wake transition in relation to adolescents' circadian preference using actigraphy. Overall, 729 participants were enrolled in the research and 443 of them wore actigraphs on the non-dominant wrist for at least three nights. According to the reduced Morningness-Eveningness Questionnaire for Children and Adolescents cut-off scores, 61 participants belonged to the evening-type category, while 38 participants belonged to the morning-type. We extracted the motor activity counts, minute-by-minute, during the

Increased influenza severity in children in the wake of SARS-CoV-2.

The SARS-CoV-2 pandemic and subsequent interruption of influenza circulation has lowered population immunity to influenza, especially among children with few prepandemic exposures. Using data from a prospective pediatric cohort study based in Managua, Nicaragua, we compared the incidence and severity of influenza A/H3N2 and influenza B/Victoria between 2022 and two prepandemic seasons. We found a higher incidence of A/H3N2 in older children in 2022 compared with pre-2020 and a higher proportion of severe influenza in 2022, primarily among children aged 0-4, suggesting an influence of the SARS-

Neuromuscular Monitoring and Wake-Up Test During Scoliosis Surgery.

A typical spine is straight and symmetrical, with all of the vertebrae facing forward when viewed from the posterior. Scoliosis is a term used to describe a lateral deviation of the spine's normal vertical line that is greater than 10° on an X-ray. More severe curves are sometimes accompanied by medical issues such as increased back discomfort and cardiorespiratory difficulties along with the cosmetic components of the deformity. The test for spinal cord integrity prior to the widespread adoption of intraoperative neurophysiologic monitoring (IOM) was the wake-up test. In this article, we revi
