Rabies in Turkey: three human cases illustrating the importance of suspecting exposure.

Rabies, a fatal encephalitis of viral origin, is still a major health problem in the developing world. It begins, after exposure, with centripetal spread of the virus through peripheral nerves to the central nervous system. The virus proliferates there and spreads to the tissues via peripheral nerves. The diagnosis is not difficult when a nonimmunized patient presents with hydrophobia after a bite by a known rabid animal. Failure to identify an exposure and administer postexposure prophylaxis, however, can lead to a fatal outcome. We report 3 fatal cases of rabies in which the risk of developi

Venomous snakebite in mountainous terrain: prevention and management.

The prevention and management of venomous snakebite in the world's mountains present unique challenges. This paper presents a series of practical, clinically sound recommendations for management of venomous snakebite in a mountain environment. The authors performed an extensive review of current literature using search engines and manual searches. They then fused the abundant knowledge of snakebite with the realities of remote first aid and mountain rescue to develop recommendations. A summary is provided of the world's most troublesome mountain snakes and the mechanisms of toxicity from their

Cause of death in avalanche fatalities.

OBJECTIVE: Avalanches pose a life-threatening risk to participants of outdoor winter activities. Determining the causes of death in avalanche fatalities can aid rescue and resuscitation strategies and hopefully improve survival. METHODS: The study population included all avalanche fatalities in Utah from the 1989-90 to 2005-06 winter seasons. The Utah Avalanche Center and Medical Examiner records were reviewed to identify accident circumstances, autopsy findings, and causes of death. RESULTS: Fifty-six avalanche deaths were identified during the study period. Most deaths occurred while parti

Ventilatory responses to hypoxia and high altitude during sleep in Aconcagua climbers.

BACKGROUND/OBJECTIVE: We examined the changes in ventilation during sleep at high altitude using the LifeShirt monitoring system on 2 climbers who were attempting to summit Mount Aconcagua (6956 m). METHODS: Prior to the summit attempt, we measured cardiovascular and pulmonary function at 401 m (Rochester, MN) and gathered respiratory and cardiovascular data during sleep using the LifeShirt monitoring system with exposure to normobaric normoxia and normobaric hypoxia (simulated 4300 m). We then monitored the ventilatory response during sleep at 3 altitudes (4100 m, 4900 m, and 5900 m). RESUL

New-onset anxiety disorders at high altitude.

OBJECTIVE: Studies on the neurologic effects of high-altitude travel have focused on psychometric and cognitive testing and the long-term effects of hypoxia on memory and cognition. Few authors have discussed overt clinical psychiatric illness during high-altitude travel, and those few have focused on patients with preexisting psychiatric diagnoses. We describe a series of patients with new-onset anxiety disorders at high altitude treated at the Himalayan Rescue Association (HRA) clinic in Pheriche, Nepal (4240 m) in the spring season of 2006. METHODS: We report on all 6 cases of anxiety-rela

Pattern of injury after rock-climbing falls is not determined by harness type.

OBJECTIVE: Experimental data indicate that when using a sit harness alone, any major fall during rock climbing may cause life-threatening thoraco-lumbar hyperextension trauma or "head down position" during suspension. To clarify the actual influence of the type of harness on the pattern and severity of injury, accidents involving a major fall in a climbing harness were analyzed retrospectively. METHODS: Individuals with a height of fall equal to or exceeding 5 m were identified through a search of accident and emergency records for the period from 2000 to 2004. Data concerning the circumstanc

Caiman bite.

Caiman crocodilus, commonly called the spectacled caiman, is a very widely distributed resident of the western-hemisphere wetlands. Caiman bites to humans can cause trauma and infection. There are few reports of caiman bites; however, there is information about bites by other members of the same family, including Alligator mississippiensis. A case of acute caiman bite to the hand is described, including initial treatment and outcome. The bite resulted in multiple lacerations, interarticular fracture, and infection. One of the lacerations was closed, and the patient was started on moxifloxacin

Risk factors associated with fatal animal-vehicle collisions in the United States, 1995-2004.

OBJECTIVE: Animal-vehicle collisions are a significant public health concern in the United States. The annual economic cost currently exceeds $1 billion from injuries and property damage. A recent study by the Centers for Disease Control and Prevention evaluated nonfatal injuries from animal-vehicle collisions, but information on fatal animal-vehicle collisions is limited. This study evaluates risk factors associated with fatal animal-vehicle collisions. METHODS: This study evaluates characteristics of fatal animal-vehicle collisions in the United States from 1995-2004 by using the Fatality A

Alligator attacks on humans in the United States.

OBJECTIVE: Encounters with alligators are increasing in the United States. Both severe injuries and fatalities can occur from an alligator attack. This study provides information on alligator attacks reported in the United States as well as infections that are commonly associated with alligator bites. METHODS: In order to collect information on the number of alligator bites, nuisance calls, and estimated alligator population of each state, calls were made to wildlife offices in all southern US states, and an online search for lay press articles was performed. Detailed information was availabl

Crocodile attack in Australia: an analysis of its incidence and review of the pathology and management of crocodilian attacks in general.

Crocodilians represent one of the oldest constant animal lineages on the planet, in no small part due to their formidable array of predatory adaptations. As both human and crocodilian populations expand, they increasingly encroach on each others' territories, bringing morbidity and mortality to both populations. In this article, the medical and herpetologic literature pertaining to injuries caused by crocodilians is reviewed, and the patterns of saltwater crocodile attacks in Australia from 1971 to 2004 are analyzed. In this review, we examine the features of crocodilians that contribute to ex

Unique occupational hazards of Alaska: animal-related injuries.

OBJECTIVE: During 1992-2000, an average of 40 fatal occupational injuries and 12,400 nonfatal occupational injuries and illnesses related to animals were recorded each year in the United States, most involving domestic farm animals. Although Alaska has a relatively small farming industry, it supports several industries that require workers to regularly be in contact with animals. This study examines the pattern and characteristics of animal-related occupational injuries in Alaska. METHODS: Two data sources were accessed: the Alaska Trauma Registry for nonfatal injuries requiring hospitalizati

Ataxia at altitude measured on a wobble board.

OBJECTIVE: To establish a simple measure of ataxia for use at high altitude. METHODS: Twenty healthy subjects took part in a trek to 5005 m. At 5 different altitudes on the route, they undertook a balance test using a wobble board. The primary objectives were to investigate disturbances of ataxia at altitude and to correlate any observed disturbances with acute mountain sickness (AMS) scores. Secondary outcomes were correlations with cerebral regional oxygenation, pulse oximetry, and age. RESULTS: After a short learning curve, the wobble board test was found to be reproducible. Subjects over

A fatal attack caused by a lowland tapir (Tapirus terrestris) in southeastern Brazil.

A 55-year-old man was attacked by a Brazilian tapir (Tapirus terrestris) after surprising and stabbing the animal in his corn plantation. The victim received deep bites in the thighs, neck, and cervical areas, resulting in severe hemorrhage and death. This is the first report of a tapir incident resulting in death and is of interest because of the severity of the contusions and lacerations caused by the provoked animal.

Passive prophylaxis with permethrin-treated tents reduces mosquito bites among North American summer campers.

OBJECTIVE: Over 2 million adolescents participate in summer-camp experiences, placing themselves at risk for mosquito-borne illness. Insect repellent is recommended but often not used. It is unknown whether permethrin treatment of a location, such as a campsite, provides passive prophylaxis reducing mosquito bites among all persons in the immediate vicinity. METHODS: This randomized, double-blind controlled trial used 0.4% permethrin (2.5% permethrin diluted by 7 parts water) sprayed once onto the external surface of canvas tents at a Boy Scout summer camp. During a 9-week period, subjects (n

Leptospirosis in a caver returned from Sarawak, Malaysia.

This article describes a case of leptospirosis in a man who returned from caving in Sarawak, Malaysia, and includes a discussion of epidemiology, pathophysiology, diagnosis, prevention, and treatment. The patient presented with symptoms of leptospirosis, which was confirmed by microhemagglutination titers. He became infected despite taking doxycycline daily for malaria prophylaxis. Leptospirosis is an important consideration in any returned traveler with fever. The spirochete spreads from animals to humans via water. Caving in tropical endemic zones may increase exposure risk due to the combin
