Pediatric necrotizing soft tissue infection: unveiling a rare complication of routine procedures - peripheral venous catheter insertion.

Necrotizing soft-tissue infection (NSTI) is a rare yet potentially fatal condition, particularly in pediatric patients. We report the first known case of a previously healthy 1-year and 7-month-old boy who developed methicillin-resistant Staphylococcus aureus (MRSA) NSTI following peripheral venous catheter (PVC) insertion. The patient developed swelling and erythema at the PVC site. Contrast-enhanced computed tomography (CT) confirmed diffuse fascial inflammation consistent with NSTI, which guided the urgent decision for surgical intervention. Aggressive management with vancomycin, meropenem,

Bilateral periorbital necrotising fasciitis associated with invasive group: a Streptococcus infection.

A previously independent 56-year-old immunocompetent woman presented with septic shock in the setting of periorbital swelling and diffuse infiltrates on chest imaging. Blood cultures were positive for growth of group A Streptococcus (GAS). Broad spectrum antimicrobials were initiated with the inclusion of the antitoxin agent clindamycin. Necrosis of periorbital tissue was noted and surgical consultation was obtained. Débridement of both eyelids with skin grafting was performed. GAS was isolated from wound cultures and also observed on periorbital tissue microscopy. The final diagnosis was bila

Management of Fournier's gangrene--report of 7 cases and review of the literature.

Fournier's gangrene is a rare infection characterized with fast-progressing myonecrosis, that affect regions of perineum, genitalia and perianal area. This retrospective study presents authors' experiences and their principles in early diagnosis and treatment of Fournier's gangrene. The goal of this paper is to point out numerous diagnostically and therapeutic difficulties that lead to a high mortality if not recognized in time. We here describe seven male patients with myonecrosis and necrotising fasciitis in scrotal, perianal and perineal regions. Average age was 61 years (form 57 to 66 year

Skin and soft tissue infections.

The spectrum of skin and soft tissue infections seen by the primary care physician can be as benign as folliculitis to as life-threatening as NF. Cellulitis remains the most common skin and soft tissue infection seen in primary care. The ever present danger of ca-MRSA, however, has changed the way primary care physicians approach the common problem of cellulitis. The presence of risk factors for colonization with ca-MRSA and a history or examination finding of skin abscess should raise the suspicion of ca-MRSA, and antibiotic therapy should include TMP-SMZ, clindamycin, doxycycline, or minocyc

Management of equivocal (early) Fournier's gangrene.

BACKGROUND: Fournier's gangrene (FG) is an acute progressive necrotizing fasciitis of the genital area and perineum with possible extension to the abdominal wall. Surgical debridement is the gold standard management modality of established patients. Equivocal (early) FG represents a challenge in diagnosis. The objective of this study was to compare conservative management and early exploration in cases of equivocal (early) FG. METHODS: This was an observational study where data of all patients diagnosed as early FG in our departments over 4 years (2011-2015) were enrolled. Patients were divid

Study to evaluate the role of severity stratification of skin and soft tissue infections (SSTIs) in formulating treatment strategies and predicting poor prognostic factors.

BACKGROUND: Diagnosis of skin and soft tissue infections (SSTIs) is difficult as they commonly masquerade as other clinical syndromes. So, the prospective cohort study was carried out to study the clinical profile, evaluate the existing method of severity stratification of SSTIs and identify factors responsible for prolonged stay and poor outcome in patients with SSTIs. METHODS: The prospective study was carried out based on clinical profile to evaluate the role of severity stratification of SSTIs in 105 patients attending the surgical outpatient department and surgical emergency. The patient

Versatility of lateral cutaneous branches of intercostal vessels and nerves: anatomical study and clinical application.

The use of the intercostal artery perforator (ICAP) flap has recently become popular in reconstructions of the breast, upper arm and trunk. Lateral cutaneous branches (LCBs) are a group of the ICAPs that penetrate the fascia near the middle axillary line. However, reports on its precise anatomy and clinical applications are quite limited. We performed an anatomical study of LCBs using cadavers. Based on the findings, we developed novel clinical application methods as follows: (1) sensate superficial circumflex iliac perforator (SCIP) flap, (2) supercharged SCIP flap, (3) ICAP-based propeller f

Pyoderma gangrenosum with wrist joint destruction: case report.

Pyoderma gangrenosum (PG) is a rare, noninfectious, neurotrophic dermatosis. We observed a case of PG mimicking cutaneous and osteoarticular infections that presented with a prolonged ulcer on the forearm, severe wrist pain, anemia, substantial local and systemic inflammation as evaluated by serum laboratory data, and carpal osteolysis. Although PG rarely damages joints, the ulcer extended to the joint and destroyed the osteochondral tissues. Advanced ulcerative colitis, which is a most common comorbidity of PG, proved to be an underlying disease. Antibiotic and surgical treatment did not heal

Systemic T-cell lymphoma presenting as an acute nonresolving eyelid mass.

A healthy 39-year-old man was referred with a 3-week history of nonresolving right upper eyelid cyst with no globe or orbital involvement. Histopathologic examination revealed a peripheral T-cell lymphoma of unspecified type, which was stage 3 on staging. He received combination cyclophosphamide, hydroxydoxorubicin, oncovin, and prednisolone therapy with an autologous bone marrow transplant and remains in complete remission 24 months posttreatment. A brief literature review of ocular adnexal T-cell lymphomas is presented.

Deep neck infections: clinical considerations in aggressive disease.

Deep neck infections are common and occur as a consequence of several etiologies. Antibiotic therapy, interventional radiology, and patient support modalities have become increasingly sophisticated, although surgery continues to be the mainstay of treatment for most patients. Today, neck infections are rarely life threatening when sound and timely management is applied.

The distally based forearm island flap in hand reconstruction.

The distally based forearm island flap is vascularized by the perforators of the distal radial artery. The skin flap is along the axis of the radial artery, and the pivot point of its subcutaneous pedicle is about 2 to 4 cm above the radial styloid process. We have treated 12 patients with 12 flaps for soft-tissue defects of the hand. Of these recipient sites, seven were in dorsal hands, two were in thumbs, two were in forearms, and one was in the palmar area. The donor-tissue variants included eight skin flaps, two adipofascial flaps, and two sensate flaps. The sizes of the flaps ranged from

MR imaging of soft-tissue infection.

MR imaging is the ideal technique to display the soft-tissue abnormalities produced by infectious processes. In general these soft-tissue alterations consist of signal intensity changes that reflect the increased water content of the soft tissues induced by the inflammatory reactions. In some cases these changes are nonspecific, but in others, MR imaging can be quite specific and helpful in detecting the presence and extent of the infection, which generally is suspected clinically on the basis of physical and laboratory findings. The MR manifestations of soft-tissue infection, including septic
