Do cardiologists and cardiac surgeons need ethics? Achieving happiness for a drug user with endocarditis.

Ethical dilemmas are commonplace in clinical cardiology. There has been a recent focus on ethical behaviour of cardiologists and debate about resource allocation and cost-effectiveness of new technologies. The case of an intravenous drug addict, with native aortic valve endocarditis complicated by a cerebral abscess and severe aortic regurgitation, is presented to illustrate some common ethical and moral dilemmas. The predominant theories in medical ethics, including the "Four-Principles Approach," is discussed, and a model to translate these ethical theories into a clinical decision-making to

Disseminated nocardiosis: a rare infectious complication following non-heart-beating donor liver transplantation.

Nocardiosis is an infrequent disease that affects patients who display a cellular immunodeficiency, such as transplant recipients on immunosuppressive treatment, but uncommonly associated with high morbidity and mortality rates. Disseminated Nocardiosis affecting the central nervous system (CNS), abdomen, skin, and lungs has been described in bone marrow, lung, and kidney transplant recipients. However, to our knowledge, no cases involving all of these structures have been reported in liver transplant recipients. Herein, we have reported a case of CNS, pulmonary, and cutaneous nocardiosis in a

[Incidence of complications of otitis media].

OBJECTIVE: Determine the incidence of otological and endocranial complications of acute otitis media, taking into account previous treatments and the development of complications. MATERIAL AND METHODS: Retrospective follow-up study between March 1996-2003 including 16 patients: 9 men, 7 women. RESULTS: Nine patients (56.25%) had intracranial complications and seven otological ones. In the first group, 6 developed meningitis by Streptococcus pneumoniae, one sigmoid sinus thromboflebitis, one multiple abscesses, and one subdural empiema and encephalitis. Otological complications were six acute

Defining bacterial meningitis and other infections of the central nervous system.

OBJECTIVE: To define central nervous system infections of infants and children that occur as co-morbid or predisposing conditions of sepsis. DESIGN: Standard pediatric infectious disease references and the pertinent literature in English were reviewed from 1960 to 2002 to ascertain the previous methods and definitions utilized in clinical studies of the epidemiology and treatment of bacterial infections of the central nervous system. An accepted definition of bacterial meningitis defined by the Infectious Disease Society of America was reviewed and adapted to the previous clinical definitions

Neonatal adrenal abscess revisited: the importance of raised inflammatory markers.

Adrenal abscesses rarely occur in neonates and usually present with non-specific signs and symptoms. Prompt diagnosis requires an index of suspicion. We describe right-sided adrenal haemorrhage and abscess formation in a newborn with hypoxic-ischaemic encephalopathy following maternal post-partum haemorrhage and sepsis with Escherichia coli and Enterococcus faecalis. Percutaneous drainage of the abscess identified an E. coli isolate identical to that in the mother plus Candida albicans.

[Chronic inflammation of the petrous bone with fatal outcome].

We report the case of a 44-year-old woman, suffering from lethal cerebritis of the temporal lobe caused by an unusual chronic otitis media with granulations. A chronic inflammatory process of the left temporal bone had affected the patient over a period of 4 years. This had started with mastoiditis accompanied by a lesion of the dura mater, leading to labyrinthitis, encephalitis and cerebellar abscess. The patient's symptoms persisted, especially vertigo and uncertainty, and then deteriorated acutely. MRI showed otogenic cerebritis. Intracranial bleeding from vasculitis had worsened the patien

Nocardial infection in a renal transplant recipient on tacrolimus and mycophenolate mofetil.

Infection with Nocardia spp. is an uncommon but important cause of morbidity and mortality in organ transplant recipients. Cotrimoxazole prophylaxis against urinary tract infection and Pneumocystis carinii pneumonia in these patients usually prevents nocardial infection also. We report the case of a patient on tacrolimus and mycophenolate mofetil who developed drug-induced diabetes mellitus followed by nocardial brain infection. This infection occurred despite conventional cotrimoxazole prophylaxis. Physicians should be aware that newer, more potent and more diabetogenic immunosuppressive regi

Gram-negative bacterial pneumonia with secondary aspergillosis in an AIDS patient.

A 40-year-old, HIV-infected female patient received antibiotic treatment for a urinary tract infection. After the initial success of therapy and a symptom-free period, she developed pneumonia with septic shock and adult respiratory distress syndrome (ARDS). In spite of intensive care and respirator therapy with positive end-expiratory pressure (PEEP), she died of infectious toxic shock. Autopsy findings showed relapsing, gram-negative, bacterial pneumonia (morphologically compatible with Klebsiella pneumonia) and secondary, invasive aspergillosis. The pathogenesis and epidemiology of these unu

Actinomycetales infection in the acquired immunodeficiency syndrome.

Four parenteral drug abusers with the acquired immunodeficiency syndrome had nonmycobacterial actinomycetales infections. Three patients had nocardiosis and one developed a streptomyces lymphadenitis. There was pericardial involvement in two patients, and two patients died. Presumptive diagnoses were often incorrect, highlighting the risks of empiric therapy in these patients. Four of the nine patients with the acquired immunodeficiency syndrome and nocardia or streptomyces infections whose cases were reported to the Centers for Disease Control also had mycobacterial disease. A common suscepti

[Current therapeutic orientation in intracranial suppuration].

Changes in diagnosis and management of intracranial infections have been studied on a continuous series of 102 cases 1968 through 1980. Use of CT scan has not increased the number of patients diagnosed in acute phase (before the fifteenth day) but has increased the rate of case observed before the second day (37 p. cent with and 27 p. cent without the CT scan). However the neurological status and specially the state of consciousness has not changed. Four specific data of abscess diagnosis have been defined on 56 cases suspected to have an intracranial infection; 16 out of these cases have prov

[Approaches to antibiotic therapy in anaerobic infections].

After a short introduction pointing out the difficulties faced by an italian clinician when he has to apply a correct aetiologic therapy to the non-sporing anaerobe infections, difficulties which are especially due to the lack of an efficient microbiologic support, the A. examines the main infections caused by these bacteria and shows the treatment for each case according to the more recent literature and to his own experience.

Calcified cystic lesion in cerebellum: A case report.

Intracranial epidermoid cysts are benign, slow-growing congenital tumors of ectodermal origin. They are rare embryonal benign cystic masses … image suggesting an intra-axial mass suspected of low-grade glioma with a differential diagnosis of brain abscess. The hypointense lesion on the T1WI sequence found in the MRI examination, with no significant contrast enhancement and restricted diffusion area on DWI, was one of the notable features described in the epidermoid cyst. Intracranial epidermoid cyst …

Primary odontogenic onset invasive mucormycosis-an under recognized clinical entity.

OBJECTIVES: The primary source of facial mucormycosis is through inhalation of fungal sporangiospores, resulting in invasive disease in paranasal sinuses. However, dental onset mucormycosis has not been well documented in literature. The aim of this study was to describe the clinical characteristics and outcomes of patients with odontogenic onset mucormycosis. METHODS: From a large cohort of mucormycosis involving the face between July 2020 and October 2021, we selected patients who had dental symptoms at onset and predominant alveolar involvement with little to no paranasal sinus disease as
