Parenteral nutrition-associated cholestasis and postnatal growth trajectories in preterm infants: A longitudinal cohort study.

OBJECTIVES: To evaluate the association between parenteral nutrition-associated cholestasis and postnatal weight and linear growth trajectories to 12 months corrected age, and to assess whether cholestasis mediates the relationship between parenteral nutrition duration and 12-month linear growth in preterm infants. METHODS: Retrospective cohort study of a standardized neonatal nutrition and follow-up database including infants born at <32 weeks' gestation. Cholestasis was defined as peak direct bilirubin ≥34.2 μmol/L. Longitudinal weight and length z-scores were analysed using generalized est

A randomized trial of an exclusive human milk diet in neonates with single-ventricle physiology: 3 and 6 months follow-up outcomes.

BACKGROUND: A prospective randomized controlled trial of exclusive human milk nutrition (EHM) in infants with single-ventricle physiology (SVP) was previously published. This study evaluates growth and health outcomes at 3-6 months. METHODS: Demographics, anthropometrics, and clinical outcomes were collected prospectively. Anthropometric z-scores were calculated using WHO standards. RESULTS: One hundred seven infants were randomized to EHM or control diet, with 23 infants meeting predefined exclusion criteria, 8 infants withdrawn/died, leaving 76 infants at discharge. At 3-6 months, 74 infan

The high-risk minority: undetected preterm SGA disproportionately contributes to adverse outcomes in a Chinese cohort.

INTRODUCTION: Small for gestational age (SGA) infants face higher risk of adverse outcomes compared to appropriate for gestational age (AGA) infants. Whether undetected SGA leads to worse clinical outcomes than detected SGA remains debated. This study compared perinatal outcomes between undetected and detected SGA in preterm and term pregnancies, and identified risk factors for missed diagnoses. MATERIAL AND METHODS: Singleton deliveries > 24 weeks at Peking University Third Hospital (January 2018-June 2023) were included. In this single-center retrospective study, participants were stratifie

A newborn with type 1 Bartter syndrome: challenges in the treatment and development during 30 months follow-up-a case report.

Type I Bartter syndrome is a rare autosomal recessive tubulopathy resulting from mutations in the SLC12A1 gene, leading to defective sodium-chloride reabsorption in the thick ascending limb of the loop of Henle. Affected neonates typically present with profound fluid and electrolyte disturbances, polyuria, and metabolic derangements. Early diagnosis and individualized management are crucial to prevent life-threatening complications and support appropriate growth and development. We report the case of a male preterm infant born at 33 weeks' gestation following a pregnancy complicated by severe

Early-life mucosal T cells direct intestinal stem cell fate via a coordinated developmental program.

Although early-life immunity was once considered immature, the human fetal immune system is dynamic and compartmentalized by the second trimester. By 21 weeks of gestation, T lymphocytes become a major immune population in the fetal small intestine (SI), yet their functional roles within this tissue remain largely undefined. To explore their unique contributions to intestinal development, we established an ex vivo co-culture system in which mucosal T cells isolated from fetal, neonatal, or adult SI donors were cultured with tissue-derived 3D SI organoids derived from various ages. Homeostatic

Staged controlled bowel expansion prior to serial transverse enteroplasty in preterm neonates with short bowel syndrome: A safe and feasible strategy to enable intestinal reconstruction.

BACKGROUND: Preterm neonates with short bowel syndrome represent one of the most challenging populations in pediatric intestinal rehabilitation. Autologous intestinal reconstruction is frequently delayed in this group because insufficient bowel dilation precludes safe lengthening, prolonging dependence on parenteral nutrition and increasing the risk of intestinal failure-associated liver disease, sepsis, and growth failure. METHODS: We conducted a retrospective single-center cohort study of preterm neonates with short bowel syndrome managed with a standardized protocol of controlled bowel exp

Neonatal and Two-Year Prognosis of Eutrophic Newborns from Monochorionic Diamniotic Twin Pregnancies Complicated by Selective Intrauterine Growth Restriction.

BACKGROUND: Monochorionic diamniotic (MCDA) twin pregnancies are at risk of complications, particularly selective intrauterine growth restriction. The objective of this study was to evaluate the two-year neurologic outcomes of the eutrophic newborns from monochorionic diamniotic twin pregnancies who were complicated by selective intrauterine growth restriction, compared to newborns from uncomplicated MCDA pregnancies. Our hypothesis was to determine whether selective IUGR in these pregnancies was specifically associated with a risk of delayed psychomotor development at two years old. METHODS:

Gastrointestinal tract organoids as novel tools in drug discovery.

Organoids, characterized by their high physiological attributes, effectively preserve the genetic characteristics, physiological structure, and function of the simulated organs. Since the inception of small intestine organoids, other organoids for organs including the liver, lungs, stomach, and pancreas have subsequently been developed. However, a comprehensive summary and discussion of research findings on gastrointestinal tract (GIT) organoids as disease models and drug screening platforms is currently lacking. Herein, in this review, we address diseases related to GIT organoid simulation an

Effects of multistrain Bifidobacteria and Lactobacillus probiotics on HMO compositions after supplementation to pregnant women at threatening preterm delivery: design of the randomized clinical PROMO trial.

BACKGROUND: As an indigestible component of human breast milk, Human Milk Oligosaccharides (HMOs) play an important role as a substrate for the establishing microbiome of the newborn. They have further been shown to have beneficial effects on the immune system, lung and brain development. For preterm infants HMO composition of human breast milk may be of particular relevance since the establishment of a healthy microbiome is challenged by multiple disruptive factors associated with preterm birth, such as cesarean section, hospital environment and perinatal antibiotic exposure. In a previous st

Impact of antenatal corticosteroids-to-delivery interval on very preterm neonatal outcomes: a retrospective study in two tertiary centers in Japan.

BACKGROUND: Antenatal corticosteroids (ACS) are administered to prevent neonatal complications and death in women at risk of imminent preterm birth. Internationally, the optimal interval from ACS to delivery (ACS-to-delivery interval) is within seven days; however, evidence in Asian populations specifically is limited. This study aimed to investigate the association between ACS-to-delivery interval and the incidence of neonatal complications in Japan. METHODS: This retrospective observational study enrolled singleton neonates born preterm at < 32 weeks of gestational age between 2012 and 2020

Long-term efficacy of sarilumab on the progression of interstitial lung disease in rheumatoid arthritis: the KEIO-RA cohort and literature review.

BACKGROUND: Necrotizing enterocolitis (NEC) is a common complication among preterm infants but can also occur in term babies. A potential relationship between the onset of NEC and hyperglycemia has been suggested, but some studies report no association, necessitating further research to clarify the link between the two conditions. This study aimed to determine the proportion of NEC cases with hyperglycemia 72 h before its onset in preterm and term infants, identify clinical factors associated with hyperglycemia in these infants, and investigate the relationship between hyperglycemia and in-hos

Opportunistic Features of Non-Clostridium botulinum Strains Containing bont Gene Cluster.

The cluster of genes determining the production of botulinum toxins is an attribute of not only the Clostridium botulinum species. This cluster is also found in other members of the Clostridium genus, such as C. baratii, C. butyricum, and C. sporogenes. The occurrence of a botulinum-like cluster has also been recorded in strains of other genera, i.e., Enterococcus faecium, as well as in a Gram-negative species isolated from freshwater sediments; however, the biological activity of bont-related genes has not been noted. It can be said that the mentioned species have a dual nature. Another speci

Early Surfactant Therapy for Respiratory Distress Syndrome in Very Preterm Infants.

BACKGROUND: It is currently considered that early initiation of nasal continuous positive airway pressure, using a less invasive exogenous surfactant administration and avoiding mechanical ventilation as much as possible to minimize lung damage, may reduce mortality and/or the risk of morbidities in preterm infants. The aim of our study was to quantify our experience and compare different strategies of surfactant administration, to investigate which method is associated with less morbidity. MATERIALS AND METHODS: A total of 135 preterm infants with early rescue surfactant administration for r

Comparison of Three Modalities of Feeding in Preterm Infants ≤32 Weeks and ≤1,250 G: A Randomized Controlled Trial.

OBJECTIVE: Early establishment of enteral feeds is desirable in very preterm infants, but it may be associated with feeding intolerance. Several feeding methods have been studied with no strong evidence to suggest the preferred feeding method to establish early full enteral feeds. We studied three modalities of feeding in preterm infants ≤32 weeks and ≤1,250 g: continuous infusion (CI), intermittent bolus by infusion (IBI), and intermittent bolus by gravity (IBG) for their effect on time to reach full enteral feeds of 180 mL/kg/d. STUDY DESIGN: We randomized 146 infants, 49 infants in each CI
