Chemotherapy extravasation injuries beyond the immediate stage: A series of 15 cases treated according to a preset surgical algorithm based on time of presentation.

Chemotherapy extravasation can cause severe harm. There is a lack of evidence-based standardization on the surgical management of such injuries beyond the immediate stage. In an algorithm connecting presentation time post-injury with surgical treatment could help standardize future treatment. This study prospectively validated a preset standardized surgical algorithm based on presentation time in a consecutive series between October 2017 and October 2020. Chemotherapeutic agent, site and extent of injury, type of surgery and outcome at a minimum of 6 months' follow-up were collected. Seven tho

The role of negative-pressure wound therapy in Fournier's gangrene: Association with mortality.

BACKGROUND: Fournier's gangrene (FG) is a rare but life-threatening necrotizing fasciitis of the perineal and genital regions characterized by rapid progression, fulminant sepsis, and high mortality. Negative-pressure wound therapy (NPWT), also called vacuum-assisted closure (VAC), is becoming more commonly utilized today in the treatment of complex wounds. This study aimed to evaluate the association between NPWT/VAC and in-hospital mortality among patients with FG requiring extensive surgical debridement. METHODS: This retrospective cohort study included 95 patients treated for FG at a tert

[Post-Traumatic Hypervirulent Klebsiella pneumoniae Infection: A Rare Case Presenting with Liver Abscess and Endogenous Endophthalmitis].

Hypervirulent Klebsiella pneumoniae (hvKp) is a pathotype characterized by high virulence capacity and rapid dissemination, which has gained increasing importance in recent years. It can cause invasive infections with multi-organ involvement, such as liver abscess, necrotizing fasciitis, meningitis and endophthalmitis and represents a significant public health threat due to its high mortality rates. Although numerous studies have investigated its risk factors, trauma-associated cases are extremely rare and different hypotheses have been proposed regarding their pathophysiology. The most widely

Extremity Soft-tissue Reconstruction With the Conjoined Latissimus Dorsi-Groin Flap.

Extensive soft-tissue defects in the extremities pose a significant challenge in reconstructive surgery. The combined latissimus dorsi (LD) and groin flap technique, a method developed by Harii et al in 1981, has shown promise in treating such defects. The purpose of this study was to illustrate the technique and results of using combined LD and groin flaps to reconstruct extensive upper and lower extremity defects. We present 4 cases of extensive soft-tissue defects treated with the combined LD-groin flap. The report details the surgical approach, including patient positioning in the semisupi

It's time to act: Understanding and combating Vibrio vulnificus.

Vibrio vulnificus is a highly pathogenic marine bacterium that can cause life-threatening conditions such as septicemia and necrotizing fasciitis; in medically fragile individuals, mortality rates can exceed 50%. Its virulence factors, including the MARTX toxin and hemolysin VvhA, function to disrupt host cells, evade immune responses, and facilitate bacterial dissemination. Climate warming has dramatically expanded the geographic range of V. vulnificus, doubling infection risks in high-latitude northern waters, while microplastic pollution and extreme weather events further enhance its ecolog

Matrix-assisted laser desorption/ionization-time of flight MS as a tool for clonal discrimination in Streptococcus dysgalactiae subsp. equisimilis.

Introduction. Streptococcus dysgalactiae subsp. equisimilis (SDSE) is an emerging pathogen closely related to Streptococcus pyogenes, causing infections from mild to severe, including necrotizing fasciitis and toxic shock syndrome. Understanding bacterial diversity is crucial for monitoring the spread of antimicrobial-resistant and highly virulent strains. Clonal analysis by whole-genome sequencing (WGS) is costly, time-consuming and requires specialized bioinformatics expertise. PFGE-defined clones are often linked to human infections, but PFGE is also demanding and costly and results can var
