McCune-Albright syndrome in a boy may present with a monolateral macroorchidism as an early and isolated clinical manifestation.

BACKGROUND: Testis enlargement in McCune-Albright syndrome (MAS) is generally bilateral and associated with clinical and biochemical manifestations of sexual precocity. CASE REPORT: We describe for the first time an unreported clinical expression of MAS in a 4.6-year-old boy presenting with monolateral testis enlargement and no signs of sexual precocity or other clinical manifestations of MAS at the time of presenting with macroorchidism. Both testosterone and LHRH-stimulated gonadotropin levels were in the prepubertal range. Serum inhibin B was increased to a pubertal level indicating Sertol

Resective surgery for hypothalamic hamartoma.

Hypothalamic hamartoma presents with precocious puberty, epilepsy or both. There are two epileptic syndromes, one presenting initially in infancy with gelastic seizures evolving rapidly into a syndrome with multiple seizures, developmental delay and a moderate to severe behaviour disorder. The other presents later with a milder epileptic syndrome, again usually including gelastic seizures, but with normal intellect and behaviour. Magnetic resonance imaging identifies and gives a detailed anatomical picture of these lesions. Direct surgery, using microsurgical techniques and neuronavigation gui

Vaginal bleeding in young children.

To evaluate prospectively vaginal bleeding in young children with emphasis on the cause, management and outcome, we analyzed the data of all children aged under 10 years old who presented with vaginal bleeding at the King Chulalongkorn Memorial Hospital between 1981 and 2000. There were 55 girls presenting with vaginal bleeding with variable degrees of severity. In 41 of the 55 girls (74.5%), the bleeding resulted from a local lesion of the genital tract. Genital tumors were seen in four girls, ie hemangioma of the vulva, sarcoma botryoides of the vagina, functional ovarian cyst and granulosa

Ovarian function in three female patients with McCune-Albright syndrome with persistent autonomous ovarian activity.

Autonomous ovarian activity persists throughout adolescence in some patients with McCune-Albright syndrome (MAS). There have been few studies of longitudinal assessment of ovarian function in these patients. We investigated the first morning voided urinary gonadotropin and ovarian steroid levels consecutively in three patients aged 3 to 7 years after withdrawal of therapy for precocious puberty. They had the triad of MAS with onset of menses within the first 3 years of life. Excessively elevated urinary estrogen levels with one or two peaks per cycle were found in all patients. In two patients

Impact of endocrine hyperfunction and phosphate wasting on bone in McCune-Albright syndrome.

Skin dysplasia, as café-au-lait spots, bone fibrous dysplasia and peripheral endocrinopathies are the main clinical features of McCune-Albright syndrome (MAS). This illness is due to activating mutations of the Gsalpha protein and is spread with a mosaic pattern in affected tissues that consist of intermixed areas of normal and mutated cells. Peripheral endocrine secretion, free of hypothalamic pituitary control, is the hallmark of the endocrine syndromes: precocious puberty, Cushing's syndrome, hyperthyroidism and gigantism/acromegaly. In addition, phosphate wasting as hyperphosphaturia is of

Fibrous dysplasia of bone.

Fibrous dysplasia of bone is a disease that can involve one or several bones and is characterized by bone deformities, pain and iterative fractures. Some patients can present with endocrine dysfunction (generally precocious puberty) and cutaneous café-au-lait spots. Some complications, such as nerve compression and malignant transformation, are uncommon. Many patients can, however, be asymptomatic. Diagnosis relies on X-ray examination and pathology. Prognosis is assessed by X-rays and markers of bone remodelling. Several breakthroughs in the understanding of the pathophysiology have been made

LH-RH analogues: I. Their impact on reproductive medicine.

In the 28 years that have passed since the elucidation of the structure of luteinizing hormone-releasing hormone (LH-RH), diverse clinical applications in the field of reproductive medicine and related fields have been established for agonists of LH-RH, based on inhibition of the pituitary-gonadal axis. Various clinical investigations with agonists of LH-RH in benign gynecologic disorders, polycystic ovary disease (PCOD), in vitro fertilization-embryo transfer (IVF-ET), benign prostatic hypertrophy (BPH), precocious puberty and contraception were reviewed. LH-RH antagonists inhibit LH, follicl

McCune-Albright syndrome progressing with severe fibrous dysplasia.

We present the case of an 11-year-old girl with McCune-Albright syndrome associated with severe fibrous dysplasia. In addition to bone lesions, she has apparent manifestations of precocious puberty. In examination, a mass at the mentum spreading to mandibular corpus bilaterally was seen. This mass has affected the mandibular teeth. It was nearly 20 x 20 x 15 cm in size and had local necrotic regions on it. The lower lip was expanded too much by the mass. Another mass filled the left maxillary sinus, expanding the left zygomatic region outwardly and closing the left nasal fossa completely. A re

Depressive symptoms associated with gonadotropin-releasing hormone agonists.

The gonadotropin-releasing hormone (GnRH) agonists are a relatively new class of drugs that are potentially effective in treating disorders that are aggravated either by estrogen or testosterone. GnRH agonists are effective in the treatment of endometriosis, as well as other disorders, such as advanced prostrate cancer, precocious puberty and uterine leiomyomata. While the GnRH agonists reduce the extent of the endometrial lesions and the occurrence of pelvic pain associated with endometriosis, these agents are associated with physical and psychiatric side effects. The adverse effects of these

Clinical uses of gonadotropin-releasing hormone analogues.

Gonadotropin-releasing hormone (Gn-RH) analogues are synthetic derivatives of the native hypothalamic peptide with alterations in their chemical structure that result in changes in biologic activity. Several Gn-RH agonists are available for clinical use, and all act through the same mechanism: first to stimulate and then to inhibit gonadotropin and gonadal steroid secretion by downregulating the pituitary Gn-RN receptors. This review should provide clinicians with a working knowledge of the physiologic and pharmacokinetic features of Gn-RH agonists. Although over 2000 articles concerning Gn-RH

Mammosomatotroph hyperplasia associated with acromegaly and hyperprolactinemia in a patient with the McCune-Albright syndrome. A histologic, immunocytologic and ultrastructural study of the surgically-removed adenohypophysis.

An 11-year-old girl, with the McCune-Albright syndrome, exhibited fibrous dysplasia of several bones, skin pigmentation, precocious puberty, growth hormone hypersecretion, acromegaly and hyper-prolactinemia. Histologic, immunocytologic and ultrastructural investigation of the surgically-removed pituitary showed massive mammosomatotroph hyperplasia. Since no adenoma was found, the abundance of these bihormonal cells, capable of producing both growth hormone and prolactin, was implicated in the causation of growth hormone and prolactin excess. Somatoliberin overproduction and/or somatostatin and

Embryonal carcinoma of the ovary: a clinicopathologic entity distinct from endodermal sinus tumor resembling embryonal carcinoma of the adult testis.

The clinical and pathologic features of 15 examples of a hitherto undescribed germ cell tumor of the ovary are delineated. This tumor resembles the embryonal carcinoma of the adult testis and may be distinguished from the endodermal sinus tumor on the basis of its histologic and immunohistochemical characteristics. An indirect immunoperoxidase method for the localization of human chorionic gonadotropin (HCG) and alpha-fetoprotein (AFP) was done on formalin-fixed paraffin-embedded tissue from 10 neoplasms; HCG was present in all 10 neoplasms, and AFP was found in seven. HCG was indentified only

Added-Value of Diffusion-Weighted Imaging (DWI) and Dynamic Contrast-Enhanced (DCE-MRI) Magnetic Resonance Imaging in the Preoperative Assessment of Cervical Cancer.

OBJECTIVES: To optimize the indications for treating girls with idiopathic central precocious puberty with GnRH analogues, since outcomes may vary. DESIGN: Comparison of adult heights with the data at initial evaluation and the target heights. PATIENTS: Group 1 patients (n = 43) were treated with GnRH analogue from 7.9 +/- 0.2 years to 10.8 +/- 0.1 years (bone age: 10.3 +/- 0.2 years to 12.2 +/- 0.1 years) and group 2 patients (n = 29) were monitored without treatment because their predicted adult heights were > 155 cm. The criteria for treatment were a predicted height < 155 cm and/or a LH/

The role of Spexin and Phoenixin-20 in girls during puberty.

BACKGROUND: Spexin has an inhibitory effect on the hypothalamus (GnRH expression), pituitary (LH and FSH expression) and ovary (estradiol expression). However, Phoenixin potentiate GnRH-stimulated GnRH receptor expression. Our aim is to assess the role of these two functionally opposite peptides in girls with Central Precocious Puberty (CPP). METHODS: This prospective study was conducted on three groups of patients (CPP: 33 patients, premature thelarche (PT): 57 patients, and healthy controls: 53 patients). The patients’ anthropometric measurements, pubertal stage, bone age, basal FSH, LH, and

Role of polygenic risk in susceptibility to accelerated pubertal onset following chronic stress exposure.

OBJECTIVE: Previous finding suggests that children growing up under chronic stress tend to experience earlier sexual maturity. The present study aims to examine polygenic risk by experience interaction in predicting pubertal timing, as well as provide insight regarding the relevance of two G × E paradigms. DESIGN AND METHODS: Data were analyzed from a 3-year prospective puberty cohort in Anhui Province, China. Breast Tanner stage and testicular volume (TV) of 997 children were annually assessed. The polygenic risk score (PRS) was computed based on 17 SNPs for early pubertal timing. Hair corti

Gut Microbiota and Short-Chain Fatty Acids: Key Factors in Pediatric Obesity and Therapeutic Targets.

Obesity in the pediatric population has become a public health problem with an increasing prevalence in recent years. The gut microbiota and its metabolites, especially short-chain fatty acids (SCFAs), play vital roles in the development of pediatric obesity. This manuscript aims to highlight the link between the gut microbiota and obesity by examining microbial imbalances, such as altered Firmicutes/Bacteroidetes ratios and altered SCFA production, especially butyrate, propionate, and acetate. Advances in genomic technologies, such as 16S rRNA sequencing, have enabled personalized treatments

Unusual imaging findings associated with abdominal pediatric germ cell tumors.

Germ cell tumors of childhood are tumors arising from germline cells in gonadal or extragonadal locations. Extragonadal germ cell tumors are characteristically located in the midline, arising intracranially or in the mediastinum, retroperitoneum, or pelvis. These tumors are generally easily diagnosed due to typical sites of origin, characteristic imaging findings, and laboratory markers. However, germ cell tumors can be associated with unusual clinical syndromes or imaging features that can perplex the radiologist. This review will illustrate atypical imaging/clinical manifestations and compli

MeCP2 duplication causes hyperandrogenism by upregulating LHCGR and downregulating RORα.

Duplication of MECP2 (methyl-CpG-binding protein 2) gene causes a serious neurological and developmental disorder called MECP2 duplication syndrome (MDS), which is usually found in males. A previous clinical study reported that MDS patient has precocious puberty with hyperandrogenism, suggesting increased MeCP2 may cause male hyperandrogenism. Here we use an MDS mouse model and confirm that MECP2 duplication significantly upregulates androgen levels. We show for the first time that MeCP2 is highly expressed in the Leydig cells of testis, where androgen is synthesized. Mechanistically, MECP2 du
