Delayed skin reactivity of patients with herpetic stromal keratitis.

Twenty-seven patients with chronic stromal keratitis due to herpes simplex virus were tested for delayed skin reactions, to investigate the non-specific immune reactivity in vivo. Skin tests were performed with five ordinary antigens-Candida, mumps, phytohemagglutinin-P (PHA-P), purified protein derivative of tubercle bacillus (PPD), and streptokinase-streptodornase (SK-SD). The reactivity of the patients was generally decreased, as compared to the age-matched 40 healthy controls. Especially, the reactivity of the patients to PHA-P was significantly decreased (p less than 0.007). The largest m

Partial reversal of the cellular immune defect in common variable immunodeficiency with indomethacin.

We administered indomethacin (25 mg q.i.d.) to two patients with common variable immunodeficiency and measured the effect on their depressed cellular immune responses. The in vitro response of both patients' lymphocytes to phytohemagglutinin increased during indomethacin administration. Both patients were anergic to skin testing before treatment and responded to two of four skin tests during treatment with indomethacin. Thus indomethacin administration in vivo appears to enhance the cellular immune respone, paralleling its previously described effects in vitro.

Failure of isoniazid prophylaxis after exposure to isoniazid-resistant tuberculosis.

We report the failure of isoniazid chemoprophylaxis to prevent the development of active pulmonary tuberculosis and tuberculin conversion in contacts of patients infected with isoniazid-resistant strains of Mycobacterium tuberculosis. Although failure of isoniazid chemoprophylaxis has been reported rarely in contracts of isoniazid-resistant tuberculosis, the cases reported here as well as previous observations by others suggest that isoniazid chemoprophylaxis for contracts of patients with isoniazid-resistant tuberculosis may not be effective and that other agents must be considered. Combined use of Quantiferon and HBHA-based IGRA supports tuberculosis diagnosis and therapy management in children.

OBJECTIVES: Interferon-γ release assays (IGRA) are designed for diagnosis of tuberculosis (TB) infection, and do not discriminate latent TB infection (LTBI) from active TB. Heparin-binding hemagglutinin antigen (HBHA) emerged as a promising antigen for TB diagnosis when used in IGRA format. Aim of this study was to prospectively evaluate the performance of an HBHA-based IGRA to support TB diagnosis and TB therapy monitoring in children with TB infection or active TB disease. METHODS: Following clinical, microbiological and radiological assessment, children (0-14 years old) were tested by the

Interferon gamma release assays based on M. tuberculosis-specific antigens in sarcoidosis patients.

INTRODUCTION: This study is a part of the project on interferon gamma release assays performed in the group of untreated sarcoidosis patients formerly BCG vaccinated. The aim of the study was to assess the rate of positive commercial interferon g release assays in sarcoidosis patients. We discussed the results in the context of hypothesis that M. tuberculosis antigens may play a role in the pathogenesis of sarcoidosis. MATERIAL AND METHODS: 151 patients, mean age 38 ± 10.3, treatment naive, with newly diagnosed pulmonary sarcoidosis were enrolled into the study. All participants underwent QFT

Confirming the diagnosis of tuberculosis in children in Northern Portugal.

Childhood tuberculosis (TB) is difficult to diagnose and there are no standardised case definitions. TB records of patients aged >5 years reported in Northern Portugal between 2000 and 2009 were reviewed. Of the 116 children diagnosed with TB, 72 (62.1%) were male; the mean age was 2.2 ± 1.2 years. Of the 32 children (27.6%) with confirmed TB, only 46.7% had provided samples for culture, showing that little effort is made to obtain specimens for confirmation. We could not identify independent factors associated with the low rate of sample collection or anti-tuberculosis treatment without confi

Efficacy of inducible protein 10 as a biomarker for the diagnosis of tuberculosis.

OBJECTIVE: This study evaluated inducible protein 10 (IP-10) as a diagnostic biomarker for specific tuberculosis (TB) infection and evaluated the ability of IP-10 to distinguish between active TB and latent TB infection (LTBI). METHODS: Forty-six patients with active pulmonary TB, 22 participants with LTBI, and 32 non-TB controls were enrolled separately. We measured IP-10 in serum and in supernatants from whole blood stimulated with TB-specific antigens. RESULTS: TB antigen-dependent IP-10 secretion was significantly increased in the active TB patients and LTBI subjects compared with contro

The mode of presentation of sarcoidosis in Finland and Hokkaido, Japan. A comparative analysis of 571 Finnish and 686 Japanese patients.

We compared the clinical picture of sarcoidosis in patients diagnosed at Mjölbolsta hospital in Finland in 1955-1987 with those diagnosed in Sapporo in 1964-1988. The female:male ratios showed a slight female predominance. The mean age (SD) at diagnosis was 41.5 (13.0) years at Mjölbolsta and 30.0 (15.4) years in Sapporo. In both series, half of the patients had been detected at mass x-ray surveys. The presenting symptoms varied considerably among the symptomatic patients. At Mjölbolsta hospital, 189 patients (33%) had cough, 21% fever, 21% general malaise, 18% dyspnoea, 18% erythema nodosum,

Optimizing tuberculosis infection screening strategies for people living with HIV in Thailand: a cost-effectiveness analysis.

BACKGROUND: To support evidence-based decision-making in Thailand, we assessed the cost-effectiveness of QuantiFERON-TB Gold Plus (QFT-Plus) compared to the Tuberculin Skin Test (TST) in people living with HIV (PLWH). The outcomes of this investigation are intended to provide valuable guidance for policymakers in Thailand and other low- and middle-income countries (LMICs) to choose the most appropriate screening tests to detect tuberculosis infection (TBI). METHODS: We evaluated QFT-Plus versus the TST using a decision-tree–Markov model under a healthcare payer perspective, expressed in 2022 U

Tuberculosis Infection in Early Childhood and the Association with HIV-exposure in HIV-uninfected Children in Rural Uganda.

BACKGROUND: In high tuberculosis (TB) burden countries, a significant proportion of the latent TB reservoir is established by the age 5 years. There are critical knowledge gaps in our understanding of the age-specific prevalence of TB infection and the influence of HIV exposure on TB infection in the first 5 years of life among HIV-uninfected children in sub-Saharan Africa. METHODS: We measured TB infection with the Quantiferon Gold-in-Tube (QFT) and tuberculin skin tests (TST) in 447 children ≤60 months and their 284 HIV-infected and IV-uninfected mothers in rural Uganda. RESULTS: The overa
