Diagnostic accuracy of optic nerve OCT and ultrasound in a large paediatric cohort referred with suspected papilloedema and very low rates of raised ICP.

BACKGROUND/OBJECTIVES: The Honey Rose trial increased referrals for suspected papilloedema. We audited such referrals from community optometrists and used long-term patient outcomes to investigate the diagnostic accuracy of OCT retinal nerve fibre layer (RNFL) thickness and B scan ultrasound optic nerve sheath diameter (ONSD) for raised intracranial pressure (rICP). SUBJECTS/METHODS: Audit of 337 children seen in a 'blurry disc' clinic. Long-term outcomes (6-42 months) were used to classify children as with or without raised ICP. Receiver operating curves investigated diagnostic accuracy of R

How to deal with the superior petrosal vein in microvascular decompression for trigeminal neuralgia?

OBJECTIVE: How to manage the superior petrosal vein (SPV) obstructing the operative field during microvascular decompression (MVD) for trigeminal neuralgia (TN) remains controversial. The authors aimed to evaluate the safety profile of a specific SPV division technique used during MVD for TN. METHODS: This retrospective analysis included patients who underwent first-time MVD for medically refractory TN from 2005 to 2025 at a single center. When the SPV obstructed the operative field, it was coagulated at its main trunk near the entry site into the superior petrosal sinus, maintaining venous c

Neglected tracts of the brainstem: transverse peduncular tract of Gudden and taenia pontis.

OBJECTIVE: The anatomy and function of the brainstem have fascinated scientists for centuries; however, the brainstem remains one of the least studied regions of the human brain. As the authors delved into studying this structure, they observed a growing tendency to forget or neglect previously identified structures. The aim of this study was to describe two such structures: the transverse peduncular tract, also known as the Gudden tract, and the taenia pontis. The authors analyzed the potential effects of neglecting these structures during brainstem surgery and the implications for clinical p

Leptomeningeal metastases in prostate cancer: A review of the current literature.

Leptomeningeal metastasis/leptomeningeal carcinomatosis (LMC; terms used interchangeably) is an inflammatory complication of primary tumors that involves the spread of the disease to the meninges (specifically the arachnoid and pia maters) and spinal cord. In the United States, approximately 110,000 new cases are diagnosed each year, and the prognosis is usually poor. Complications of LMC include cognitive impairment, cranial nerve dysfunction, ischemic stroke, and mortality. The survival times of untreated and treated LMC are approximately 4-6 weeks and 2-4 months, respectively. Leptomeningea

Revisiting the Efficacy of Redo Microvascular Decompression for Trigeminal Neuralgia.

BACKGROUND: Microvascular decompression (MVD) is an effective nondestructive neurosurgical procedure for trigeminal neuralgia (TN). However, some patients may undergo surgery failure or experience pain recurrence, sparking debates on the need for reoperation. METHODS: We conducted a retrospective analysis of 103 cases of patients with primary TN who underwent redo MVD at our center between January 2020 and December 2022. Comparative prognostic assessments were performed by comparing these cases against a cohort of 348 patients who underwent primary MVD during the same study period. RESULTS:

Tinnitus: an underreported condition following microvascular decompression for hemifacial spasm.

PURPOSE: While hearing loss is a well-known condition following microvascular decompression (MVD) for hemifacial spasm (HFS), tinnitus is an underreported one. This study aims to identify prevalence, characteristics, severity, and predictors of tinnitus following MVD for HFS. METHODS: A single-center cohort of 55 HFS patients completed a questionnaire approximately 5 years following MVD. Data encompassed tinnitus presence, side, type, onset, and severity measured by a 10-point Visual Analogue Scale (VAS). Descriptive, correlation, and logistic regression analyses were conducted. RESULTS  : At

A rare coexistence of a non-dysraphic intradural lipoma and a cyst in the lumbar spine - A case report and literature review.

INTRODUCTION: Spinal intradural lipomas are rare entities, particularly when not associated with spinal dysraphism. Their diagnosis is often delayed due to their slow growth and presentation in dorsal components. Although the association between lipomas and cysts has been previously noted, the coexistence of lipomas with fluid-filled cysts is exceedingly rare. We present a case that highlights the presentation, management, and outcomes of a patient with the unusual coexistence of these two lesions in the lumbar region. CASE REPORT: An eight-year-old female presented to our outpatient neurolog

Histo-anatomical mutilations of developing chick brain induced by in-ovo fluoride and bifenthrin exposure.

Comparative developing brain histo-anatomical pathologies of Fluoride ions and Bifenthrin in-ovo exposures were explored in the golden black variety of domestic chick. Three exposure groups were -the Vehicle control group (Vg); Fluoride (F) group and the Bifenthrin (Bn) group each with forty fertilized eggs and received their respective group treatment at zero day of incubation. Embryos were extracted, dissected from head region and the embryonic whole brains were recovered after 14 days of incubation. The embryonic brains were preserved in bouin fixative for 24 h for further studies. The morp

Pleural Effusions on MRI in Autosomal Dominant Polycystic Kidney Disease.

Autosomal dominant polycystic kidney disease (ADPKD) has cystic fluid accumulations in the kidneys, liver, pancreas, arachnoid spaces as well as non-cystic fluid accumulations including pericardial effusions, dural ectasia and free fluid in the male pelvis. Here, we investigate the possible association of ADPKD with pleural effusion. ADPKD subjects (n = 268) and age-gender matched controls without ADPKD (n = 268) undergoing body magnetic resonance imaging from mid-thorax down into the pelvis were independently evaluated for pleural effusion by 3 blinded expert observers. Subjects with conditio

Case report: Interdisciplinary treatment of complex C1/C2 fractures in a patient with concomitant three-vessel coronary artery disease requiring bypass surgery.

BACKGROUND: Acute myocardial infarction (MI) frequently leads to consciousness disturbance following hemodynamic collapse. Therefore, MI can occur together with upper cervical spine trauma. Herein, we report the successful treatment of complex C1/C2 fractures in a patient with concomitant three-vessel coronary artery disease (CAD). CASE PRESENTATION: A 70-year-old patient presented in our emergency outpatient clinic after a hemodynamic collapse without neurological deficits or heart-related complaints. Computed tomography (CT) scan of the cervical spine revealed a dislocated odontoid fracture

MR Brain Screening in ADPKD Patients : To Screen or not to Screen?

BACKGROUND: Adult polycystic kidney disease (ADPKD) still represents a major cause of renal failure and intracranial aneurisms (IA) have a higher prevalence in ADPKD than in the general population. Current guidelines suggest performing brain MRI only in the subjects with a positive familiar history of IAs or subarachnoid hemorrhage (SAH). This is a retrospective case-control analysis to evaluate the usefulness of a MR screening program in ADPKD patients. METHODS: We retrospectively analyzed all ADPKD patients followed in our outpatient clinic between 2016 and 2019 who underwent a brain MRI sc

Does pituitary compression/empty sella syndrome contribute to MRI-negative Cushing's disease? A single-institution experience.

OBJECTIVE: Cushing's disease arises from functioning adrenocorticotropic hormone (ACTH)-secreting pituitary adenomas. These tumors can be very small and evade detection by MRI. Empty sella syndrome is a phenomenon by which an arachnoid outpouching of CSF into the sella leads to compression of the pituitary, likely due to intracranial hypertension (a common issue in Cushing's disease), further leading to difficulty in visualizing the pituitary gland that may contribute to difficulty in finding a tumor on MRI, so-called MRI-negative Cushing's disease. The authors sought to examine the associatio

Trigeminal neuralgia and hemifacial spasm associated with vertebrobasilar artery tortuosity.

BACKGROUND: The tortuous vertebrobasilar artery (TVBA) often causes neurovascular conflicts in patients with trigeminal neuralgia (TN) and hemifacial spasm (HFS). Implementation of microvascular decompression (MVD) in these circumstances is hindered due to stiffness of the enlarged and dilated arteries and is often accompanied by poor outcomes. The surgical strategy in cases of trigeminal neuralgia and hemifacial spasm associated with the TVBA should be clarified in order to achieve good outcomes. MATERIAL AND METHODS: MVD was performed in 268 TN patients and 71 HFS patients. The TVBA as a co
