NMR-based serum metabolomics of dementia patients and search for metabolic biomarkers associated with cognitive decline.

Cognitive decline (CD) and dementia are major age-associated neurodegenerative conditions lacking minimally invasive biomarkers that reflect underlying disease biology. Systemic metabolic alterations closely mirror brain bioenergetic dysfunction and may provide accessible indicators of disease progression. This study applied quantitative 1H NMR-based serum metabolomics to identify metabolic signatures associated with cognitive decline and to evaluate their diagnostic relevance. The serum samples from individuals with cognitive decline (CD, n = 40) and cognitively normal controls (NCD, n = 54)

The association between breastfeeding and prevalence of metabolic syndrome in women with a previous major pregnancy complication.

INTRODUCTION: Maternal complications of pregnancy, including preeclampsia, gestational diabetes mellitus, spontaneous preterm birth and placental abruption are individually associated with an increased risk of premature heart disease. Breastfeeding may reduce early postpartum cardiometabolic risk in this group. This study aimed the association between breastfeeding and the prevalence of cardiovascular risk factors and metabolic syndrome (MetS) in the early postpartum period amongst women with previous pregnancy complication. METHODS: We conducted a cross-sectional analysis of 524 women who at

Metabolic and inflammatory roles of glial cells in neurodegenerative and cerebrovascular diseases.

Chronic neuroinflammation is a hallmark of neurodegenerative and cerebrovascular diseases and is largely driven by dysfunctional activation of microglia and astrocytes. Recent advances in single-cell transcriptomics and metabolic profiling have revealed the remarkable heterogeneity and plasticity of these glial cells, highlighting their dual roles in neuroprotection and neurotoxicity. Upon activation, microglia adopt pro-inflammatory phenotypes and undergo metabolic reprogramming characterized by a shift from oxidative phosphorylation to glycolysis, resulting in the release of cytokines and re

CIRBP mediates hypoxia-induced mitochondrial metabolic reprogramming in microglia to regulate polarization and anxiety-like behavior.

Exposure to high-altitude hypoxia can lead to anxiety-like behaviors, social issues, and other dysfunctions of the central nervous system (CNS), but the molecular mechanisms behind these effects are not fully understood. Microglial M1 polarization and changes in mitochondrial metabolism are crucial in hypoxic brain injury. The cold-inducible RNA-binding protein (CIRBP) is known to regulate mitochondrial balance and inflammatory responses. However, its role in hypoxia-induced microglial metabolic changes, polarization issues, and anxiety-like behaviors is still unclear. This study established a

A hypothesis on the aetiology of adenomyosis: is there an association between cumulative number of menstruations and severity of adenomyosis on ultrasound?

RATIONALE: Adenomyosis is a common uterine disorder, yet its aetiology remains debated. The tissue injury and repair (TIAR) hypothesis suggests that repeated microtrauma from menstrual contractions damages the endometrial-myometrial interface, thereby contributing to development and progression of adenomyosis. We investigated whether cumulative menstrual exposure is associated with severity of adenomyosis on ultrasound. METHODS: This exploratory retrospective study was conducted at Amsterdam University Medical Centre, as a sub-analysis of the prospective UteroVue cohort study. For the UteroVu

Subacute ruminal acidosis induces colonic inflammation and barrier damage via cGAS-STING pathway in Hu sheep fed a high-grain diet.

High-grain diets widely applied in intensive ruminant farming frequently induce subacute ruminal acidosis, accompanied by intestinal inflammation and barrier damage, while the underlying molecular mechanisms remain unclear. Given the central role of the cGAS-STING cascade in inflammatory injury, this work explored its function in high-grain-induced colonic damage using lactating Hu sheep. In vivo, 12 healthy sheep were randomly divided into two equal groups, fed with either a low-grain (grain:forage=3:7, LG) or high-grain (grain:forage=7:3, HG) diet for 8 weeks, followed by comprehensive detec

Practical evaluation and management of symptomatic benign pleural effusions.

PURPOSE OF REVIEW: Benign pleural effusions comprise a heterogeneous group of disorders, but recent advances have been uneven across different etiologies. This review summarizes the developments most likely to influence current clinical practice, focusing on heart failure-related pleural effusions, hepatic hydrothorax, and postcoronary artery bypass graft (CABG) pleural effusions. RECENT FINDINGS: Heart failure-related pleural effusions have experienced the greatest advances recently. A blood-free pleural fluid triple combination (protein, lactate dehydrogenase, and cholesterol) provides diag

Integrated molecular and clinical profiling of primary mitochondrial oxidative phosphorylation disorders in an Indian cohort: Insights from genetics, neuroimaging, and machine learning.

Primary mitochondrial disorders are clinically and genetically heterogeneous and remain underdiagnosed in resource-limited settings. We performed a retrospective observational study (March 2016- January 2024) at a tertiary neurology center in Eastern India to characterize the clinical, biochemical, neuroimaging, electrophysiological, and molecular features of suspected mitochondrial disease and to explore interpretable machine-learning approaches for syndromic stratification. Forty-eight patients from 42 unrelated families were classified as mitochondrial encephalomyopathy, lactic acidosis, an

Compound Heterozygous Sickle Cell-Beta Thalassemia Presenting As Chronic Hemolytic Anemia With Microcytosis and Prominent Left Ventricular Trabeculation: A Case Report.

Sickle cell-beta thalassemia, a compound heterozygous hemoglobinopathy, can be difficult to diagnose because clinical features and laboratory findings often overlap with other hematological disorders. We report a 26-year-old man from Assam who presented with eight days of progressive jaundice. Further evaluation revealed chronic hemolysis that had gone undiagnosed for years. He also reported one week of abdominal distension and generalized weakness. Clinical examination revealed pallor, icterus, and moderate splenomegaly. Laboratory evaluation demonstrated anemia (hemoglobin 6.3-7.9 g/dL), mic

Transcriptional profiling of the hippocampus and hypothalamus reveals a potential role of thyroid hormone in gamma-aminobutyric acid (GABA) synthesis in dairy cows fed a high-concentrate diet.

Subacute rumen acidosis (SARA) is a common metabolic disorder in dairy cows fed high-concentrate (HC) diets in intensive farming systems. No typical clinical sign of illness but some common signs include decreased dry matter intake, laminitis and diarrhea are observed in HC cows indicating a certain stressful and depressive behavior. However, transcriptional change in energy metabolic center of the hippocampus and hypothalamus in HC cows has not been reported. Twelve mid-lactation dairy cows were randomly assigned to a HC group (60% concentrate, n = 6) or a low-concentrate (LC) group (40% conc

Transcriptional imprints of prenatal stress: sex- and time-specific signatures in early neurodevelopment.

Prenatal stress (PNS) is a well-established animal model of foetal exposure to maternal stress and related psychiatric disorders. While its biological and behavioural effects on the offspring have been widely studied in adolescence and adulthood, early-life signatures remain largely unexplored. Here, we assessed whole-genome transcriptomic changes by RNA-sequencing, followed by pathway analyses, in hippocampi collected at postnatal day (PND)2 and PND14 from male and female rats exposed to PNS by physical maternal restraint, compared with non-stressed counterparts (n = 6 per group). We first an

Genetic Dissection of Energy Deficiency in Autism Spectrum Disorder.

Background/Objectives: An important new consideration when studying autism spectrum disorder (ASD) is the bioenergetic mechanisms underlying the relatively recent rapid evolutionary expansion of the human brain, which pose fundamental risks for mitochondrial dysfunction and calcium signaling abnormalities and their potential role in ASD, as recently highlighted by insights from the BTBR mouse model of ASD. The rapid brain expansion taking place as Homo sapiens evolved, particularly in the parietal lobe, led to increased energy demands, making the brain vulnerable to such metabolic disruptions

Thrombotic Thrombocytopenic Purpura Triggered by Chronic Lymphocytic Leukemia: A Case Report.

INTRODUCTION: Chronic lymphocytic leukemia (CLL) is an incurable lymphoproliferative disorder characterized by the accumulation of mature malignant B cells in the blood, bone marrow, and secondary lymphoid tissues. While it has a heterogenous clinical course, individuals with CLL are at increased risk for autoimmune complications, infections, and secondary non-hematologic secondary malignancies. Peripheral autoimmune cytopenias are a well-known phenomenon in CLL. However, other autoimmune complications, including immune thrombotic thrombocytopenic purpura (TTP), are rare and less investigated.

Factors Contributing to the Efficacy of Fecal Microbiota Transplantation for Diarrhea-Dominant Functional Bowel Disorders.

INTRODUCTION: In cases of effective fecal microbiota transplantation (FMT) for irritable bowel syndrome (IBS), donor feces have been observed to be enriched in Bifidobacterium spp. Moreover, FMT for functional bowel disease can improve psychiatric symptoms. Although intestinal dysbiosis has received attention as one of the pathophysiologies of IBS, the efficacy of FMT for IBS has not yet been established. In this study, we performed a post hoc analysis of the efficacy of FMT, focusing on metabolites in donor feces. METHODS: FMT was performed in 12 patients, 8 with refractory diarrhea-predomin
