Risks and benefits for patients with relapsed or refractory diffuse large B-cell lymphoma in early-phase clinical trials: a systematic review and meta-analysis.

BACKGROUND: The treatment landscape for relapsed or refractory diffuse large B-cell lymphoma has changed profoundly with the introduction of novel drug classes, some approved solely on the basis of single-arm early-phase trials. We aimed to evaluate antitumour activity and safety outcomes across drug classes in early-phase trials in relapsed or refractory diffuse large B-cell lymphoma since 2000. METHODS: We did a systematic review and meta-analysis of phase 1-2 trials. We searched PubMed, Embase.com, Web of Science and Wiley/Cochrane Library from database inception to May 9, 2025. We include

A Deep Learning Approach to Predict Conductive Hearing Loss in Patients With Otitis Media With Effusion Using Otoscopic Images.

IMPORTANCE: Otitis media with effusion (OME) is one of the most common causes of acquired conductive hearing loss (CHL). Persistent hearing loss is associated with poor childhood speech and language development and other adverse consequence. However, to obtain accurate and reliable hearing thresholds largely requires a high degree of cooperation from the patients. OBJECTIVE: To predict CHL from otoscopic images using deep learning (DL) techniques and a logistic regression model based on tympanic membrane features. DESIGN, SETTING, AND PARTICIPANTS: A retrospective diagnostic/prognostic study

Association between hospital-level factors and early rehabilitation initiation in patients with acute ischemic stroke: a multilevel analysis using a Japanese administrative database from a rural prefecture.

BACKGROUND: Early rehabilitation after acute ischemic stroke improves functional outcomes and is recommended by clinical guidelines. However, initiation of early rehabilitation varies substantially across hospitals, and the contribution of hospital-level factors remains unclear. OBJECTIVE: To examine the association between hospital-level factors and early rehabilitation initiation, adjusting for patient-level characteristics. METHODS: We conducted a multicenter cross-sectional study using the Japanese Diagnosis Procedure Combination database from 43 hospitals in Yamagata Prefecture between

Semantic and Phonological Brain Networks in Older Adults: A Systematic Scoping Review.

Background/Objectives: Despite many neurobiological models of semantics and phonology, there is no consensus regarding their functional organization. Specifically, there is considerable debate concerning the precise functional roles of the left inferior frontal gyrus and inferior parietal lobe, where these two networks may overlap. Meta-analyses addressing this topic have focused on neurologically intact young adults, yet theories of cognitive aging suggest older adults may exhibit a different distribution of their language networks. This scoping review specifically explores the interplay betw

Mortality Burden for Patients With Untreated Aortic Regurgitation.

BACKGROUND: Aortic valve replacement (AVR) is indicated in patients with severe aortic regurgitation (AR); however, certain clinical factors may identify patients with less-than-severe AR at high mortality risk if untreated. OBJECTIVES: The authors sought to characterize key associations with mortality across the spectrum of AR in patients not treated with AVR from a large, contemporary database. METHODS: We analyzed patients >18 years of age with documented AR assessment in a deidentified real-world data set from 27 U.S. institutions with appropriate permissions (egnite Database, egnite, In

Validating the German version of the Personality Disorder Severity-ICD-11 Scale using nominal response models.

The International Classification of Diseases (ICD-11) features a new classification of personality disorders (PD), focusing on the severity of PD. Although there are numerous self-report measures that assess PD severity, to date only the Personality Disorder Severity-ICD-11 (PDS-ICD-11) is based on ICD-11's operationalization of PD. Initial results indicated that the PDS-ICD-11 measures a unidimensional construct, but the assumptions made for scoring its bipolar items had not been fully examined. The aim of this study is to fill this gap and investigate the latent structure of the German versi

Acute abdomen in pregnancy: Are tocolytics and early surgical intervention justified?

INTRODUCTION: Twitter and media coverage on poliomyelitis help maintain global support for its eradication. OBJECTIVE: To test our hypothesis that themes of polio-related tweets and media articles would differ by location of interest (hashtag of country name mentioned in the tweet; country name mentioned in media articles) but would be similar to each other (tweets and media articles) for each location of interest. METHODS: We retrospectively examined a 40% random sample of Twitter data containing the hashtag #polio from January 1, 2014, to April 30, 2015 (N = 79,333), from which we extracte

Redundancy-as-masking: formalizing the Artificial Age Score (AAS) to model memory aging in generative AI.

INTRODUCTION: Artificial intelligence can exhibit aging-like patterns not as a function of chronological time, but through systematic asymmetries in output-level observable memory performance under different context-persistence conditions. In large language models, semantic cues, such as the name of the day, may remain stable across sessions, whereas episodic details, such as the sequential progression of experiment numbers, may collapse when conversational context is reset. To capture this phenomenon, this study introduces the Artificial Age Score (AAS), a log-scaled, entropy-informed metric …

An auditable pipeline for fuzzy full-text screening in systematic reviews: integrating contrastive semantic highlighting and LLM judgment.

Full-text screening is the major bottleneck of systematic reviews (SRs), particularly in domains such as population health modelling of noncommunicable diseases (NCDs), where decisive eligibility information is scattered across long and heterogeneous full texts. In this methodological study, we introduce a scalable and auditable pipeline that reframes inclusion and exclusion as a fuzzy decision process. We evaluate the approach within the Population Health Modelling Consensus Reporting Network (POPCORN) and benchmark it against statistical and crisp baselines. Articles are parsed into overlapp

A Conversational Agent for Providing Personalized Preexposure Prophylaxis (PrEP) Support: Protocol for Chatbot Implementation and Evaluation.

BACKGROUND: Chatbots have the potential to reduce barriers to preexposure prophylaxis (PrEP), including lack of awareness, misconceptions, and stigma, by providing anonymous and continuous support. However, in the context of PrEP, chatbots are still nascent; they lack personalized informational expertise, peer experiential expertise, and human-like emotional support to facilitate future PrEP uptake. These personalized and relatable forms of support are crucial for increasing engagement, influencing health decisions, and fostering resilience and well-being. OBJECTIVE: In this paper, we describ

A multimodal multi-agent LLM framework for identifying key drivers of sleep disorders.

INTRODUCTION: Sleep quality and sleep disorders are influenced by interacting lifestyle, behavioral, physiological, and occupational determinants, but most existing studies examine these factors in isolation. Traditional statistical methods may be limited in modeling complex interactions, while many machine-learning approaches remain insufficiently interpretable for clinically meaningful sleep research. METHODS: We developed an interpretable large language model (LLM)-based multi-agent multimodal framework for sleep disorder analysis. The framework includes three specialized agents: a Data An

Temperature-induced destabilization mechanisms of whipping cream: emulsion properties, aeration behavior, and flavor deterioration.

This study elucidated the destabilization mechanism of whipping cream under different storage conditions (4 °C, 25 °C, and temperature fluctuation) over three months, while exploring the potential of large language model (LLM) for shelf-life prediction. Multiple analyses were conducted, including emulsion properties, rheological characteristics, interfacial microstructure, thermodynamic behavior, aeration performance, and volatile flavor profiles. Results showed that with prolonged storage, all samples exhibited progressive emulsion thickening, creaming, and weaker resistance to deformation, a

Who, What, and Where: Composite-Semantics Instance Search for Story Videos.

Who, What and Where (3W)are the three core elements of storytelling, and accurately identifying the 3W semantics is critical to understanding the story in a video. This paper studies the 3W composite-semantics video Instance Search (INS) problem, which aims to find video shots about a specific person doing a concrete action in a particular location. The popular Complete-Decomposition (CD) methods divide a composite-semantics query into multiple single-semantics queries, which are likely to yield inaccurate or incomplete retrieval results due to neglecting important semantic correlations.
