Optimal fluoroscopic viewing angles of left-sided heart structures in patients with aortic stenosis and mitral regurgitation based on multislice computed tomography.

BACKGROUND: Transcatheter interventions are currently undertaken using "generic" fluoroscopic viewing angles. However, the position and orientation of heart structures may vary across patients and disease-specific remodeling processes. OBJECTIVE: This study uses multislice computed tomography to determine optimal fluoroscopic viewing angles of the aortic and mitral annuli, the left atrial appendage and the atrial septum. We explored differences between patients with severe aortic stenosis (AS) and severe mitral regurgitation (MR). METHODS AND RESULTS: The multislice computed tomographies of

Prognostic value of heart valve calcifications for cardiovascular events in a lung cancer screening population.

To assess the prognostic value of aortic valve and mitral valve/annulus calcifications for cardiovascular events in heavily smoking men without a history of cardiovascular disease. Heavily smoking men without a cardiovascular disease history who underwent non-contrast-enhanced low-radiation-dose chest CT for lung cancer screening were included. Non-imaging predictors (age, smoking status and pack-years) were collected and imaging-predictors (calcium volume of the coronary arteries, aorta, aortic valve and mitral valve/annulus) were obtained. The outcome was the occurrence of cardiovascular eve

Frontal sinus volume predicts incidence of brain contusion in patients with head trauma.

BACKGROUND: The function of the paranasal sinuses is a topic of debate. One hypothesis suggested has been that sinuses act as a "crumple zone," serving to protect the contents of the head from impact. In this study, we examine the interactions of the frontal sinus and the brain in the setting of head trauma. Our formal hypothesis is that frontal sinus volume is less in head trauma patients with contusion than in head trauma patients without contusion. METHODS: Computed tomographic (CT) scans of patients who sustained blunt head trauma performed at the University of Pittsburgh Medical Center f

Association of atherosclerosis in the descending thoracic aorta with coronary artery disease on multi detector row computed tomography coronary angiography in patients with suspected coronary artery disease.

The association between atherosclerosis in the descending thoracic aorta (DTA) visualized on computed tomography coronary angiography (CTA) and coronary artery disease (CAD) has not been extensively explored. Therefore, a comprehensive analysis of DTA atherosclerosis on CTA was performed and the association of DTA atherosclerosis with CAD was evaluated in patients with suspected CAD. A total of 344 patients (54 ± 12 years, 54% men) with suspected CAD underwent CTA. CTA were classified based on CAD severity in no signs of atherosclerosis or minor wall-irregularities <30%, non-significant CAD 30

Successful transfemoral aortic valve replacement in a bicuspid aortic stenotic valve: requirements for a safe implant.

Although transcatheter aortic valve replacement (TAVR) has been accepted as an attractive alternative for high-risk patients with severe symptomatic aortic stenosis (AS), patients with congenital bicuspid AS has been typically disqualified for this indication due to an implied risk of device dislocation, distortion, or device malfunctioning. Nonetheless, bicuspid AS is not uncommon and frequently missed by transthoracic echocardiography. We reported an interesting case of a high-risk patient with severe symptomatic bicuspid AS who underwent successful TAVR and discussed the anatomic requiremen

Late complications and pathological findings of the arterial switch operation.

We describe aortic root dilatation, severe aortic regurgitation, and pulmonary artery stenosis that were accidentally diagnosed 23 years after the arterial switch operation for transposition of the great arteries in situs inversus. We successfully performed the modified Bentall procedure and pulmonary artery reconstruction. The pathology of the dilated aortic root revealed intimal atherosclerosis and linear necrosis of the tunica media, suggesting the vulnerability of the pulmonary artery to systemic pressure.

Factors associated with blunt cerebrovascular injury in patients with cervical spine injury.

Blunt cerebrovascular injury (BCVI) is known to be a potentially fatal complication of cervical spine injury (CSI). Methods for screening the appropriate population remain to be elucidated, especially in Japan. This retrospective study was conducted to predict the risk factors relevant to BCVIs. Among 92 patients with CSI transferred to our institution from April 2007 to March 2012, 40 patients (35 men, 5 women) with neurological deficits and/or significant cervical spine fracture including fracture of transversarium, facet, body, lamina, and spinous process, underwent multi-detector computed

Quadricuspid aortic valve with mild aortic regurgitation and persistent foramen ovale: a multimodality imaging of rare concomitant findings.

Quadricuspid aortic valve is a rare congenital heart disease that is mostly detected incidentally during echocardiography or angiography, or at autopsy. This pathology is frequently associated with other congenital anomalies of the heart such as abnormalities of coronary ostia, coronary stenosis, aortic root dilation and coarctation of the aorta, as well as significant aortic valve dysfunction. Herein is presented the first documented case of an asymptomatic middle-aged male patient with incidentally diagnosed quadricuspid aortic valve and a preserved valve function associated with a patent fo

Aortic translocation using the hemi-mustard procedure for corrected transposition.

The management of congenitally corrected transposition of the great arteries and associated lesions is frequently challenging. Restrictive ventricular septal defect and mild pulmonary stenosis are contraindications to the double switch procedure, including the atrial-Rastelli switch procedure, due to the production of postoperative left ventricular outflow tract obstruction. We describe a case of aortic translocation using the hemi-Mustard procedure after left ventricular training in order to prevent postoperative left ventricular outflow obstruction.

Left Ventricular Outflow Tract Geometry as a Predictor of Permanent Pacemaker Implantation After Transcatheter Aortic Valve Implantation Using the Evolut Series.

BACKGROUND: Predicting the need for permanent pacemaker implantation (PPM) after transcatheter aortic valve implantation (TAVI) is crucial for procedural planning and patient counseling. AIMS: To evaluate whether left ventricular outflow tract (LVOT) morphology, specifically a tapered configuration, predicts conduction disturbances requiring PPM after TAVI using the Evolut self-expanding valve. METHODS: This retrospective multicenter study included patients undergoing TAVI with the self-expanding Evolut series. Pre-TAVI MSCT analysis included LVOT shape, membranous septum (MSep) length, and

The role of chest pain symptoms in predicting coronary artery disease severity and MACE events in patients with zero CAC: a study from a large CCTA cohort.

Coronary artery calcium (CAC) is a well-established predictor of major adverse cardiovascular events (MACE) and is considered a key marker in the assessment of coronary artery disease (CAD). While its prognostic value has been extensively validated in asymptomatic populations, its utility in symptomatic patients, particularly those with CAD, remains underutilized. To examine the relationship between chest pain symptoms, CAD severity, and the risk of MACE in patients with a zero CAC score. A cohort of 10,908 patients with a zero CAC score underwent coronary computed tomography angiography (CCTA

Patient-Specific Computer Simulation to Elucidate the Role of Contact Pressure in the Development of New Conduction Abnormalities After Catheter-Based Implantation of a Self-Expanding Aortic Valve.

BACKGROUND: The extent to which pressure generated by the valve on the aortic root plays a role in the genesis of conduction abnormalities after transcatheter aortic valve replacement (TAVR) is unknown. This study elucidates the role of contact pressure and contact pressure area in the development of conduction abnormalities after TAVR using patient-specific computer simulations. METHODS AND RESULTS: Finite-element computer simulations were performed to simulate TAVR of 112 patients who had undergone TAVR with the self-expanding CoreValve/Evolut R valve. Computed tomography-based fat and muscle characteristics are associated with mortality after transcatheter aortic valve replacement.

BACKGROUND: Computed tomography (CT)-based fat and muscle measures are associated with outcome in large populations. We tested if muscle and fat characteristics are associated with long-term outcomes after TAVR. METHODS: We included 403 clinical CTs performed prior to TAVR at our center between 2008 and 2016, measuring area (cm2) and density (Hounsfield units, HU) of both psoas muscles (PM), subcutaneous adipose (SAT), and visceral adipose tissue (VAT). Area measures were indexed to height, log-transformed and both area and density were standardized for analysis. We assessed the association o
