INTRODUCTION: Nasal dermal sinus in children (NDSC) is a rare malformation (1/20,000 to 1/40,000). Apart from local infection, they present as median nasal lump or pit on the dorsum and their diagnosis and treatment are often delayed. Consequences of untreated NDSC are: local infection, meningitis, and empyema, due to their frequent intracranial extension.
PATIENTS AND METHODS: Six cases of NDSC were retrospectively reviewed (5 boys and one girl), all treated between 2006 and 2012 in our institution (Nancy University Hospital). All patients underwent a brain and facial CT-scan and MR imaging

BACKGROUND: Extensive data exist regarding the success rates and long-term outcomes of transsphenoidal adenomectomy (TSA) of growth hormone (GH)-secreting pituitary tumors; however, few data exist regarding the extent of adenomectomy.
OBJECTIVE: To evaluate surgical outcomes for the treatment of GH-secreting pituitary adenomas with regard to the extent of adenomectomy.
METHODS: A retrospective study of 282 patients with GH-secreting pituitary tumors who underwent TSA. Three surgical paradigms (1, 2, and 3) were applied, all of which differed in extent of adenomectomy. All participants were e

BACKGROUND: The management of hydrocephalus associated with a posterior fossa tumor is debated. Some authors emphasize the advantages of an immediate tumor removal that may normalize the cerebrospinal fluid (CSF) dynamics. However, in clinical practice, the mere excision of the lesion has been demonstrated to be accompanied by a persisting hydrocephalus in about one third of the cases. Preoperative endoscopic third ventriculostomy (ETV) offers several advantages. It may control the intracranial pressure (ICP), avoid the necessity of an emergency procedure, allow appropriate scheduling of the o

BACKGROUND: Traumatic brain injury represents a significant cause of mortality and permanent disability in the adult population. Posttraumatic CSF rhinorrhea is one of the conditions most strongly associated with severe brain injury. Knowledge on the natural history of the illness and the outcomes of patients with transcranial subfrontal approach for posttraumatic CSF rhinorrhea approach may help the neurosurgeon in the decision-making process. This study was conducted to analyse the outcome of trans-cranial sub-frontal approach for traumatic CSF rhinorrhea, with duroplasty and fibrin glue.

INTRODUCTION: We studied whether the choice of timing of discussing organ donation for the first time with the relatives of a patient with catastrophic brain injury in The Netherlands has changed over time and explored its possible consequences. Second, we investigated how thorough the process of brain death determination was over time by studying the number of medical specialists involved. And we studied the possible influence of the Donor Register on the consent rate.
METHODS: We performed a retrospective chart review of all effectuated brain dead organ donors between 1987 and 2009 in one D

Post Thoracotomy Pain Syndrome (PTPS) is defined as pain that occurs or persists in the area of the thoracotomy incision for at least 2 months following the initial procedure. The true incidence of PTPS is hard to define as literature reports a wide range of occurrence from 5% to 90%. Thoracotomy is associated with a high risk of severe chronic postoperative pain. Presenting symptoms include both neuropathic pain in the area of the incision, as well as myofascial pain commonly in the ipsilateral scapula and shoulder. Pain management can be challenging in these patients. Multiple treatment

STUDY DESIGN: Resident's case problem.
BACKGROUND: Cauda equina syndrome (CES) is a rare, potentially devastating, disorder and is considered a true neurologic emergency. CES often has a rapid clinical progression, making timely recognition and immediate surgical referral essential.
DIAGNOSIS: A 32-year-old male presented to a medical aid station in Iraq with a history of 4 weeks of insidious onset and recent worsening of low back, left buttock, and posterior left thigh pain. He denied symptoms distal to the knee, paresthesias, saddle anesthesia, or bowel and bladder function changes. At the

Since many years we routinely use diagnostic selective nerve root blocks (SNRB) at our department when evaluating patients with cervical radiculopathy. Frequently patients who also presented with headache reported that the headache disappeared when the nerve root responsible for the radicular pain was blocked with local anaesthetics. Headache has been described as a companioning symptom related to cervical radiculopathy but has never before been evaluated with SNRB performed in the lower cervical spine. For this reason we added to our routine an evaluation of the response from the SNRB on head

OBJECTIVE: Clinical and microscopic evidence suggests the existence of sensory neuropathy in patients with severe chronic venous insufficiency (CVI). A clinical evaluation was conducted to determine whether a sensory neuropathy was present and, if so, to determine its extent and distribution.
METHODS: The study was performed in a university-affiliated Veterans Affairs Medical Center. Twenty-three limbs were studied in 14 male veterans with mild or moderate CVI. The exclusions included diabetes, previous ipsilateral extremity surgery, or other diseases associated with neuropathy. Sensory thres

The diagnosis and treatment of posterior plagiocephaly is one of the most controversial aspects of craniofacial surgery. The features of true lambdoid synostosis versus those of deformational plagiocephaly secondary to positional molding are inadequately described in the literature and poorly understood. This has resulted in many infants in several craniofacial centers across the United States undergoing major intracranial procedures for non-synostotic plagiocephaly. The purpose of this study was to describe the detailed clinical, imaging, and operative features of true lambdoid synostosis and

Chronic intractable benign pain (CIBP) is defined as non-neoplastic pain of greater than 6 months duration without objective physical findings and known nociceptive peripheral input. To test the CIBP concept, 283 consecutive chronic pain patients were examined independently by a neurosurgeon and physiatrist and only congruent physical findings were coded. Because they did not fit the CIBP definition, patients with the following primary treatment diagnoses were eliminated: degenerative disease of the spine and spinal stenosis; degenerative disease of the hips; radiculopathy; malignancy; deaffer

Neurosurgery is a surgical specialty that routinely deals with life-threatening pathology that has a narrow therapeutic window and a margin of error. These clinical realities, with most emergencies being medico-legal cases, often expose neurosurgical practice to medico-legal scrutiny. Adequate literature is available on medicolegal aspects of neurosurgery from other countries such as North America, Australia, and Europe. However, literature on medicolegal issues of neurosurgery in India is relatively sparse and fragmented. Here, we attempt to review the medico-legal aspects of neurosurgery in

OBJECTIVE: Magnetic resonance imaging (MRI) is integral to acute cerebrovascular care but creates safety challenges beyond metal screening. We characterized MRI-related safety incidents and safety-confirmation breakdown points in a national Japanese incident-reporting database.
METHODS: We conducted a retrospective cross-sectional descriptive analysis of 54,310 adverse-event reports from 2013 to 2024. Candidates were identified by co-occurrence of MRI-related and cerebrovascular terms. Eligible reports described incidents during preparation, transfer, scanning, monitoring, or return transfer

BACKGROUND: Transforaminal Endoscopic Lumbar Discectomy (TELD) is increasing in popularity as a minimally invasive technique for treating lumbar disc herniation (LDH). However, TELD presents technical challenges that may result in a flat learning curve. This study analysed the first patients treated by a single senior neurosurgeon transitioning from tubular microdiscectomy, focussing on the initial learning curve for operative time, recurrence, and complications.
METHODS: A retrospective study was conducted using data from a consecutive cohort of the first 213 patients operated for LDH by TEL

OBJECTIVE: Extradural spinal nerve sheath tumors with foraminal involvement are rare, and the gold standard for surgical resection is not established yet. Traditionally, lumbar intra-extraforaminal schwannomas have been resected through a posterior midline or paraspinal open approach. The lateral transpsoas approach represents a suitable, minimally invasive approach which can provide direct access to the extraforaminal and lumbar plexus region avoiding bone removal, hence the necessity of lumbar fusion. The aim of this study is to share our experience of extraforaminal lumbar schwannomas appro

A 65-year-old woman was first diagnosed in 2011 with stage III clear cell renal cell carcinoma (pT3aNxM0) of the left kidney, managed successfully with radical nephrectomy. She remained disease-free for three years. In 2014, she developed a metachronous stage III papillary thyroid carcinoma (T1N1M0), treated with total thyroidectomy, nodal dissection, and adjuvant radioactive iodine therapy (100 mCi). In 2019, two pulmonary lesions were detected in the left upper lobe. Biopsy confirmed metastatic clear cell carcinoma. Due to intolerance to sunitinib, the patient was treated with pazopanib. By
