Unclassified Mixed Germ Cell-Sex Cord-Stromal Tumor of the Ovary: An Unusual Case Report.

Unclassified mixed germ cell-sex cord-stromal tumor (UMGC-SCST) is a rare ovarian neoplasm composed of germ cells and sex cord elements, which occurs in genetically and phenotypically normal women without the usual histological features seen in gonadoblastoma. Few cases have been reported in the literature so far. The age of presentation is more frequent in girls younger than 10 years of age, although it can also occur in adult women. It can be associated with isosexual pseudoprecocity. The preferred management is the resection of the gonad that contains the tumor and the conservation of the o

Metastatic Choriocarcinoma Masquerading as a Congenital Glabellar Hemangioma.

Infantile choriocarcinoma (ICC) is a rare, highly malignant form of gestational trophoblastic neoplasia. Rapid diagnosis and initiation of treatment are paramount in reaching a successful outcome. Patients with these tumors typically present with a triad of anemia, hepatomegaly, and precocious puberty. Cutaneous manifestations of ICC are extraordinarily rare with few documented cases. Here, we describe a male neonate who presented to our Dermatology clinic with a rapidly growing, markedly vascular glabellar mass associated with abnormal laboratory values suggestive of Kasabach-Merritt phenomen

Evaluating the potential utility of kisspeptin to treat reproductive disorders.

The kisspeptins, encoded by the KISS1 gene, are a group of newly discovered peptides, which have been found to play an important regulatory role in human reproduction. Loss of function mutations of kisspeptin or the kisspeptin receptor have been shown to cause pubertal failure; whereas activating mutations cause precocious puberty. Central and peripheral administration of kisspeptin to mammals stimulates gonadotrophin secretion via gonadotrophin releasing hormone (GnRH) stimulation. Similar observations have been reported in human studies as well as an increase in luteinizing hormone (LH) puls

Hypothalamic pituitary complications in Kabuki syndrome.

Kabuki syndrome is characterized by distinctive facial features, multiple anomalies and mental retardation. In this syndrome, structural CNS abnormalities are commonly observed, but congenital abnormalities in the pituitary gland or hypothalamus have rarely been reported. We searched the published medical literature on the complications in hypothalamic pituitary axis in this syndrome. As a result, only nine patients with Kabuki syndrome had been reported to have complications in hypothalamic pituitary axis in previous papers. Among the nine reported patients and one presented case in this repo

Huge bilateral ovarian cysts in adulthood as the presenting feature of Van Wyk Grumbach syndrome due to chronic uncontrolled juvenile hypothyroidism.

Juvenile primary hypothyroidism causing cystic ovaries and pseudoprecocious puberty (Van-Wyk Grumbach syndrome (VWGS)) is well documented in literature. There are only a few reports of primary hypothyroidism presenting as ovarian cysts in adults. Here we present a case of huge bilateral ovarian cysts in adulthood as the presenting feature of VWGS due to chronic uncontrolled juvenile hypothyroidism. Large uniloculor right ovarian cyst (119 × 81 × 90 mm) and a multicystic left ovary (55 × 45 × 49 mm) were detected in a 24 year lady with secondary amenorrhea, galactorrhea, and palpable abdominal

[Atypical pituitary tumors].

The significant progress made in recent years, especially in the field of imaging procedures and immunocytochemistry, has caused great improvement in the identification and differentiation of less known pituitary tumours. Thyrotropin (TSH)-secreting adenomas constitute about 1-2% of all pituitary tumours and their presence leads to central hyperthyroidism. This condition is characterised by a diffuse goiter, elevated circulating levels of free thyroid hormones and a nonsuppressed serum TSH. Gonadotropin-secreting tumours in rare cases may result in ovarian hyperstimulation, testicular enlargem

G protein-coupled receptors involved in GnRH regulation: molecular insights from human disease.

In the past two decades, an increasing body of evidence has demonstrated that several G protein-coupled receptor (GPCR)-ligand pairs are critical for normal human reproductive development and function. Patients harboring genetic insults in either the receptors or their cognate ligands have presented with reproductive disorders characterized by varying degrees of GnRH deficiency. These disorders include idiopathic hypogonadotropic hypogonadism (IHH) and Kallmann Syndrome (KS). Conversely, mutations in some of these ligand-receptor pairs have been associated with accelerated reproductive maturat

Gamma knife radiosurgery for refractory epilepsy caused by hypothalamic hamartomas.

BACKGROUND: Hypothalamic hamartomas are associated with precocious puberty and chronic epilepsy characterized by gelastic seizures. The seizure disorder is usually refractory to most antiepileptic drugs. Gamma knife surgery has emerged as an alternative to microsurgical removal or radiofrequency ablation to improve seizure control. We present our experience with radiosurgery in 4 patients afflicted by this disorder. METHODS: Using gamma knife radiosurgery, 4 patients with intractable gelastic seizures and complex epilepsy were managed. Patient age varied from 5 to 29 years. The duration of sy

First report on the efficacy of l-alanine-based in situ-forming implants for the long-term parenteral delivery of drugs.

The recent advent of biotechnologies has led to the development of labile macromolecular therapeutic agents that require complex formulations for their efficient administration. This work reports a novel concept for the systemic, sustained delivery of such agents. The proposed approach is based on the spontaneous self-assembly of low-molecular weight amphiphilic amino acid derivatives in a hydrophobic pharmaceutical vehicle. The injectable, in situ-forming organogels were obtained by mixing N-stearoyl l-alanine (m)ethyl esters with a vegetable oil and a biocompatible hydrophilic solvent. The g

Rett syndrome at an institution for the developmentally disabled.

Rett syndrome (RS) is a condition apparently limited to females characterized by normal early development followed by the abrupt loss of acquired function and beginning autistic behavior in late infancy. Manifestations in RS include decelerating head growth, unusual "hand-writing" movements, gait apraxia, neuromuscular tone disturbance, and seizures. No biochemical, hematologic, cytologic, or cytogenetic procedures have been shown to confirm the diagnosis of RS. With the exception of one family with 2 affected half-sisters, all cases have been sporadic. Six profoundly retarded residents of Cen

Hepatoblastoma producing both alpha-fetoprotein and human chorionic gonadotropin. Clinicopathologic analysis of four cases and a review of the literature.

A clinicopathologic study was done of four cases of hepatoblastoma with precocious puberty, together with an analysis of 21 such cases reported in the literature. All four patients were males, the age ranging from 11 to 35 months. Three of the four patients died within 12 months after operation, but the fourth is living. Histologically, all patients had a hepatoblastoma with the coexistence of both fetal and embryonal type cells, although predominantly fetal in two and predominantly embryonal in the other two. Tumor giant cells were rarely encountered in all these cases. Both alpha-fetoprotein

Sex Cord-Stromal Tumours in Children: A Case Series.

Sex cord-stromal tumours (SCSTs) in children are rare gonadal neoplasms exhibiting differentiation towards sex cord stromal elements. This case series explores paediatric SCSTs, presenting clinical, gross, histologic, and immunohistochemical findings, with a focus on limited information available about these tumours in African children. This retrospective study includes 9 cases of ovarian and testicular SCSTs (8 female and 1 male) in patients below 16 years of age. Female patients presented with various SCST subtypes, including juvenile granulosa cell tumours (JGCTs), fibroma, and mixed tumour

Long-acting vs monthly GnRH agonists: A comprehensive review of efficacy, safety, and health-system implications.

PURPOSE: Gonadotropin-releasing hormone (GnRH) analogs are widely used in the treatment of hormone-dependent conditions such as prostate cancer, breast cancer, and central precocious puberty. These agents suppress sex hormone secretion by persistently downregulating the hypothalamic-pituitary-gonadal axis. To improve convenience and adherence, various long-acting injectable formulations, administered monthly, quarterly, or every 6 months, have been developed. However, there is ongoing debate regarding the relative advantages and optimal choice among these formulations. SUMMARY: Evidence from

Chronic Exposure to 6PPD-Q Induces HPO Axis Dysfunction and Metabolic Disturbances in Female Offspring Mice.

N-(1,3-dimethylbutyl)-N'-phenyl-p-phenylenediamine-quinone (6PPD-Q) is an emerging environmental pollutant with limited research on its reproductive toxicity. In this study, female offspring mice were exposed to 6PPD-Q during gestation and lactation. The results revealed that 6PPD-Q exposure disrupted the hypothalamus-pituitary-ovary (HPO) axis in postnatal day (PND) 22 female offspring mice, leading to precocious puberty and estrous cycle irregularities, accompanied by substantial metabolic alterations. At PND 71, disturbances in the HPO axis and metabolic functions were observed. In both in

Bone Age Assessment Using Various Medical Imaging Techniques Enhanced by Artificial Intelligence.

Bone age (BA) reflects skeletal maturity and is crucial in clinical and forensic contexts, particularly for growth assessment, adult height prediction, and managing conditions like short stature and precocious puberty, often using X-ray, MRI, CT, or ultrasound imaging. Traditional BA assessment methods, including the Greulich-Pyle and Tanner-Whitehouse techniques, compare morphological changes to reference atlases. Despite their effectiveness, factors like genetics and environment complicate evaluations, emphasizing the need for new methods that account for comprehensive variations in skeletal

Differential life cycle toxic effects and molecular mechanisms of Di(2-ethylhexyl) phthalate (DEHP) exposure on the female reproductive system.

Di(2-ethylhexyl) phthalate (DEHP), the most prevalent plasticizer worldwide, can enter the human body via various exposure pathways, including ingestion, inhalation, skin contact, and medical applications. It has been observed to exhibit characteristic patterns of accumulation in the female reproductive system. Research has demonstrated that DEHP poses a threat to reproductive health in females across various life stages. Exposure during childhood has been demonstrated to induce central precocious puberty (CPP). Exposure during reproductive age has been shown to be closely associated with redu

Isolated Inhibin-Producing Granulosa Cell Tumor Causing Primary Amenorrhea: A Case Report.

BACKGROUND: Juvenile granulosa cell tumors (JGCTs) are the most common pediatric ovarian sex cord-stromal tumor. Because most granulosa cell tumors are hormonally active, patients commonly present with signs and symptoms of endocrine dysfunction, including precocious puberty and menstrual irregularities. Most patients with JGCTs present with stage I disease confined to the ovary that can be managed with surgical excision alone with a favorable prognosis. CASE: A 15-year-old girl presented with primary amenorrhea. Evaluation revealed suppressed follicle-stimulating hormone, elevated luteinizin

Puberty Starts in the Gut: Intestinal Clues to Early Puberty-Rethinking Biomarkers in Pediatric Endocrinology.

Central precocious puberty (CPP) may be influenced by gut microbiota through changes in short-chain fatty acids (SCFAs), β-glucuronidase activity, and enterohepatic estrogen recycling. This narrative review integrates current evidence from human and animal studies exploring microbial contributions to pubertal timing. Across multiple cohorts, CPP is associated with loss of SCFA-producing commensals, such as Bacteroides, and increased abundance of taxa like Alistipes, Ruminococcus, and Lachnoclostridium. These microbial shifts are linked to altered SCFA profiles, diminished anti-inflammatory and

Pediatric Adrenocortical Carcinoma Presenting With Virilization: A Case of a Low-Grade Tumor in a Young Child.

Adrenocortical carcinoma (ACC) is an uncommon and severe malignancy, particularly in the pediatric population. It often manifests with warning signs of hormonal imbalance, such as hirsutism and precocious puberty. Timely diagnosis is crucial for effective management. We present the case of a 4-year-old female who presented with indications of virilization, including excessive hirsutism, acne, clitoromegaly, and a moon-shaped facial appearance, which raised suspicion of Cushing's disease. An abdominal ultrasound and computed tomography angiography (CTA) were performed, identifying a left suprar
