Suture anchor repair of quadriceps tendon rupture after total knee arthroplasty.

Disruption of the extensor mechanism after total knee arthroplasty (TKA) is a devastating complication, usually requiring surgical repair. Although suture anchor fixation is well described for repair of the ruptured native knee quadriceps tendon, no study has discussed the use of suture anchors in quadriceps repair after TKA. We present an illustrative case of successful suture anchor fixation of the quadriceps mechanism after TKA. The procedure has been performed in a total of 3 patients. A surgical technique and brief review of the literature follows. Suture anchor fixation of the quadriceps

Relevance of botulinum toxin injection and nerve block of rectus femoris to kinematic and functional parameters of stiff knee gait in hemiplegic adults.

Stiff knee gait (SKG) is common in hemiplegic patients. The main focus of treatment is rectus femoris (RF) spasticity. The aims of this study were to evaluate the effect of botulinum toxin injection (BTI) in the RF muscle on peak knee flexion during swing phase and its quantitative and functional impact on gait. We also wished to evaluate the correlation between the effects of nerve block and BTI on peak knee flexion. 10 adult hemiplegic subjects (>6 months post stroke or traumatic brain injury) with SKG and inappropriate RF EMG activity during mid-swing phase were included. 3D gait analysis,

Electromyographic patterns suggest changes in motor unit physiology associated with early osteoarthritis of the knee.

OBJECTIVE: To assess characteristics of active motor units (MUs) during volitional vastus medialis (VM) activation in adults with symptomatic knee osteoarthritis (OA) across the spectrum of radiographic severity and age-comparable healthy control volunteers. METHODS: We evaluated 39 participants (age 65+/-3 years) in whom weight-bearing knee X-rays were assigned a Kellgren & Lawrence (KL) grade (18 with KL grade=0; four each with KL grades=1, 2 and 4; nine with grade 3). Electromyography (EMG) signals were simultaneously acquired using surface [surface EMG (S-EMG)] and intramuscular needle el

Split-set resistance training in-season increases maximal vertical power without enhancing athletic performance: a comparison with failure training in youth handball players.

BACKGROUND: We tested the hypothesis that splitting resistance training sets would increase lifting velocity and lead to superior athletic performance adaptations, without compromising strength or muscle hypertrophy, compared to volume-matched sets performed to failure in youth handball players. METHODS: Thirty resistance-trained youth handball players (8 females, 22 males; 18±2 years) completed a seven-week lower-body program performed either as all sets to failure (FAILURE) or volume-matched with repetitions split into double the number of sets (SPLIT). Pre- and post-intervention testing in

Testosterone Therapy and Tendon Injury: Clinical Evidence, Biological Mechanisms, and Sports Medicine Implications.

Testosterone therapy increases lean mass and can improve muscle strength in men with hypogonadism. This review examines the relationship between testosterone therapy, tendon adaptation, and tendon injury, including the possibility of a mismatch between changes in muscle strength and tendon capacity. This narrative review critically evaluates the clinical and experimental evidence relating testosterone exposure to tendon injury. The present review focuses specifically on tendon-related safety and is not intended to assess the overall benefit-risk profile of testosterone therapy. Prescription-ba

Effects of plyometric vs. strength training on strength, sprint, and functional performance in soccer players: a randomized controlled trial.

Plyometric training (PT) has been shown to have numerous benefits and few harmful effects. This study aimed to compare the effects of PT vs. strength training on muscle strength, sprint, and lower limb functional performance in soccer players. Ninety participants (mean age 22.5 years) were equally and randomly divided into three groups: a plyometric training group (PTG; n = 30), a strength training group (STG; n = 30), and a control group (CG; n = 30). In PTG: bounding, hurdling, and drop jumping exercises were performed. In STG: isometric exercises of knee extension (at 60º knee flexion), qua

Supervised, Heavy Resistance Training Is Tolerated and Potentially Beneficial in Women with Knee Pain and Knee Joint Hypermobility: A Case Series.

INTRODUCTION: Adults with generalised joint hypermobility including knee joint hypermobility (GJHk) report more knee joint symptoms when compared to adults without GJHk. There is no consensus on best practice for symptom management. For instance, controversy exists regarding the appropriateness and safety of heavy resistance training as an intervention for this specific group. This case series aims to describe a supervised, progressive heavy resistance training program in adults with GJHk and knee pain, the tolerability of the intervention, and the outcomes of knee pain, knee-related quality o

Iron status, catabolic/anabolic balance, and skeletal muscle performance in men with heart failure with reduced ejection fraction.

BACKGROUND: Metabolic derangements related to tissue energetics constitute an important pathophysiological feature of heart failure. We investigated whether iron deficiency and catabolic/anabolic imbalance contribute to decreased skeletal muscle performance in men with heart failure with reduced ejection fraction (HFrEF), and whether these pathologies are related to each other. METHODS: We comprehensively examined 23 men with stable HFrEF (median age [interquartile range]: 63 [59-66] years; left ventricular ejection fraction: 28 [25-35]%; New York Heart Association class I/II/III: 17/43/39%).

Whole-Body Muscle MRI in Non-5q Spinal Muscular Atrophy: Patterns, Genotype Prediction, and Diagnostic Implications.

BACKGROUND AND OBJECTIVES: Spinal muscular atrophy (SMA) is a group of genetically heterogeneous motor neuron disorders characterized by progressive, predominantly proximal weakness. Non-5q SMA refers to forms occurring without SMN1 pathogenic variants or deletions. More than half of non-5q SMA cases remain genetically unresolved, and muscle MRI may reveal characteristic patterns supporting diagnosis. METHODS: We retrospectively analyzed multicenter data from adults presenting with chronic, progressive, motor-predominant weakness, beginning proximally in the upper or lower limbs, with exclusi

Functional deficits after anterior cruciate ligament injury are associated with altered neuromuscular control: a cross-sectional study.

OBJECTIVES: Persistent muscle weakness after anterior cruciate ligament (ACL) injury may coexist with altered antagonist coactivation, but how these neuromuscular deficits differ according to injury chronicity and relate to patient-reported knee function remains incompletely characterized. This study aimed to compare knee muscle strength and antagonist coactivation among acute and chronic ACL injury groups, and healthy controls, and to evaluate the associations among muscle strength, antagonist coactivation, and Knee injury and Osteoarthritis Outcome Score (KOOS) subscale scores. METHODS: Thi
