The mind and the body are clearly intertwined in ways that are only now being discovered. In the orthopaedic world, injuries and diseases are often classified and described in a very organized, discrete fashion-The radius is fractured, the ACL or meniscus or rotator cuff is torn, the ankle is sprained, and/or the lumbar spine has a disc herniation. Although it is, in many ways, almost comforting to think about injuries or orthopaedic issues in this manner, what about the many patients who fail to fall into this classification? What about the thousands of patients with severe unexplained chroni

This narrative review synthesizes current evidence on extracorporeal shock wave therapy for pain management in musculoskeletal rehabilitation, emphasizing its analgesic mechanisms, clinical applications, and safety. As a noninvasive physical therapy, extracorporeal shock wave therapy is increasingly used for pain relief in conditions such as chronic low back pain, plantar fasciopathy, knee osteoarthritis, rotator cuff injury, and myofascial pain syndrome. Its analgesic effects arise through multiple pathways, including peripheral neuroregulation characterized by degeneration and subsequent reg

The developmental course of musculoskeletal deformities in neonatal brachial plexus palsy (NBPP) has not been studied extensively. The goals of this study were to: (1) evaluate a new animal model of NBPP, (2) characterize the development of musculoskeletal abnormalities in paralyzed shoulders, and (3) investigate the expression of myogenic and adipogenic genes in paralyzed rotator cuff muscles. Neonatal mice were divided into neurotomy and sham groups. The neurotomy group underwent surgical transection of the superior trunk of the brachial plexus within 24 h of birth. The sham group underwent

Successful treatment of acute fractures of the proximal humerus with prosthetic replacement is a therapeutic challenge to the orthopaedic surgeon, and requires proper elevation of the patient, proper surgical technique, and meticulous rehabilitation. Fractures that require prosthetic replacement as the definitive treatment include 4-part fractures and fracture dislocations, head-split fractures with > 40% articular surface involvement, and selected 3-part fractures. The trauma series of radiographs (including true anteroposterior and lateral views in the scapular plane, and axillary view) is e

Arthrography of the shoulder was performed on 50 patients with acute traumatic shoulder dislocations. The mean interval between injury and arthrogram was 2.3 days, with a median interval of one day. Anterior dislocations occurred in 96% of patients, and posterior dislocations in 4%. Initial dislocations were present in 74%, and recurrent dislocations in 26%. The most frequent abnormality identified or arthrograms was an enlarged but intact shoulder capsule (58%), most commonly seen in shoulders subject to recurrent dislocations (77%). Shoulder joint capsular tears or disruptions were seen in 4

The repetitive overhead use of the arm required by many sports may lead to impingement in the vulnerable avascular region of the supraspinatus and biceps tendons. The impingement syndrome may spill over at any time to involve the adjacent biceps tendon, subacromial bursa, and acromioclavicular joint and, as a continuum, with the passage of time, may eventuate in degeneration and partial, even complete thickness, rotator cuff tears later in life. Physical findings, particularly a positive impingement sign, confirm the diagnosis. In the young competing athlete, therapeutic measures consist of ca

Arthroscopic rotator cuff tendon repair is a common elective procedure performed by trained orthopaedic surgeons with a relatively low complication rate. Specifically, isolated neuropraxia of the anterior interosseous nerve (AIN) is a very rare complication of shoulder arthroscopy. An analysis of peer-reviewed published literature revealed only three articles reporting a total of seven cases that describe this specific complication following standard shoulder arthroscopic procedures. This article reports on three patients diagnosed with AIN neuropraxia following routine shoulder arthroscopy do

Shoulder pain and pathology are common in manual wheelchair (MWC) users with paraplegia, and the biomechanical mechanism of injury is largely unknown. Establishing patterns of MRI characteristics in MWC users would help advance understanding of the mechanical etiology of rotator cuff disease, thus improving the logic for prescribed interventions. The purpose of this study was to report detailed shoulder MRI findings in a sample of 10 MWC users with anterolateral shoulder pain. The imaging assessments were performed using our standardized MRI Assessment of the Shoulder (MAS) guide. The tendon m

Handbikes come in different models and setups, but only limited knowledge is available on the handbike-user interface. The aim of this study was to identify optimal handbike setups, assuming that in such a setup mechanical efficiency is high, while shoulder load is low. Thirteen subjects with a spinal cord injury (paraplegia) performed handcycling with different handbike setups at constant power output: four crank positions (two distances, two heights) and four backrest inclinations. The O2-consumption, kinetics, and kinematics were measured to calculate mechanical efficiency and shoulder load

Histologic sections from 12 en bloc surgical specimens were studied in order to clarify the pathogenesis of bursal-side rotator cuff tears (BSRCTs). The specimens consisted of the bony insertion, the partially torn area, and the musculotendinous junction of the supraspinatus tendon. There were eight men and four women, with an average age of 48.7 years. Trauma was noted in three instances. All patients exhibited clinical evidence of subacromial impingement, which was confirmed at surgery. Histologic sections were stained with azan or hematoxylin and eosin. All the tears developed from within 1

BACKGROUND: Fatty infiltration in skeletal muscle is directly linked to loss of muscle strength and is associated with various adverse physical outcomes such as muscle atrophy, inflammation, insulin resistance, mobility impairments, and even mortality in the elderly. Aging, mechanical unloading, muscle injury, and hormonal imbalance are main causes of muscle fat accumulation, and the fat cells are derived from muscle stem cells via adipogenic differentiation. However, the pathogenesis and molecular mechanisms of fatty infiltration in muscles are still not fully defined. Fatty acid-binding prot

BACKGROUND: Fragment displacement is a key determinant of treatment strategy and functional outcomes in isolated greater tuberosity (GT) fractures. However, the relative contributions of fracture morphology and shoulder dislocation to displacement remain unclear. This study aimed to identify factors associated with fragment displacement, with a particular focus on facet involvement, fracture type, and the presence of shoulder dislocation using three-dimensional computed tomography (CT). Secondary displacement during nonoperative management was also explored.
METHODS: We retrospectively analyz

BACKGROUND: Some surgeons believe that they can identify patients who are at high risk for shoulder redislocation and that these patients are best served by immediate surgical stabilization. This natural history study was performed to examine the validity of this concept and to determine whether it is possible to predict the need for future surgery at the time of the index injury and examination.
METHODS: One hundred and thirty-one patients were followed for an average of four years after their first shoulder dislocation. An extensive history was recorded and a thorough physical examination w

PURPOSE: To report a new technique of arthroscopic biceps tenodesis using bioabsorbable interference screw fixation and the early results.
TYPE OF STUDY: Prospective, nonrandomized study.
TECHNIQUE: The principle of arthroscopic biceps tenodesis is simple: after biceps tenotomy, the tendon is exteriorized and doubled on a suture; the biceps tendon is then pulled into a humeral socket (7 or 8 mm x 25 mm) drilled at the top of the bicipital groove, and fixed using a bioabsorbable interference screw (8 or 9 mm x 25 mm) under arthroscopic control.
PATIENTS: 43 patients treated with this techniq

The shoulder apparatus is of elegant structural design, affording great ROM with substantial power in many planes of movement. It is the underlying anatomic relationships that allow great mobility that also render the shoulder susceptible to injury. Injury in repetitive overhand activities is usually in the form of impingement, which may result from many factors, including multidirectional instability, anterior subluxation, and imbalanced force couple mechanisms, among others. Diagnosis requires a thorough history and physical examination. The impingement sign and test are among the most usefu

BACKGROUND: Margin convergence repair is a technique that embraces the philosophy of "harnessing the ox rather than roping the bull" for the repair of massive rotator cuff tears and has been shown to provide satisfactory functional outcomes. However, previous studies have generally relied on traditional scoring systems, leaving the clinical relevance of the outcomes from the patient's perspective and the effect of patient characteristics on these results largely unknown. Therefore, this study aimed to evaluate the Minimal Clinically Important Difference (MCID), substantial clinical benefit (SC

PurposeWe aimed to find the distinctions between IV and IA TXA concerning intraoperative visual clarity, postoperative pain management during ARCR.MethodsIn our retrospective cohort, propensity score matching study, we included patients who had a history of rotator cuff tears and at least six months of unsuccessful conservative treatment. The primary outcome, visual clarity during surgery, was evaluated using a five-grade arthroscopic visual scale (AVS) by the operating surgeon. This scale ranged from grade 1, indicating optimal clarity, to grade 5, the least clarity necessitating a switch to
