Cerebrospinal fluid (CSF) dynamics in the cervical spinal subarachnoid space (SSS) have been thought to be important to help diagnose and assess craniospinal disorders such as Chiari I malformation (CM). In this study we obtained time-resolved three directional velocity encoded phase-contrast MRI (4D PC MRI) in three healthy volunteers and four CM patients and compared the 4D PC MRI measurements to subject-specific 3D computational fluid dynamics (CFD) simulations. The CFD simulations considered the geometry to be rigid-walled and did not include small anatomical structures such as nerve roots

Thymoma with metastasis to the central nervous system is extremely rare. Herein, three rare cases of thymoma with intracranial metastasis are reported. Case 1 (thymoma, B3 in the WHO classification); cranial magnetic resonance imaging (MRI) revealed a mass lesion with a ring enhancement effect in the right temporal lobe. The cystic and hemorrhagic lesion was subtotally removed and radiation therapy of 40 Gy was administered to the tumor cavity containing the remains of the tumor around the ependyma. The patient received combination chemotherapy with carboplatin and gemcitabine hydrochloride, a

REASONS FOR PERFORMING STUDY: Despite modern medical diagnostic imaging, it is not possible to identify reliably the exact location of spinal cord compression in horses with cervical vertebral stenotic myelopathy (CVSM). Vertebral canal endoscopy has been successfully used in man and a technique for cervical vertebral canal endoscopy (CVCE) has been described in equine cadavers.
OBJECTIVE: To determine the feasibility and safety of CVCE in healthy mature horses.
METHODS: Six healthy mature horses were anaesthetised. A flexible videoendoscope was subsequently introduced via the atlanto-occipi

OBJECT: Traumatic subdural effusion (TSE) is a common sequela of traumatic brain injury. Surgical intervention is suggested only when TSE exerts mass effect. The authors have found that many patients with TSE exerting mass effect have concomitant hydrocephalus. Patient experiencing this occurrence were studied, and the pathogenesis of this phenomenon was discussed in the context of recent advances in the understanding of CSF circulation.
METHODS: During a 2-year period, the authors' institution treated 14 patients with TSE who developed hydrocephalus, after 1 of the patients suffered subdural

Twenty-five children with benign intradural extramedullary tumors/lesions were retrospectively analyzed to see the frequency of different histotypes in this location and their variation from the Western world. The duration of symptoms, clinical profile and surgical outcome of these cases were evaluated. The age of children ranged from 1.5 to 18 years, with a mean age of 7.5 years. Thirteen amongst them were male. Follow-up ranged from 3 months to 5 years with a mean of 19 months. McCormick functional grading was used retrospectively to find out the functional outcome in these children. Sixty-e

OBJECTIVE: During its course between the brainstem and the lateral rectus muscle, the abducens nerve usually travels forward as a single trunk, but it is not uncommon for the nerve to split into two branches. The objective of this study was to establish the incidence and the clinical importance of the duplication of the nerve.
METHODS: The study was performed on 100 sides of 50 autopsy materials. In 10 of 11 cases of duplicated abducens nerve, colored latex was injected into the common carotid arteries and the internal jugular veins. The remaining case was used for histological examination.

Glossopharyngeal neuralgia is an uncommon craniofacial pain syndrome that is occasionally associated with cardiac syncope. However, we relate Chiari I syndrome as a cause of this clinical picture for the first time in the literature. The authors analyze the relevant literature and discuss the pathogenesis and treatment of associated syndromes. We describe the case of a 45-year-old female patient who presented with a 3-year history of left glossopharyngeal neuralgia with occasional dysphagia and episodes of syncope when eating or swallowing. The pain was not disseminated to the right side and w

BACKGROUND: Extracerebral fluid collections in infancy are a common diagnostic problem, because by noninvasive imaging studies (including cranial ultrasonography, CT and NMR), no definite differentiation between two distinct pathological conditions can be found until today: An enlargement of the subarachnoid spaces in children with macrocephaly is a frequent observation of mostly unknown etiology but is known to be associated with a good prognosis. If surgery is necessary in these patients, ventricular shunting is required. On the other hand subdural effusions are often of traumatic origin and

Immunohistochemical methods have been used to describe the distribution of neurophysin I- and oxytocin-stained fibers in the spinal cord. In albino rats of either sex, such fibers descend through the dorsal part of the lateral funiculus to the caudal end of the cord. Fibers leave the lateral funiculus to innervate the marginal zone of the dorsal horn at all levels, and the intermediolateral column at thoracic, lumbar and sacral levels. Some fibers course medially through the intermediate gray to end in the central gray, which is innervated at all levels, and also appears to be innervated by fi

Traditional white matter (WM) dissection techniques, such as the Klingler method, employ wooden or metal instruments that may cause myelin wear and limit fine anatomical separation. Inspired by clinical water-jet applications, we investigated the feasibility of an air-jet dissector (pneumodissection) for WM dissection, leveraging the Coandă effect to gently separate contiguous fiber tracts while preserving their integrity. A portable air compressor was modified with an extended tube and an 18G blunt needle tip. A left cerebral hemisphere, prepared according to a modified Klingler freeze-thaw p

Traumatic brain injury (TBI) is a complex and often-devastating condition. This disease involves damage to cerebral structures: meninges (dura, arachnoid, pia), cerebral cortex, white matter tracts, and deeper structures (basal ganglia, brainstem), along with mechanisms including contusions, hematomas (epidural/subdural), diffuse axonal injury from shear forces, secondary edema compromising blood-brain barrier, and ischemia/hemorrhage caused by vascular disruption. The pathophysiological process of TBI above varies significantly among individuals. However, prevalent TBI treatments still focus

BACKGROUND: Epidermoid cysts are benign, slow-growing, and extra-axial lesions that typically develop between the brain structures. Intradiploic and intra-axial involvement is exceptionally rare. We present a case of an intradiploic epidermoid cyst with distinct clinical and radiological features. This case highlights an unusual location and radiological presentation of an epidermoid cyst located within the skull base and may be misdiagnosed as other pathologies.
CASE DESCRIPTION: A 44-year-old male presented with a dull, aching frontal headache and slowly growing right frontal swelling, wors

Bacterial infection is a key trigger of inflammatory pathways that damage the preterm brain, even without direct bacterial brain invasion. However, the mechanisms underlying the involvement of intracranial tissues and cells in early immune responses to acute systemic inflammation in preterm infants remain unknown. Lipopolysaccharide (LPS) was administered with a single intraperitoneal injection into postnatal day (P) 7 mice. Four hours later, the leptomeningeal macrophages initiated an intracranial reaction by producing IL-1β, which set the stage for the arachnoid, pial, and perivascular fibro

The management of vestibular schwannoma (VS) remains one of the most formidable challenges in neurosurgery owing to the eloquent nature of surrounding anatomy. Although endoscopy-assisted microsurgery has recently gained momentum in cerebellopontine angle region surgery, the feasibility of pure endoscopic technique has been rarely reported. Here we present the operative technique and preliminary outcomes of fully endoscopic retrosigmoid trans-petrosal fissure approach (ER-TPFA) for VS surgery. Clinical data of 36 consecutive cases of VS treated with the ER-TPFA from March 2021 to March 2023 we

Hydrocephalus is characterized by the accumulation of excess cerebrospinal fluid (CSF) in the cranium due to an imbalance between production and absorption of CSF. The standard treatment involves the implantation of a shunt to divert excess CSF into the peritoneal cavity, but these shunts exhibit high failure rates over time. In pursuit of improved reliability and performance, this study proposes a miniaturized valve designed to mimic the natural one-way valve function of the arachnoid granulations and thereby replace the shunts. A benchtop testing setup was employed to characterize the behavi

OBJECTIVE: Microvascular decompression (MVD) is the only curative treatment modality for hemifacial spasm (HFS). Although generally considered to be safe, this surgical procedure is surrounded by many risks and possible complications. The authors present the spectrum of complications that they met in their case series, the possible causes, and the strategies recommended to minimize them.
METHODS: The authors reviewed a prospectively maintained database for MVDs performed from 2005 until 2021 and extracted relevant data including patient demographics, offending vessel(s), operative technique,

BACKGROUND: Cervical spinal injury often disrupts the supraspinal vasomotor pathways projecting to the thoracic sympathetic preganglionic neurons, leading to cardiovascular dysfunction. The current guideline is to maintain the mean arterial blood pressure at 85 to 90 mmHg using a vasopressor during the first week of the injury. Some studies have demonstrated that this treatment might be beneficial to alleviate secondary injury and improve neurological outcomes; however, elevation of blood pressure may exacerbate spinal hemorrhage, extravasation, and edema, exacerbating the initial injury.
