[Unusual location of osteochondral lesions in adolescent's knees].

Osteochondral knee lesions in adolescents are primarily located in loading areas at the level of the femoral condyles. Lesions located in the trochlea are exceptional and account for less than 1%. The etiology of juvenile osteochondritis dissecans (JOCD) is still unknown. Osteochondral lesions may be caused by direct frontal trauma of the femoral condyles and by direct trauma of the patella on the trochlea in dislocations of the latter. At this level both mechanisms may cause both chondral and osteochondral lesions. We present herein two cases with the same characteristics that include one pat

[Effect of an oral anti-estrogen agent (mepitiostane) on the regression of intracranial meningiomas in the elderly].

We report here the regression of meningioma following treatment with the anti-estrogen agent mepitiostane in a series of cases. The first case was that of a 72-year-old woman who presented with coma status due to non-communicating hydrocephalus. A large presumed meningioma within the cerebello-pontine angle was detected on gadolinium-enhanced magnetic resonance imaging (MRI). The patient recovered from the neurological deficit following endoscopic third ventriculostomy treatment, and was administered mepitiostane (10mg/day) orally. Gadolinium-enhanced MRI showed a marked regression (85%) of th

Preservation of cranial nerves during removal of the brain for an enhanced student experience in neuroanatomy classes.

Neuroanatomy teaching at the University of Leeds includes the examination of isolated brains by students working in small groups. This requires the prosected brains to exhibit all 12 pairs of cranial nerves. Traditional methods of removing the brain from the skull involve elevating the frontal lobes and cutting each cranial nerve as the brain is reflected posteriorly. This can leave a substantial length of each nerve attached to the skull base rather than to the removed brain. We have found a posterior approach more successful. In this study, five adult heads were disarticulated at the level o

Prenatal diagnosis of recurrent autosomal dominant osteogenesis imperfecta associated with unaffected parents and paternal gonadal mosaicism.

OBJECTIVE: To present the prenatal diagnosis of recurrent autosomal dominant osteogenesis imperfecta (OI) associated with unaffected parents and paternal gonadal mosaicism. MATERIALS AND METHODS: A 37-year-old woman was referred for genetic counseling at 18 weeks of gestation because of advanced maternal age and a family history of OI. The woman had a daughter who was affected with OI type III and carried an insertion frameshift mutation of c.4308_4309insA in exon 52 of the COL1A1 gene. The woman and her husband were non-consanguineous and healthy. Amniocentesis was performed at 18 weeks of g

[Pontomedullary lacerations and concomitant injuries: a review of possible underlying mechanisms].

Anatomically, brainstem is constituted of medulla oblongata, pons and mesencephalon. Traumatic lesions of brainstem most commonly occur on pontomedullary junction. There are several possible mechanisms of pontomedullary lacerations. The first mechanism includes impact to the chin, with or without a skull base fracture, and most often leads to this fatal injury, due to impact force transmission through the jawbone and temporomandibular joint. The second mechanism includes lateral and posterior head impacts with subsequent hinge fractures, where occurrence of pontomedullary lacerations in these

Finite element method for analysis of stresses arising in the skull after external loading in cranio-orbital fractures.

BACKGROUND AND PURPOSE: The craniofacial skeleton remains not fully recognised as far as its mechanical resistance properties are concerned. Heretofore, the only available information on the mechanism of cranial bone fractures came from clinical observations, since the clinical evaluation in a living individual is practically impossible. It seems crucial to implement computer methods of virtual research into clinical practice. Such methods, which have long been used in the technical sciences, may either confirm or disprove previous observations. The aim of the study was to identify the areas o

Oral and craniofacial findings in a patient with methylmalonic aciduria and homocystinuria: review and a case report.

Combined methylmalonic aciduria and homocystinuria (MMA-HC) is a rare metabolic disease characterized by an inborn defect in B12 vitamin metabolism. This case report concerns an 11-year-old patient with MMA-HC, which developed during the neonatal period. The patient shows some of the facial features that were already reported in the literature (high forehead, large floppy, low-set ears, flat philtrum and hypotonia of perioral and masticatory muscles) but no dolichocephalic skull nor long face. The patient also shows signs that had not been previously described: epicanthal folds, broad nasal br

Gunshot injuries due to celebratory gun shootings.

UNLABELLED: Traditional shooting with guns often occurs and leads to unwanted gunshot injuries in areas where celebrations are held. Such injuries have been classified as celebratory gun shooting injury in the international disease classification system. CASE: An 8-year-old female patient presented with respiratory arrest. The heartbeats normalized upon cardiopulmonary resuscitation. On physical examination, the only pathological finding was a skin defect measuring 1 x 1 cm on the midline and located 2 cm in front of the coronal suture. Cranial CT revealed a bone defect of 0.5 cm in the area

[A case of non-traumatic tension pneumocephalus 12 years after initial ventriculo-peritoneal shunt].

The authors presented a patient with non-traumatic tension pneumocephalus 12 years after initial ventriculo-peritoneal shunt. A 60-year-old man had a past history of subarachnoid hemorrhage. At ÿtime, he underwent clipping surgery and emplacement of a ventriculoperitoneal shunt with a low pressure valve, and was discharged from hospital in a bedridden condition. After 12 years, he was transferred to our hospital because of dysarthria and right hemiparesis. On admission, he had multiple dimples of the scalp in the left frontal area where craniotomy had been carried out 12 years before. Head CT

Epidermoids of the paranasal sinuses and beyond: endoscopic management.

BACKGROUND: Epidermoid tumors of the craniofacial skeleton are uncommon, with the exception of acquired cholesteatoma of the temporal bone. These lesions may be primary embryologic in origin or, less frequently, may be iatrogenic or posttraumatic. METHODS: We report three cases of unusually encountered epidermoids, all of which were managed by endoscopic marsupialization via paranasal sinus approaches. We also review the pathophysiology and clinical presentation of these rare entities. RESULTS: The first patient is a 60-year-old man with a congenital epidermoid of the petrous apex removed vi

A comparison of the craniofacial morphology in 2-month-old unoperated infants with unilateral complete cleft lip and palate, and unilateral incomplete cleft lip.

This paper reports a cephalometric analysis of the craniofacial morphology in infants with unoperated unilateral complete cleft lip and palate (UCCLP) and unoperated unilateral incomplete cleft lip (UICL). The purpose of the study was to determine the nature and extent of the craniofacial deviations in UCCLP as compared to the morphology in UICL, which has previously been shown to be close to normal. The samples comprised 82 infants with UCCLP (58 males and 24 females) and 75 with UICL (48 males and 27 females). The mean age was about 2 months in both groups. The cephalometric analysis of cran

Nonepithelial tumors of the nasal cavity, paranasal sinuses, and nasopharynx. A clinicopathologic study. XIII: Meningiomas.

Twelve cases of meningiomas involving the nasal cavity and paranasal sinuses are reported. Meningiomas only rarely involve the upper respiratory tract; these 12 cases were found among the 566,000 surgical pathology cases that have been studied in this laboratory. In this series, five meningiomas probably arose in the cranial cavity and secondarily extended into the nasal cavity and paranasal sinuses. Because radiographs demonstrated hyperostosis or bone destruction, three other tumors could have involved the cranial cavity, but this was never proven. In two other cases, the meningiomas appeare

[A case of a traumatic anterior cerebral artery aneurysm following the penetration of the skull base by an iron rod].

A 61-year-old male fell from a position 1 m high when building a house. An iron rod, which protruded upward from a solid base in cement, penetrated this patient's neck 15 cm to the head and was successfully extracted by himself. On admission, he complained of headache and vomiting. General examination disclosed nasal bleeding, intraoral bleeding, and L figured skin laceration in the left side of his neck at the level of the thyroid cartilage. Mild disorientation (JCS2) was noted. Otolaryngological examination disclosed hyperemia on the left side of the vocal cord as well as at the dome of the

Comparative Outcomes of Fronto-Orbital Advancement and Strip Craniectomy in the Management of Metopic Craniosynostosis.

OBJECTIVE: Metopic craniosynostosis results in characteristic trigonocephaly and may impair cranial growth. Surgical correction is traditionally performed using fronto-orbital advancement (FOAR), while strip craniectomy with postoperative helmet therapy (SC) has emerged as a minimally invasive alternative. This study compares perioperative outcomes and postoperative cranial shape between FOAR and SC. METHODS: A retrospective cohort study was conducted at a tertiary pediatric craniofacial unit. Patients with isolated metopic craniosynostosis treated with FOAR or SC between 2015 and 2024 were i

Factors influencing blood loss in fronto-orbital advancement and remodeling: A retrospective review of 231 cases.

Fronto-orbital advancement and remodeling (FOAR) is associated with substantial blood loss, making it crucial to understand the factors influencing intraoperative blood loss during this procedure. A total of 231 craniosynostosis patients who underwent FOAR between 2003 and 2023 were retrospectively reviewed. Younger patients (<18 months) demonstrated higher weight-adjusted blood loss. Patients with multiple suture synostosis, syndromic craniosynostosis, elevated intracranial pressure (ICP), and dural tears had significantly higher total and weight-adjusted blood loss. Blood loss showed a moder

Abandoning the Supraorbital Bandeau in Anterior Craniosynostosis Repairs, for a Single-Segment Reconstruction.

BACKGROUND: Following correction of anterior sutural fusions, long-term forehead irregularities may arise. Based on the premise that frontal reconstructions using a seamless construct might produce better long-term aesthetic results, the supraorbital bandeau was abandoned for a single-piece frontal reconstruction. The purpose of this review was to compare outcomes and complications between children who underwent reconstruction with the traditional supraorbital bandeau and those who underwent reconstruction with a single bony segment. METHODS: A retrospective sequential chart review was perfor

Multimaterial 3D printing preoperative planning for frontoethmoidal meningoencephalocele surgery.

INTRODUCTION: Surgical correction of frontoethmoidal meningoencephalocele, although rare, is still challenging to neurosurgeons and plastic reconstructive surgeons. It is fundamental to establish reliable and safe surgical techniques. The twenty-first century has brought great advances in medical technology, and the 3D models can mimic the correct tridimensional anatomical relation of a tissue organ or body part. They allow both tactile and spatial understanding of the lesion and organ involved. The 3D printing technology allows the preparation for specific surgery ahead of time, planning the
