Medical management of osteonecrosis of the hip: a review.

Osteonecrosis or avascular necrosis (AVN) of the hip is a progressive disease mainly affecting adults in their third, fourth or fifth decade of life. Studies into the natural history of the disease suggest that femoral head collapse occurs within 2-3 yrs with associated degenerative changes and at that stage arthroplasty is the most reliable treatment option. Therefore prevention of femoral head collapse is highly desirable in this young patient group. In early stage disease, before femoral head collapse (Ficat and Arlet stage 1-3) core decompression of the femoral head is currently the most w

Femoral cortical stress lesions in long-term bisphosphonate therapy: a herald of impending fracture?

OBJECTIVE: Lateral cortical stress reactions have been documented to precede femoral insufficiency fractures after long-term bisphosphonate therapy. We studied the natural history of femoral stress lesions associated with long-term bisphosphonate therapy. DESIGN AND SETTING: A retrospective clinical and radiologic review of all patients with radiologically documented femoral stress lesions associated with bisphosphonate therapy was carried out in a tertiary center involved with geriatric trauma care. PATIENTS: Of 1463 geriatric hip fractures occurring from May 1, 2004, to July 31, 2008, 33 w

Zoledronic acid protects against osteosarcoma-induced bone destruction but lacks efficacy against pulmonary metastases in a syngeneic rat model.

Osteosarcoma (OS) is the most common primary malignant tumor of bone in children and adolescents. In spite of successful control of the primary tumor, death from lung metastasis occurs in more than a third of patients. To investigate the efficacy of zoledronic acid (ZOL) on the development, progression and metastatic spread of OS, we used a rat model of OS, with features of the disease similar to human patients, including spontaneous metastasis to lungs. Rat OS cells were inoculated into the tibial marrow cavity of syngeneic rats. OS development was associated with osteolysis mixed with new bo

Evidence-based guidelines for the pharmacological treatment of postmenopausal osteoporosis: a consensus document by the Belgian Bone Club.

Several drugs are available for the management of postmenopausal osteoporosis. This may, in daily practice, confuse the clinician. This manuscript offers an evidence-based update of previous treatment guidelines, with a critical assessment of the currently available efficacy data on all new chemical entities which were granted a marketing authorization. Osteoporosis is widely recognized as a major public health concern. The availability of new therapeutic agents makes clinical decision-making in osteoporosis more complex. Nation-specific guidelines are needed to take into consideration the spe

Intravenous bisphosphonate treatment and pregnancy: its effects on mother and infant bone health.

INTRODUCTION: Type 1 Gaucher's disease (GD1) is a lysosomal storage disorder associated with disabling bone involvement. The choice treatment for Gaucher's disease is enzyme replacement therapy (ERT). The use of bisphosphonate treatment for osteopenia and osteoporosis has been suggested. CASE: A 22-year-old woman diagnosed with GD1 had received ERT intermittently, depending on availability of the enzyme since the enzyme was not always available. Due to severe bone involvement and multiple vertebral fractures, intravenous administration of 60 mg of pamidronate every 3 months and safe contracep

Bisphosphonates treatment and orthodontic considerations.

Currently, the use of oral and systemic forms of bisphosphonates is increasing dramatically in a large group of patients either in the form of anti-osteoporosis medications or as a part of a chemotherapeutic regimen for several malignant diseases. As adult orthodontic treatment has become more widely accepted in most orthodontic practices, orthodontists must be aware of the risks, benefits, and effects of bisphosphonates use on the patient's general health status, as well as on their orthodontic treatment outcomes. This review aims to discuss the use of bisphosphonates, the complications assoc

Bisphosphonate-associated osteonecrosis of the jaw, with healing after teriparatide: a review of the literature and a case report.

This paper reports the case history of a patient who had bisphosphonate-associated osteonecrosis of the jaw (ONJ) in which adjunctive treatment with teriparatide was used. The patient was treated for 5 years with alendronate for osteoporosis and developed ONJ after extraction of maxillary teeth. An implant was placed at the site of the extracted teeth. The pathology report confirmed the clinical diagnosis of ONJ; treatment was changed from alendronate to teriparatide and the ONJ resolved. To our knowledge, this is the third case history reported in the literature in which teriparatide was succ

Medical management of metastatic bone disease.

PURPOSE OF REVIEW: To highlight recent advances in the medical management of metastatic bone disease. RECENT FINDINGS: Progress has been made in quality of life assessment and pain measurement; in novel ways to use bisphosphonates, especially in the adjuvant setting for prevention of bone metastases and in combination with other pharmaceutical agents, where synergistic enhancement holds high potential; in targeted therapies using novel agents, especially with denosumab which is approaching registration; in the use of bone biomarkers to direct therapy and as surrogate endpoints; in development

Multicystic bone disease (Gorham-Stout syndrome) in a spider monkey (Ateles geoffroyi ).

BACKGROUND: In April 2000, a 2.5-year-old pet female Geoffroyi's spider monkey presented for reduced activity, a subdued demeanor, and boney enlargement involving both radii. METHODS: On further examination, polyostotic bone cysts were identified involving many of the tubular bones and were identified radiographically. Microscopic examination of a bone biopsy revealed hemorrhage and other characteristics typical of an aneurysmal bone cyst. In addition, excessive osteoclasia was noted, in association with fibrotic areas rather than with Howship's lacunae as expected from a growing animal. RES

Bisphosphonate-modified gold nanoparticles: a useful vehicle to study the treatment of osteonecrosis of the femoral head.

Legg-Calvé-Perthes disease (LCPD) is a juvenile form of osteonecrosis of the femoral head that presents in children aged 2-14 years. To date, there is no effective medical therapy for treating LCPD largely due to an inability to modulate the repair process, including the predominance of bone resorption. This investigation aims to evaluate the feasibility of using gold nanoparticles (GNPs) that are surface modified with a bisphosphonate compound for the treatment of osteonecrosis at the cellular level. Studies have found osteoclast-mediated resorption to be a process that contributes significan

Height loss in elderly women is preceded by osteoporosis and is associated with digestive problems and urinary incontinence.

SUMMARY: This study explores risk factors for height loss and consequences in terms of health and wellbeing, in older women. Osteoporosis, low body-mass index, being born in Europe and using medications for both sleep and anxiety were risk factors for height loss. Height loss was associated with digestive problems; excessive height loss was also associated with urinary stress-incontinence and a decline in self-rated health. INTRODUCTION: Height loss is associated with osteoporosis, but little is known about its consequences. We aimed to examine the risk factors for height loss and the symptom

Renal osteodystrophy and aging.

The bone disease seen in our aging dialysis population is a complex mixture of osteoporosis and renal osteodystrophy. Attention must be paid to both of these issues. Hip fractures are increased with aging and this increase is further aggravated by renal failure. Preventive management with Vitamin D and bisphosphonates is reviewed.

New results from the use of bisphosphonates in cancer patients.

PURPOSE OF REVIEW: Metastatic bone disease results from cancer-bone cell interactions in the bone marrow microenvironment rather than direct destruction by cancer cells. Additionally, cancer treatments may have adverse effects on bone health. Therefore, a clear rationale exists for bone-targeted therapies to prevent skeletal complications and potentially modify the underlying disease. RECENT FINDINGS: Studies in metastatic disease have focused on refining treatment, largely through the use of bone metabolism markers. Use of bisphosphonates in early cancer has become increasingly important to

Skeletal-related events (SREs) in breast cancer patients with bone metastases treated in the nontrial setting.

BACKGROUND: Clinical trials exploring bisphosphonate (BP) use in patients with bone metastases from breast cancer (BC) consistently state skeletal-related event (SRE) rates >50%. These trials predominantly include patients with bone only disease, and also frequently use screening radiology to detect asymptomatic fractures. Both these variables are likely to increase the incidence of SREs and thereby overestimate BP benefit. Little data exists regarding the natural history of SREs among patients treated with i.v. BPs in the nontrial setting. MATERIALS AND METHODS: Charts from two institutions

Incidence of bisphosphonate-associated osteonecrosis of the jaws in breast cancer patients.

BACKGROUND: Bisphosphonate-associated osteonecrosis of the jaws (BP-ONJ) is a relatively new disease. The aim of this study was to evaluate the prevalence of BP-ONJ in breast cancer patients with osseous metastasis and bisphosphonate therapy. METHODS: A retrospective study was conducted in a EUSOMA accredited breast unit in Germany. All patients treated from January of 2000 to March of 2006 with metastatic breast cancer and bisphosphonate therapy were reviewed. All patients were contacted, and missing data were completed through structured interviews with their dentists and physicians (n = 75

The role of inferior alveolar nerve involvement in bisphosphonate-related osteonecrosis of the jaw.

PURPOSE: Hypesthesia or anesthesia of the lower lip (Vincent's symptom) is a common sign in patients with osteomyelitis of the mandible, especially in severe cases. PATIENTS AND METHODS: We observed an involvement of the inferior alveolar nerve in patients with bisphosphonate-related osteonecrosis of the jaw (BRONJ). Surprisingly, we found Vincent's symptom also in patients with limited and early stages of BRONJ. RESULTS: These patients were successfully treated by surgical removal of the necrotic bone combined with preoperative and postoperative administration of antibiotics. We report on t

Higher bone matrix density exists in only a subset of patients with bisphosphonate-related osteonecrosis of the jaw.

PURPOSE: Because few treatment options exist for bisphosphonate-related osteonecrosis of the jaw (BRONJ), developing methods of early detection and prevention is vital. Computed tomography could represent a useful tool for early detection of BRONJ, yet its effectiveness would necessitate changes in tissue density. The aim of this study was to quantify bone matrix density of tissue from patients with BRONJ. PATIENTS AND METHODS: Bone samples from 13 patients with confirmed BRONJ were scanned by use of high-resolution micro-computed tomography. Mean density and density distribution were assesse

Dental extractions and bisphosphonates: the assessment, consent and management, a proposed algorithm.

Bisphosphonate-associated osteonecrosis of the jaws (BONJ) is recognised as a significant complication related to the use of bisphosphonates and currently is gaining importance due to the increasingly widespread use of these medications. Patients are placed into low or high risk groups of developing BONJ depending on the systemic condition for which they have received bisphosphonates. Numerically, the largest group worldwide is patients receiving bisphosphonates for osteoporosis and these generally fall into the low risk group for BONJ. The high risk group, while numerically smaller, is compos
