A case of urinary incontinence by hydroxychloroquine in a geriatric patient.

WHAT IS KNOWN AND OBJECTIVE: Rheumatoid arthritis is an autoimmune disorder characterized by persistent synovitis and systemic inflammation. Genetic factors account for approximately 50% of cases of rheumatoid arthritis and environmental factors include smoking. Urinary incontinence may occur as a medication adverse effect. We present the first report of a case of hydroxychloroquine-induced urinary incontinence in rheumatoid arthritis. DETAILS OF THE CASE: A 71-year-old female with a history of rheumatoid arthritis developed urinary incontinence as an adverse drug reaction to hydroxychloroqui

Polymorphic variation in NFKB1 and other aspirin-related genes and risk of Hodgkin lymphoma.

We found that regular use of aspirin may reduce the risk of Hodgkin lymphoma (HL), a common cancer of adolescents and young adults in the United States. To explore possible biological mechanisms underlying this association, we investigated whether polymorphic variation in genes involved in nuclear factor-kappaB (NF-kappaB) activation and inhibition, other inflammatory pathways, and aspirin metabolism influences HL risk. Twenty single nucleotide polymorphisms (SNP) in seven genes were genotyped in DNA from 473 classical HL cases and 373 controls enrolled between 1997 and 2000 in a population-ba

Prediction of compound signature using high density gene expression profiling.

DNA microarrays, used to measure the gene expression of thousands of genes simultaneously, hold promise for future application in efficient screening of therapeutic drugs. This will be aided by the development and population of a database with gene expression profiles corresponding to biological responses to exposures to known compounds whose toxicological and pathological endpoints are well characterized. Such databases could then be interrogated, using profiles corresponding to biological responses to drugs after developmental or environmental exposures. A positive correlation with an archiv

Inherited thrombocytopenias: from genes to therapy.

BACKGROUND AND OBJECTIVES: Inherited thrombocytopenias are a heterogeneous group of rare diseases characterized by a reduced number of blood platelets. Some of these diseases are exclusive to megakaryocytes and platelets, while in others the pathology extends to other cell types. Although the defective genes, coding for membrane glyoproteins, cytoskeleton components and intracellular signaling pathways, as well as transcription factors, have been identified in most cases, the pathophysiology of these disorders is often unknown. This review describes recent contributions to clinical and diagnos

Reliability between two observers using a protocol for diagnosing essential tremor.

Protocols with demonstrated reliability have been established for the diagnosis of numerous movement disorders. whereas in the essential tremor (ET) literature, there is no discussion about the reliability of diagnostic protocols. Lack of knowledge of the reliability of diagnostic protocols in ET limits the use of these protocols because reliability is an essential requirement for scientific quality in data management. The objective of this study was to determine the reliability of a protocol for diagnosing ET. The protocol consists of a Tremor Interview, a videotaped Tremor Examination, and a

Identification of Glioma Phenotypic Subtypes From Multimodal MRI Data Using Hierarchical Multi-Kernel Learning.

BACKGROUND: Gliomas are the most common primary brain tumors, exhibiting significant phenotypic variability even within the same grade. Identifying glioma subtypes through non-invasive methods could improve patient management. METHODS: In this study, we applied hierarchical multi-kernel learning to identify glioma phenotypic subtypes using MRI data (T1CE and T2FLAIR) from the First Hospital of Shanxi Medical University (FHSXMU) and Shanxi Provincial People's Hospital (SPPH) (n = 246). We further validated our findings using an independent dataset of similar tumor characteristics from The Canc

A Structure-Property Screening Framework for Polymer Shell Encapsulation of Phase-Change Materials: Random Forest and Bayesian Gaussian Process Surrogates with Multi-Objective Optimization of Polymerization Routes.

Confining a phase-change material (PCM) within a polymer shell yields leak-proof, mechanically robust latent-heat storage media, but selecting a shell chemistry and polymerization route requires balancing competing targets: latent-heat storage density (ΔH, the melting enthalpy per unit capsule mass), core loading content (LC), capsule diameter (d), and a melting temperature (Tm) matched to the application. Because the literature characterizes each method-shell-core combination in isolation, these structure-property relationships cannot be compared quantitatively across studies. We present a pr

Leveraging long-read assemblies and machine learning to enhance short-read transposable element detection and genotyping.

Transposable elements (TEs) are parasitic genomic elements that are ubiquitous across the tree of life and play a crucial role in genome evolution. Advances in long-read sequencing have allowed highly accurate TE detection, though at a higher cost than short-read sequencing. Recent studies using long reads have shown that existing short-read TE detection methods perform inadequately when applied to real data. In this study, we use a machine learning approach (called TEforest) to discover and genotype TE insertions and deletions with short-read data by using TEs detected from Drosophila melanog

Deep Unsupervised Domain Adaptation for Translating Cancer Dependency Maps From Cell Lines to Breast Cancer Tumor Genomics.

The Cancer dependency maps (DepMap) identify genetic dependencies in cancer cells using large-scale loss-of-function screens, providing a foundation for cancer-specific treatment strategies. However, discrepancies exist between cancer cell line models (CCLs) and patient-derived tumor models, particularly in translating findings to clinical settings. To bridge this gap, computational approaches such as artificial intelligence-based domain adaptation can assist in aligning laboratory and patient-derived molecular data, thereby improving the translation of preclinical findings into personalized t

Content-based retrieval of fundus images and diabetic retinopathy detection using variants of local texture features.

Content-based retinal image analysis plays a crucial role in the early diagnosis of ocular diseases. In this study, we proposed a novel approach for efficient content-based retinal image retrieval and Diabetic Retinopathy (DR) detection using variants of local texture features derived from Local Binary Pattern (LBP), Local Ternary Pattern (LTP), and Gray Level Co-occurrence Matrix (GLCM). The methodology begins with meticulous image preprocessing to enhance feature extraction, followed by the extraction of LBP, LTP, and GLCM features, which capture intricate texture patterns and enrich the fea

Identifying scoliosis in a population-based adult cohort: automation of a validated method based on total body dual energy X-ray absorptiometry scans.

PURPOSE: Scoliosis is a three-dimensional spinal deformity that, if left untreated, can progress to painful disability and require invasive surgical correction. In this work, we propose an algorithm to automate the established manual DXA Scoliosis Method (DSM) for measuring scoliosis in adults, that simultaneously quantifies curve angle, direction and apex location. METHOD: The automation pipeline comprises two stages: (i) segmentation of the spine and computation of its spine curve, and (ii) geometric analysis of that curve to identify the apex and derive the maximum modified-Ferguson angle

A multiscale modeling approach to study the role of mechanics and inflammation in the pathophysiology of articular cartilage.

Mechanical loading regulates chondrocyte health in articular cartilage. While physiological stimuli maintain homeostasis, supra-physiological stimuli from joint injuries disrupt it, leading to osteoarthritis (OA). OA progression involves complex mechanical and biochemical interactions across multiple length scales, which are challenging to investigate experimentally. In silico models provide an effective framework to explore these mechanisms. This study developed an integrated multiscale modeling framework for articular cartilage. It combined finite element (FE) models at tissue and cellular s

Surface-based stress tomography of architected metamaterials via physics-constrained generative learning.

Internal stress fields govern failure and performance in architected metamaterials, yet volumetric stress evaluation typically requires volumetric imaging or repeated finite element (FE) simulations. Such requirements limit rapid iteration and practical non-destructive assessment, particularly for bicontinuous spinodoid architectures whose mechanical behavior is strongly geometry-dependent. Here, we present a physics- and topology-constrained generative framework that reconstructs internal 3D stress fields directly from surface observations alone. By leveraging surface-derived stress represent

Association of apolipoprotein A2 isoform index with demographic, behavioral, and clinical risk factors for pancreatic ductal adenocarcinoma in a single-center observational study: considerations for development of a pancreatic cancer screening algorithm?

BACKGROUND: Pancreatic ductal adenocarcinoma (PDAC) is associated with high mortality rates therefore early diagnosis and treatment is essential in reducing mortality. Blood tests that can identify high-risk individuals is an unmet medical need. In this pilot observational study, remnant samples from routine clinical lab orders were tested on 265 patients presenting with abdominal pain and/or presence of pancreatitis, pancreatic cyst, gall stones, abdominal infections (due to herpes or H. pylori), hepatic cirrhosis/hepatitis, non-pancreatic cancer, or pre-, new onset, controlled and uncontroll

Case Report: Sustained immune and pulmonary recovery three years after hematopoietic stem cell transplantation for ITCH E3 ubiquitin ligase deficiency.

BACKGROUND: Itchy E3 ubiquitin ligase deficiency (ITCH deficiency) is a rare monogenic immune dysregulation syndrome characterized by early-onset multisystem autoimmunity and significant morbidity and mortality. Only one prior case report has described a patient treated with hematopoietic stem cell transplantation (HSCT) with clinical improvement, and no established disease-modifying or curative therapy algorithm exists. CASE DESCRIPTION: We describe a female patient who presented in early childhood with chronic lung disease requiring tracheostomy and long-term mechanical ventilation and late

Classification and discrimination of emotion dysregulation disorders using machine learning.

Attention-deficit/hyperactivity (ADHD), bipolar (BD) and borderline personality (BPD) disorders are severe psychiatric illnesses often presenting with overlapping emotion dysregulation symptoms. To date, it is unknown whether these disorders share a continuum on emotion dysregulation. To address this, we employed machine learning (ML) algorithms on psychological and environmental features from 232 adults to predict and discriminate these disorders. We recruited 92 patients diagnosed with either ADHD, BD, or BPD. Considering heritability, we recruited 67 genetically unrelated ADHD, BD and BPD o

A hierarchical reinforcement learning and predictive control framework for automated multi-variable anesthesia.

BACKGROUND AND OBJECTIVE: Automated multi-variable anesthesia must balance conflicting objectives amid patient variability, safety constraints, and partial observability. The state-of-the-art robust approach uses a Genetic Algorithm (GA) to optimize Generalized Predictive Control (GPC) parameters across a population, but the resulting fixed-parameter controller is conservative and slow to adapt. Here, we develop an adaptive drug-administration framework that maintains performance while adapting to patient variability and remaining robust to surgical stimulation and clinician intervention.
